[cze][eng] Teoretická část práce se zabývá objasněním základní problematiky onemocnění,vyšetřovacími metodami v kardiologii, prevencí kardiovaskulárních onemocnění, rizikovými faktory a rolí sestry. Praktická část obsahuje zpracovaná a vyhodnocená data, získaná během výzkumného šetření.Předmětem práce bylo zmapovat spolupráci mezi sestrou a pacientem s kardiovaskulárním onemocněním v rámci poskytované sekundární prevence. Pro naplnění výzkumné části bakalářské práce jsme stanovili čtyři hlavní cíle: 1. Zmapovat úlohu sestry v sekundární prevenci u pacienta s kardiovaskulárním onemocněním. 2. Zmapovat spolupráci ve vztahu sestra-pacient v rámci sekundární prevence kardiovaskulárních onemocnění. 3. Zjistit očekávání pacientů od sester v oblasti sekundární prevence na lůžkovém oddělení. 4. Zjistit očekávání pacientů od sester v oblasti sekundární prevence v ambulantní péči. V návaznosti na tyto cíle jsme si položili následující výzkumné otázky: 1. Jsou pacienti srozumění s komplikacemi, které mohou nastat při nedodržování preventivních opatření? 2. Jaký je postoj pacientů k získávání informací v oblasti preventivních opatření? 3. Jaká je úloha sestry v sekundární prevenci u pacienta s kardiovaskulárním onemocněním? 4. Jak spolupracují sestry a pacienti v rámci sekundární prevence v kardiologii? 5. Jaká očekávání mají pacienti v oblasti sekundární prevence na lůžkovém oddělení od sester? 6. Jaká očekávání mají pacienti v oblasti sekundární prevence v ambulantní péči od sester? Na dané výzkumné otázky lze v praktické části nalézt relevantní odpovědi. Pro empirickou část této bakalářské práce bylo využito kvalitativní šetření. Sběr dat byl realizován pomocí polostrukturovaných rozhovorů prováděných v nemocnici Tábor a.s., a Českých Budějovicích, a.s. na standardní a ambulantní jednotce kardiologie. Rozhovory se sestrami a pacienty byly uspořádány do několika částí. První část obsahuje identifikační údaje, druhá se týká úlohy sestry v sekundární prevenci u pacienta s kardiovaskulárním onemocněním, dále vzájemné spolupráce mezi sestrou a pacientem a poslední část je označena jako očekávání v rámci poskytované sekundární prevence. Celkem se zúčastnilo 6 sester a 6 pacientů. Získaná data byla pro větší přehlednost zpracována do myšlenkových map. Při interpretaci získaných dat byla použita i přímá citace respondentů bez lingvistické úpravy. Z výsledků rozhovorů zkoumaných skupin vyplývá, že spolupráce a vzájemná komunikace mezi sestrou a pacientem je oblastí, v které je co zlepšovat. Z výzkumu celkově vyplývá, že vzájemná a dobrá spolupráce se významně podílí na léčbě nemocného. Pacientům je poskytováno dostatečné množství informací. Nejčastějším zprostředkovatelem informací bývá lékař. Informace jsou nejčastěji sdělovány ústní formou nebo prostřednictvím brožur a letáčků. Z výzkumného šetření vyplynulo, že pacienti jsou dobře orientováni v dané problematice. Mají přehled o režimových opatření, lécích a komplikacích, které mohou nastat při nedodržování doporučených rad. U pacientů s kardiovaskulárním onemocněním je nevyhnutelná změna životního stylu,která jak vyplývá z výzkumného šetření, činí pacientům největší problém. Řada pacientů nedodržuje důležité zásady a ohrožuje tak své zdraví i život. V praxi může tato práce sloužit ke zvýšení informovanosti v oblasti zdravotního rizika u kardiologicky nemocných v rámci sekundární prevence. Zároveň by výsledky mohly posloužit studentům oboru ošetřovatelství jako doplněk k výuce. Výsledkem práce bylo vypracování nejdůležitějších zásad, které by měl pacient dodržovat.The theoretical part deals with the basic issues clarification of the disease, examination methods in cardiology, prevention of cardiovascular diseases, risky factors and the nurse´s part. The practical part contains processed and evaluated data, gained during the research. The subject matter of the thesis was to map the cooperation between the nurse and the patient with cardiovascular disease within the bounds of providing secondary prevention. To fulfil the research part of the bachelor´s thesis we have set four main aims: 1. To map the nurse´s part in the secondary prevention by a patient with cardiovascular disease. 2. To map the cooperation in the relationship a nurse-a patient within the bounds of secondary prevention of cardiovascular diseases. 3. To find out patients´ expectations from nurses in the field of secondary prevention at the inpatient ward. 4. To find out patients´ expectations from nurses in the field of secondary prevention in the outpatient care. In connection with these aims we have given the following research questions: 1. Do the patiens know about possible complications which may appear if they do not follow prevention measures? 2. What is the patients´attitude toward gaining information in the field of prevention measures? 3. What is the nurse´s part in the secondary prevention by a patient with a cardiovascular disease? 4. How do nurses and patiens cooperate within the bounds of secondaary prevention in cardiology? 5. What expectations do the patiens have in the field of secondary prevention at the inpatient ward? 6. What expectations do the patiens have in the field of secondary prevention in the outpatient care from nurses? You may find relevant answers to the research given questions in the practical part. A qualitative research has been used for the empiric part of this bachelor´s thesis. Data collection has been realised by means of half-structured dialogue carried out in the hospital of Tabor a.s. and České Budějovice a.s., at the standard and premium service wards of cardiology. Dialogues with nurses and patiens have been arranged into several parts. The first part contains indentification data, the second concerns the nurse´s part in the secondary prevention by a patient with cardiovascular diseas, furthermore mutual cooperation between the nurse and athe patient and the last part is marked as expectations within the bounds of providing secondary prevention. Alltogother 6 general nurses and 6 patients took part. The data gained was processed into charts of thoughts. While interpreting the gained data respondents´ direct quotation was used without linguistic modification. From the dialogues results of the studied groups it is obvious that cooperation and mutual communication between the patient and the nurse is a field where improvements are needed. On the whole, it is obvious, from the research,that mutual and good cooperation takes part in the patient´s treatment. Patients are provided sufficient number of information. The most frequent agent of information is the doctor. Informtion is passed most often orally or by means of brochures and leaflets. The research shows that patiens are well informed in the given problematic. They have knowledge about regime measures, cures and complications which may arrive when not observing recommended advice. The change of lifestyle is inevitable by patiens with cardiovascular diseas. However, this is the biggest problem for the patiens according to the research. Many patiens do not follow important principles and therefore they endanger their health and life. In practice, this thesis may be used to improve awarness in the field of health risk by patiens of cardiology within the bounds of secondary prevention. At the same time the results could help the students of health care as a complement to teaching. The result of the thesis was working out the most important principles which the patient should follow.
