In this thesis, detailed investigation into the electrical field of the heart is presented in patients with various manifestations of myocardial ischemia. Methods of standard scalar electrocardiography and body surface potential mapping were used through statistical and mathematical analysis in order to compare their diagnostic performance. Body surface mapping proved to be more powerful research and diagnostic tool than standard electrocardiography and also constituted a framework for data entering the computer electrical model of the human heart and torso. Global and local ischemic changes were the primary objectives of the individual studies of this thesis. Different patient populations were studied including post myocardial infarction population, patients with variant angina and patients referred for percutaneous coronary angioplasty, and the population of patients suffering from ventricular arrhythmias related mostly to their postinfarction status. Traditional depolarization and repolarization changes were studied together with novel diagnostic parameters such as minimal potential loss within the QRS complex, single-beat cardiac micropotentials and distribution of the QT interval on the human thorax. Identifikace globálních a lokálních ischemických změn elektrického pole srdce zahrnuje celé spektrum přístupů k analýze EKG signálu od kvalitativních a empirických visuálních hodnocení až po vyspělé matematické metody. V laboratorních podmínkách se potvrdila významná úloha BSPM, které umožňuje komplexnější a přesnější hodnocení ischemických, ale i neischemických změn depolarizace a repolarizace než křivky skalárního EKG. Ověřili jsme diagnostickou hodnotu tradičních EKG parametrů (kmit Q, repolarizační negativní potenciály, elevace ST), ale i nových kriterií v BSPM (časový a prostorový rozklad QRS, mikropotenciály z jedné systoly, chyba měření disperze intervalu QT), které nelze ze standardního EKG odvodit. K zobrazení elektrokardiografických dat ve vztahu k anatomickým strukturám jsme použili open-source softwarové prostředí map3d, které je jedním z možných vědeckých nástrojů na cestě k vývoji elektrokardiografického zobrazování. Také jsme ověřili funkci transformační matice odvozené z jednoho z vyspělých realistických modelů srdce a hrudníku k úspěšné redukci systému svodů pro záznam EKG. Výsledky uvedené v této tezi jsou pro nás podnětem k další výzkumné činnosti na poli elektrokardiografického modelování směřujícího nejen k ověřování fundamentálních teorií elektrokardiografie, ale i k potenciálnímu...
