[cze][eng] Tématem mé bakalářské práce je ovlivnění stresové inkontinence u žen, a to pomocí různých fyzioterapeutických metod. Práce se skládá z teoretické a praktické části. V teoretické části jsou zpracovány poznatky z fyziologie a anatomie močového ústrojí a pánevního dna. Dále je zde uvedena teorie inkontinence, diagnostika a léčba stresové inkontinence. Stresová inkontinence je stav, při kterém není možné vědomě regulovat odchod moči za zvýšeného nitrobřišního tlaku. Patří mezi jedny z nejčastějších ženských obtíží. Jedná se o multifaktoriální problém, neboť ženám působí fyzické, psychické, sociální, tak i ekonomické problémy. Stresová inkontinence je příznakem, který může mít několik příčin. Za normálních okolností reaguje močová trubice na zvýšení nitrobřišního tlaku zvýšením svého tlaku, jímž je uzavřena. Moč v tomto případě nemůže z močového měchýře uniknout ven. Pokud však močová trubice není ve své fyziologické poloze, což může být způsobeno dysfunkcí svalů dna pánevního, moč nekontrolovatelně uniká. Svaly dna pánevního jsou neodmyslitelnou součástí hlubokého stabilizačního systému. Jsou důležitými stabilizátory celého trupu a společně s bránicí se podílejí na správném dýchání. Stresová inkontinence se vyskytuje ve větší míře u žen vyššího věku, není ale výjimkou, že jí trpí i ženy mnohem mladší. Trpí jí až jedna pětina žen. Problém stresové inkontinence se však nevyhýbá ani mužům. Stresová inkontinence se dělí do tří stupňů dle závažnosti. Mezi rizikové faktory patří například věk, obezita, těhotenství, obstipace, kouření a další. Důležitá je vždy správná diagnostika inkontinence a zvolení vhodné léčby. Stresovou inkontinenci lze léčit pomocí konzervativních i chirurgických metod. Nejčastěji je inkontinence řešena pouze pasivními metodami, užíváním léků a používáním inkontinenčních pomůcek. V nejtěžších případech operativním zákrokem. Ženy mají ale možnost řešit inkontinenci využitím fyzioterapie. Touto léčbou je možné příznaky inkontinence plně odstranit nebo je alespoň zmírnit na únosnou mez. Cvičením lze inkontinenci předcházet. Cílem fyzioterapie je poučit ženy o fungování močových cest a především svalstva pánevního dna. Pomoci jim s tréninkem aktivace a relaxace svalů pánevního dna a jejich zapojení při dýchání společně s bránicí. Důležité je zaměření se na psychiku ženy. Snažíme se o podpoření správného sebevědomí žen. Na ženu musí být nahlíženo komplexně a individuálně. Praktická část bakalářské práce byla zpracována formou kvalitativního výzkumu. Do výzkumu byly zapojeny dvě pacientky, které trpí stresovou inkontinencí. Byl vypracován vstupní kineziologický rozbor, na jehož základě byla vypracována individuální cvičební jednotka pro každou z pacientek. Závěrem byl vypracován výstupní kineziologický rozbor. Využita byla zejména posturální terapie, terapie plosky nohy, techniky dle Čápové, terapie dle Ludmily Mojžíšové a další. Takto zvolená terapie byla úspěšná a byla pacientkami subjektivně hodnocena pozitivně. Dle pacientek mělo cvičení vliv nejen na problémy spojené s inkontinencí, ale i na zlepšení jejich celkové kondice, jak fyzické, tak psychické. The topic of my bachelor's thesis is affecting the women stress incontinence using different physiotherapeutic methods. The thesis consists of a theoretical part and a practical part. The theoretical part covers the problematic of physiology and anatomy of the urinary system and pelvic floor. It also explains the theory of incontinence, diagnosis and treatment of the stress incontinence. Stress incontinence is a state in which one cannot intentionally regulate the urine drain using increased intrapelvic pressure. It is one of the most common women diseases. It is a multifactorial problem, because it causes not only physical, but also mental, social and even economic problems. Stress incontinence is a symptom which can have several causes. Under normal circumstances, the urethra reacts on increase of the intrapelvic pressure by increasing its own pressure and thus it is closed. Therefore it cannot leak the urine. If, however, the urethra is not in its physiologic place, which can be caused by a pelvic floor muscle malfunction, the urine is being leaked uncontrollably. The pelvic floor muscles are crucial part of the core stabilization system. They are very important stabilizers of the whole torso and together with diaphragm they ensure the proper breathing. The stress incontinence can be treated also using conservative and surgical methods. In most cases, the incontinence is treated using only passive methods, by using medicines and applying incontinence tools. In the worst cases, they are treated by a surgery. Women, however, have a possibility to treat the incontinence by physiotherapy. Using this treatment, the symptoms of incontinence can be eliminated or at least reduced to a manageable level. The incontinence can be prevented by doing particular exercises. The goal of the physiotherapy is to teach the women how the urinal system and mainly the pelvic floor work. To help them with the training of the activation and relaxation of the pelvic floor muscles and how to include them during the breathing together with the diaphragm. It is important to focus on the woman mentality. We are trying to support the proper self confidence of the women. The women have to be treated comprehensively and individually. The practical part of the thesis was done using qualitative research. Two patients suffering from stress incontinence were treated during the research. Based on the entrance kinesiologic analysis, an individual exercise plan was created for each of the patients. In the end, the output kinesiologic analysis was done. Mainly, the postural therapy, the flat foot therapy, techniques by Capova, therapy by Ludmila Mojzisova, etc. were used. It is clear from the results that the used therapy was successful and it was rated very well by the patients. According to the patients, the exercises had influence not only on their incontinence problems, but it also helped to improve their overall condition, both physical and mental.
