Stenózy hrtanu a průdušnice tvoří významnou součást spektra diagnóz pacientů léčených na klinikách ORL, chirurgie hlavy a krku. Léčba akutních příznaků má jasné řešení v intubaci, či chirurgickém zpřístupnění dýchacích cest, zejména tracheotomií. Pro dlouhodobou léčbu chronických stenózujících procesů však zatím neexistuje spolehlivé, dostatečné a přitom ekonomicky dostupné řešenÍ. Náročné chirurgické rekonstrukce spolu s dilatacemi, laserovými zákroky, nebo vyčkávací taktikou dlouhodobé tracheostomie jsou nejčastěji užívanými léčebnými modalitami. Mnozí autoři referují o vynikajících výsledcích operací (62,69,72,103,1 75). Zásahy do citlivých struktur však mohou mit za následek nevratné poškození zahrnující jizvení, restenózy i poruchy hlasového aparátu vedoucí k poruše vývoje řeči a tím k narušení sociální adaptace. Dosavadní možnosti léčby stenóz dýchacích cest u dětí nelze považovat za dostatečné. Ke všem pacientům je třeba přistupovat individuálně jak z hlediska věku, tak pro častou přítomnost přidružených onemocnění, a proto v této problematice nelze vytvořit jednoznačně platné "doporučené postupy". Použití stentů se široce uplatňuje při léčbě stenotických onemocnění dýchacích cest dospělých jako primární modalita (16,101), i jako podpora zajišťující dostatečný průsvit lumina po dobu hojení po... In treating chronic stenosis and strictures, an unequivocal, generally -accepted approach has not been established. The predominant method of treatment is surgical. Other methods applied include endoscopic (laservaporation, aryteooidectomy) and extemal (expansive, resection) procedures. 10 the pediatric population, the desired method is one which has a minimal1y harmful impact to the growing child. For these reasons, indications to exteosive surgical treatments are limited. Dilatation of stenosis and stricture, and temporary maintaining of lumen patency using stents individual1y measured "to size" seem to afford greater advantages for use in the pediatric population. In cooperation with the Institute of Macromolecular Chemistry (Academy of Sciences, Czech Republic), we have worked on the development of stents suitable for clinical use that exhibit a combination of useful properties (rigidity, absorbability, biocompatibility, microbial inactivity).
