Tato práce se zaměřuje na výzkum metod pro prvotrimestrální screening vrozených vývojových vad plodu. V literární rešerši jsou popsány ultrazvukové markery aneuploidie, algoritmus prvotrimestrálního screeningu a stručná charakteristika chromozomálních aberací diagnostikovatelných v I. trimestru těhotenství. Nuchální translucence je nejdůležitějším markerem aneuploidie v I. trimestru a práce se dále zabývá jejím měřením podle současně používané metodiky, která byla vytvořena Nadací fetální medicíny (FMF). Cílem je popsat limitace této metodiky a vysvětlit nepřesnosti, které vznikají v důsledku doporučení FMF. Největším nedostatkem této metodiky je vliv parametrů nastavovaných na ultrazvukovém přístroji před a během vlastního měření, proto je třeba vytvořit novou metodiku, která tímto nebude ovlivněna. Na základě znalosti vlivů jednotlivých parametrů je vytvořena nová metodika, která nezávisí na změně nastavení parametrů ultrazvuku. V závěru práce je provedena studie, která srovnává směrodatné odchylky výsledků měření podle obou metodik. This thesis focuses on the research of methods for first trimester screening of congenital defects of the fetus. Ultrasound markers of aneuploidy, the algorithm of the first trimester screening and brief description of chromosomal aberrations diagnosable in the first trimester of pregnancy are described in the literature search. Nuchal translucency is the most important marker of aneuploidy in the first trimester and the work also deals with the measurement methodology currently used, which was created by the Fetal Medicine Foundation (FMF). The aim is to describe the limitations of the methodology and explain inaccuracies that arise due to FMF recommendations. The biggest drawback of the methodology is the influence of parameters set on ultrasound machines before and during the actual measurement, therefore it is necessary to come with new methodology not affected by them. The new methodology is based on the knowledge of the parameter influence and is independent of parameter settings of ultrasound machines. The final part of the thesis includes a study comparing the standard derivation of measurement results in both methods.
