[cze][eng] Nemoci dýchacích cest patří k nejčastějším chorobám. Závažným jevem je posun prvních projevů nemoci do raného dětského věku. Významný podíl v terapii horních dýchacích cest, spodiny lební, slzných cest a orbity představuje funkční endoskopická endonazální chirurgie. Cílem endoskopické endonazální chirurgie je zlepšení kvality života nemocných.Po operaci se mohou přechodně vyskytnout komplikace. Jedná se zejména o masivní krvácení z nosního nitra, do očnice, poranění orbity a zrakového nervu, penetrace do nitrolebí s následnou likvorovou píštělí a event. meningitidou či mozkovým abscesem. Po operaci je pacient informován o režimových opatřeních, které předává lékař se sestrou. Role sestry je spolu s lékařem důkladně podat pacientovi informace o režimových opatřeních. Pochopení a dodržení těchto informací napomáhá ke zlepšení kvality života ze strany pacienta po EES operaci. Tato bakalářská práce je rozdělena na část teoretickou a empirickou. Teoretická část je zaměřena na problematiku endoskopické endonazální chirurgie, na předoperační a pooperační péči o pacienty. Dále pak na potřeby pacientů, kteří tuto operaci podstupují a na režimová opatření po propuštění do domácí péče. Cílem práce bylo: 1. Zjistit nejčastější obavy, které má pacient v souvislosti s operací. 2. Zjistit oblasti nesaturovaných potřeb pacientka hospitalizace v pooperačním období. 3. Zjistit názor operovaných nemocných na poskytovanou ošetřovatelskou péči. K těmto cílům byly stanoveny čtyři výzkumné otázky: 1. Jaké jsou nejčastější obavy pacienta před a po operaci? 2. Jaké mají pacienti nejčastější potřeby v pooperačním období? 3. Jaké potřeby zůstaly neuspokojeny? 4. V čem vidí pacienti největší problém z hlediska ošetřovatelské péče? Ke zpracování bakalářské práce bylo použito kvalitativní výzkumné šetření. Metoda a technika sběru dat byl polostrukturovaný rozhovor. Výzkumné šetření bylo realizováno u respondentů v nemocnici na konci hospitalizace. Rozhovory probíhaly anonymně za ústního souhlasu každého respondenta v časovém období od 26. 2. do 30. 3. 2016. Cílem této bakalářské práce bylo mimo jiné i to, že vznikl ucelený přehled o obtížích pacientů s touto problematikou. Výsledky bakalářské práce by mohly sloužit ke zkvalitnění a zefektivnění ošetřovatelské péče o pacienty před a po endoskopických endonazálních výkonech nebo jako studijní materiál pro začínající sestry otorinolaryngologických pracovišť. Dále je tato bakalářská práce nástinem uvedené problematiky pro ty, kteří se rozhodnou studovat perioperační péči o pacienty.Respiratory diseases belong to the most common medical affections. Their first symptoms are now being shifted to the early infancy. Functional endoscopic endonasal surgery represents an important option in the treatment of upper airways, skull basis,lacrimal ducts and orbital affections. The aim of endoscopic endonasal surgery (EES) is the improvement of patient´s quality of life.There may be some transient postoperative complications. These include especially massive intranasal and intraorbital hemmorhage, optical nerve and orbital injury, intracranial penetration with subsequent liquor fistula or eventually meningitis and cerebral abscess. Postoperatively, patients are informed about about the regime measures by the nurse and doctor. The role of the nurse is, in cooperation with doctor, to deliver all the precise and relevant information to the patient. Full understanding and adequate adherence to these instructions has the potential to improve patient´s quality of life after EES. This bachelor´s dissertation is divided into theoretical and empirical section. Theoretical part focuses on the issues of endoscopic endonasal surgery, including preoperative and postoperative care. Further on, it explores the needs of patients undergoing this type of surgery with special attention to regime measures after hospital discharge to home care. The aim of the study was: 1. To identify the most common concerns that patient experiences in association with surgery. 2. To explore the unsaturated and unfulfilled needs of the patients in postoperative period. 3. To gather and explore patient´s opinions on nursing care. To investigate these aims four research questions were stated: 1. What are the most common preoperative concerns of the patients? 2. What are the patient´s most common needs in the postoperative period? 3. What are the patient´s unfulfilled needs? 4. What are the major problems in nursing care from patient´s point of view? Qualitative research investigation was used in this dissertation work. Semistructured interview was chosen as a method for data collection. The research investigation was conducted with hospital respondents at the end of hospital stay. All the interviews were anonymous, preceded by oral consent of the respondent, and were conducted in time period from 26. 2. to 30. 3. 2016. In conclusion, this dissertation project explored the basic issues of EES. The aim of this work was to create a basic data source of the patient´s problems and issues after ESS. The results of this bachelor´s dissertation could be used to improve and make nursing care of ESS Patients more effective. It may also serve as a study material for novice nurses of ear-nose-throat clinics and also be a guide for further researchers.
