[cze][eng] Tato bakalářská práce má název Ošetřovatelská péče po operacích páteře. Pacientů s potížemi různých oblastí páteře velmi přibývá a není divu, že i indikací k operačnímu řešení. Je to dáno trendem společnosti a vyšší zátěží na organismus. Tím přibývá i operačních postupů, které si žádají specifické ošetřovatelské postupy při ošetřování v časném pooperačním období. Práce poukazuje na problematiku specifické ošetřovatelské péče o pacienty různých onemocnění a úseků páteře. Poukazuje na to, že neexistuje univerzální měřítko postupu ošetřovatelské péče po různém typu operace na stejném úseku páteře a správnost postupu specifické ošetřovatelské péče v souladu s následnou rehabilitační péčí je nezbytná. Lidí s onemocněním páteře indikovaných k operaci nadále přibývá, výzkum udává čísla dnešní doby proti době před dvaceti lety, troufáme si říci, až katastrofální. Není se čemu divit. Nepřiměřené zátěže neubývá, také sedavým způsobem života v zaměstnání nebo doma jen připočítává k riziku vzniku těchto onemocnění. Ty mají za následek dyskomfort hlavně v osobním životě člověka, ale je tím ohrožena i jeho pracovně-sociální role a narušení ekonomických jistot, bez kterých se v dnešní době neobejdeme. Ve své práci se zabýváme už vzniklými patologickými projevy indikovaných k operaci. Jednoduše řečeno tím, co se s pacientem děje, je-li naším ambulantním lékařem-neurochirurgem doporučen k operačnímu řešení. Nedostatek ošetřovatelské literatury ohledně této specifické problematiky byl pro nás důvodem výběru tohoto tématu bakalářské práce. V teoretické části rozebereme základní skladbu - anatomii a fyziologii páteře, základní termíny vzhledem k páteři- základní pohyby, zakřivení páteře. Vzhledem k tomu, že je naše práce zaměřena na ošetřovatelskou péči po operačním řešení onemocnění různých úseků páteře na neurochirurgii, traumatologii a infekčním oddělení v Českých Budějovicích, teoretická část patří i základnímu teoretickému nastínění nejčastěji prováděných operačních výkonů na těchto odděleních Nemocnice v Českých Budějovicích, a.s. Vlastní výzkumnou částí řešíme specifika ošetřovatelské péče po zmíněných operačních výkonech, které souvisí s časnou rehabilitační péčí a úzké spolupráci s fyzioterapeuty. Je třeba zdůraznit, že týmová spolupráce je základním kamenem v péči o pacienta po operaci páteře, jehož následná rehabilitační péče a edukace v péči o sebe sama po propuštění z ústavního ošetřování je jedním ze zásadních předpokladů pro úspěšnou rekonvalescenci a navrácení se do své pracovně sociální role. Výzkumná část je zaměřena na zjištění těchto cílů: C1: Zjistit, zda sestry znají správné postupy časné rehabilitační péče o pacienty po operacích páteře C2: Zjistit, jaká je úroveň spolupráce mezi sestrami a fyzioterapeuty v oblasti specifik o pacienty po operacích páteře C3: Zjistit, zda vzdělání a doba praxe sester má vliv na poskytování specifické ošetřovatelské péče o pacienty po operacích páteře Dále pomocí výzkumných otázek týkající se této problematiky: V1: Jakým způsobem sestry a fyzioterapeuti spolupracují při péči o pacienta po operaci páteře? V2: Souvisí doba praxe sester s vyššími znalostmi v oblasti poskytování specifické ošetřovatelské péče pacientovi po operaci páteře? V3: Má vzdělání sester vliv na znalosti v oblasti specifik ošetřovatelské péče o pacienta po operaci páteře? V4: Jakým způsobem sestry prohlubují své znalosti v oblasti specifik ošetřovatelské péče o pacienty po operacích páteře? Výzkumným souborem jsou 4 sestry z Neurochirurgie 4 sestry z Traumatologie a 4 sestry z Infekčního oddělení v Českých Budějovicích, a.s.. Základním prvkem našeho výzkumného procesu je polostrukturovaný rozhovor, kterým si budeme ověřovat své očekávané cíle a výchozí výsledky. The title of my thesis is Nursing care after spinal surgery. The number of patients with problems of different parts of the spine is growing as well as indications to operational solutions. This is due to society trends and a greater load on the body. Therefore operational procedures requiring specific nursing procedures for treatment in the early post-operative period increase. The thesis highlights the issue of specific nursing care for patients with various diseases and parts of spine. It points out that there is no universal measurement of nursing care for different types of operations on the same section of the spine and it is necessary to choose the right procedure in specific nursing care along with subsequent rehabilitation care. The number of people with diseases of the spine indicated for surgery continues to increase. Research mentions modern days numbers compared with the time 20 years ago and I dare say the numbers are disastrous. There is nothing to wonder about. Disproportionate load does not decrease, a sedentary way of life either at work or at home also adds to the risk of developing these diseases. These have resulted in discomfort mainly in personal life of a human, but also threaten their working and social role and disrupt the economic security, which we cannot do without nowadays. In our thesis we deal with already incurred pathological manifestations indicated for a surgery. In other words, what happens to the patient, if they are recommended by our outpatient physician - neurosurgeon for a surgery. The lack of nursing literature on this specific issue was the reason for us to choose this theme. In the theoretical part we will discuss the general structure - spine anatomy and physiology, basic terms related to the basic movements, and curvature of the spine. Since our thesis is focused on nursing care after a surgical treatment of diseases of various sections of the spine at the department of Neurosurgery, Traumatology and the Infectious department in České Budějovice, the theoretical part includes basic theoretical outlines of the most important surgical procedures performed at these departments in the hospital of České Budějovice a.s.. Furthermore, we will get to the specifics of nursing care after these surgeries related to early rehabilitation care and close cooperation with physiotherapists. It should be emphasized that teamwork is the cornerstone in the care of patients after spinal surgery, whose subsequent rehabilitation and education in self-care after being discharged from the hospital care is one of the basic prerequisites for a successful recovery and return to their working and social role. The research part focuses on the following objectives: Aim 1: To determine whether nurses know the correct procedures for early rehabilitation of patients after spinal surgery Aim 2: Find out what is the level of collaboration between nurses and physiotherapists in the specifics of patients after spinal surgery Aim 3: To determine whether the nurse´s education and length of practice has an impact on the provision of specific nursing care for patients after spinal surgery Furthermore, using the research questions related to this issue: Research 1: How do nurses and physiotherapists cooperate in a patient´s care after spinal surgery? Research 2: Is a nurse´s length of practise connected with higher expertise in providing specific nursing care to the patient after a spinal surgery? Research 3: Does the education of nurses affect the knowledge of the specifics of the nursing care to the pacients after a spinal surgery? Research 4: How do nurses extend their knowledge in specifics nursing care for pacients after spinal surgery?
