[cze][eng] Současná doba je charakterizována tím, že dochází k velkým vědeckým a technickým pokrokům nejen v medicíně, ale i v oboru ošetřovatelství, které nyní vychází mnohem více z vědeckého poznání, než tomu bylo dříve. Ošetřovatelství jako vědní disciplína vyžaduje odborně vzdělané sestry, jež budou zajišťovat kvalitní ošetřovatelskou péči. Proto i vývoj moderní medicíny zvláště v odvětví kardiologie potvrzuje nutnost změnit chápání a postavení sestry v péči o pacienta a současně posílit prestiž a postavení sestry ve společnosti. Hlavním výzkumným záměrem disertační práce bylo zjistit, jak je vnímáno pečovatelské chování sestrami a pacienty u radiofrekvenční katétrové ablace (RFA) pro fibrilaci síní (FS), jaký vliv má RFA na kvalitu života pacientů s FS a jaká je úroveň edukace u pacientů s FS. Pro jeho zodpovězení byly stanoveny adekvátní cíle práce. K naplnění cílů byl v empirické části prováděný výzkum rozdělen do dvou fází. V první fázi byl použit nestandardizovaný dotazník, který měl za úkol posoudit potřeby pacientů s FS před a po RFA s odstupem 1 roku. Ve druhé fázi, která zkoumala pouze stav po 1 roce od provedení RFA, byl ještě navíc použit standardizovaný dotazník CBI - 24 a společně s ním byl rozdán pacientům další formulář s otázkami, který zjišťoval obsah a formu edukace pacientů. Dále byla data získána od sester, které pacienty v souvislosti s RFA ošetřují. Výsledky provedeného výzkumu ukazují, že kvalita života u pacientů s FS je před ablací snížena. Problémovými oblastmi pacientů jsou bolest, fyzické a psychické potíže. Zároveň je z našeho výzkumného šetření dále patrné, že při porovnání oblastí uspokojování fyzických a psychických potřeb pacientů je pacienty lépe hodnoceno uspokojování potřeb fyzických. Další posuzovanou oblastí byla edukace pacientů. Z vyhodnocení získaných výsledků můžeme poukázat na skutečnost, že bylo lépe posuzováno spíše obsahové zaměření edukace, než volba její formy. Poslední částí výzkumu bylo zhodnocení vnímání poskytované ošetřovatelské péče z pohledu pacientů a sester. Z výsledků šetření je patrné, že sestrami je lépe hodnocena technická zdatnost než humanistický přístup k pacientům. Zároveň bylo zjištěno, že pacienti lépe hodnotili u sester oblasti zaměřené na výkon, než vytvoření vztahu jistoty a bezpečí pro pacienty. Jako problematická se dle výsledků jevila také oblast komunikace. Stanovené cíle práce se nám podařilo naplnit a získat jak teoretická, tak i praktická doporučení. Analýzou výsledků jsme zjistili, že je potřeba podporovat humanistický přístup v ošetřovatelské péči o kardiologické pacienty, komunikaci a edukaci.The current period is characterized by profound scientific and technological progress not only in the medicine but also in the field of nursing, which is now based more on scientific knowledge than ever before. Nursing as a science requires professionally trained nurses providing high quality nursing care. Therefore, the development of modern medicine, particularly in the field of cardiology, justifies the need to change the perception and status of nurses with respect to patient care together with strengthening the prestige and status of nurses in society. The main research intention of the dissertation was to determine how the nursing behaviour in connection with the procedure of radiofrequency catheter ablation (RFA) due to atrial fibrillation (AF) is perceived by nurses and patients, what is the impact of the RFA on the life quality of patients with AF, and what is the level of education in patients with AF. To meet these goals the research, the empirical part was divided into two phases. In the first phase, a non-standardized questionnaire was used, role of which was to assess the needs of patients with AF before and after RFA comparing baseline and 1 year follow-up data. In the second phase, which only examined the conditions 1 year after the RFA, a standardized questionnaire CBI - 24 (Caring Behaviour Inventory) was additionally used together with another form with questions investigating education of patients. Further data were obtained from nurses taking care for patients during the medical intervention. The research results show that quality of life of patients with AF before ablation was reduced. Patients´ most common problem areas included pain, physical and mental problems. Our research also showed that if we compared meeting patients´ physical and mental needs, meeting physical needs was evaluated better. Another area under consideration was the education of patients. Evaluated results showed the fact that education was focused on its content rather than its form. Evaluation of the perception of nursing care from the perspective of patients and nurses was the last part of the research. The results of the survey showed that nurses evaluated technical competence better than the humanistic approach to patients. It was also discovered that patients evaluated areas focused on performance of nurses better than creating a relationship of security and safety for patients. Finally, the results indicated that communication is also problematic area. We managed to meet the set goals and gain both theoretical and practical recommendations. Analyzing the results, we found out that there is a need to support humanistic approach in nursing care for cardiac patients, communication and education.
