Preimplantační analýza umožňuje genetické vyšetření embryí vzniklých procesem in vitro fertilizace, a to před jejich transferem do dělohy. V rámci preimplantačního vyšetření lze provádět jak screening náhodných aneuploidií (PGS), tak cílenou diagnostiku chromosomových aberací či monogenních onemocnění (PGD), popř. jejich kombinaci (PGD/PGS). Preimplantační vyšetření jsou tedy vhodná nejen pro páry s obecnými reprodukčními problémy (např. vyšší maternální věk), ale také pro páry s genetickou zátěží v rodinné anamnéze. Metoda aCGH umožňuje v jednom experimentu rychlou a přesnou analýzu nebalancovaných chromosomových aberací v celém embryonálním genomu, tedy spojení screeningu náhodných aneuploidií (PGS) se současnou detekcí případné nebalancované formy familiární aberace (PGS/PGD). V genetické laboratoři IVF Zentren Prof. Zech jsme metodou aCGH vyšetřili za rok 2014 celkem 469 embryí od 98 klientek. Biopsie embryií byly prováděny ve stádiu blastocysty, kdy bylo šetrně odebráno několik buněk trofoektodermu. U všech embryonálních vzorků byla provedena celogenomová amplifikace (WGA) a pro vyšetření aCGH byla použita platforma 24sure BlueGnome. Preimplantační screening (PGS) byl proveden celkem u 366 embryí a kombinované vyšetření (PGD/PGS) celkem u 103 embryí. Průměrný věk našich klientek v době vyšetření byl 40,5 roku. Z výsledků vyšetření vyplývá, že se zvyšujícím se maternálním věkem u embryí významně stoupá četnost sporadických aneuploidií. Provádění preimplantačního screeningu je tedy zejména vhodné u klientek s vyšším maternálním věkem. Také u klientek, které podstoupily kombinované vyšetření z důvodu genetické zátěže v rodině (PGD/PGS) a klientek, u kterých byly indikovány cykly s dárcovskými gametami, však screeningové vyšetření může zvýšit úspěšnost IVF cyklu a šanci na donošení zdravého dítěte. Preimplantation genetic analysis allows testing embryos produced by in vitro fertilisation before their transfer into the uterus. Preimplantation genetic analysis can be performed as screening of random aneuploidies (PGS) or targeted diagnosis of familial chromosomal aberrations or monogenic diseases (PGD) or combination of both approaches (PGD/PGS). Preimplantation genetic testing is appropriate for wide spectre of patients undregoing human assisted reproduction treatment. Array comparative genomic hybridization (aCGH) is sensitive and high throughput method for detection of chromosomal copy number changes on whole embryonal genome, therefore allows performing screening of random aneuploidies (PGS) in combination with diagnosis of unbalanced chromosomal familial aberrations (PGD/PGS). The data for this study were obtained from Genetic Laboratory IVF Zentren Prof. Zech in Pilsen during 2014. The results of 469 examined embryonal samples resulting from 98 clients were analysed. All biopsies of trophectoderm cells were performed on blastocyst stage. All embryonal samples underwent whole genome amplification (WGA) and were processed using 24sure microarrays (BlueGnome). PGS was performed for 366 embryos in total and combined analysis including PGS and PGD was performed for 103 embryos. An average maternal age at the time of analysis was 40,5. This study demonstrates that there is significantly higher rate of aneuploidy in patients of higher maternal age. A suitability of PGS for older patients was confirmed. In other groups of patients containing IVF cycles with donated oocytes or patients carrying familial chromosomal aberrations a higher profit of fertility treatment was observed.
