[cze][eng] Demence, také nazývaná jako nemoc stáří, se přestává týkat pouze vyspělých zemí, je dnes problémem globálním. Populace v České republice stárne, je to způsobeno snižující se porodností a úmrtností ve všech věkových skupinách. Demence patří mezi častější onemocnění seniorů než cévní mozková příhoda a diabetes mellitus. U lidí ve věku 65 let je riziko rozvoje stařecké demence 5 %, ve věku 75 let stoupá na 10 %. Ve věku nad 85 let trpí onemocněním 30-50 % seniorů (Jirák, Holmerová, a kol., 2009). Většina sester se na svých odděleních s klienty s demencí setká, a proto by měly být vzdělané v oblasti specifických potřeb při ošetřování těchto klientů.Cílem práce bylo 1. Zjistit specifické problémy ošetřovatelské péče u klienta se stařeckou demencí. 2. Zmapovat možnosti vzdělávání sester ve zdravotnických zařízeních v problematice klienta se stařeckou demencí. 3. Zjistit negativní faktory působící na psychiku sester při ošetřovatelské péči o klienta se stařeckou demencí. K těmto cílům byly stanoveny čtyři výzkumné otázky. 1. Jaké nejčastější problémy řeší pracovníci ve zdravotnictví při ošetřovatelské péči s klienty se stařeckou demencí? 2. Jakým způsobem komunikuje sestra s klientem se stařeckou demencí? 3. Jaké jsou možnosti vzdělávání sester v problematice stařecké demence? 4. Jaké jsou negativní dopady na psychiku u pracovníků ve zdravotnictví, kteří pracují s klienty se stařeckou demencí?Bakalářská práce na téma problematika ošetřovatelské péče u klienta se stařeckou demencí, byla zpracována formou kvalitativního šetření, metodou dotazování. Technikou sběru dat byl polostrukturovaný rozhovor a byl sestaven pro sestry z různých standardních oddělení nemocnice Strakonice. Rozhovory tvořilo sedmnáct otázek a se souhlasem sester byly natáčeny na diktafon a následně doslova přepsány. Výzkumné šetření probíhalo od 22. 2. 2013 do 1. 3. 2013.Z výzkumného šetření vyplynulo, že všechny sestry se často setkávají s klienty s onemocněním demence a znají specifické problémy u ošetřování těchto klientů. Z výsledků také vyplývá, že mají přehled o různých formách vzdělávání v problematice stařecké demence, přesto se nevzdělávají a považují ostatní sestry za neinformované. Při výzkumném šetření jsme také zjistili, že na odděleních žádné formy vzdělávání v této oblasti nemají. I přes nedostatek informací, se však sestry snaží problematické situace s klientem řešit dle svých zkušeností, nebo sbíráním informací od ostatních zdravotníků, což bývá obvykle nedostačující. Díky těmto souvislostem, dochází ke zhoršené komunikaci mezi sestrou a klientem s demencí, nastává tak špatná spolupráce jak s klientem, tak i jeho rodinou a často se u sester objevují negativní dopady na jejich psychiku, jako je např. únava, stres a psychické vyčerpání, které se odráží i v jejich rodinném životě.Myslíme si, že by se sestry měly více motivovat ke vzdělávání se v problematice stařecké demence. Vzdělávání sestrám přinese teoretickou znalost o onemocnění demence, pomůže jim navázat lepší spolupráci s klientem s demencí a budou schopny předcházet i správně řešit možné problematické situace při jeho ošetřování, a také se naučí, jak s klientem se stařeckou demencí vhodně komunikovat. Výstupem mé bakalářské práce je návrh na seminář, určený nejen pro sestry z oddělení následné péče, který obsahuje základní informace o problematice ošetřování klienta se stařeckou demencí (Příloha 5).Dementia, also called old age disease is no more the problem of developed countries only, it has become a global problem. The population in the Czech Republic is growing old, which is caused by decreasing birth rate and decreasing mortality in all age groups. Dementia belongs to more frequent diseases than brain stroke and diabetes mellitus among seniors. In 65 year-old people the risk of dementia development is 5 %, at 75 it grows to 10 %. More than 30-50 % of seniors over 85 suffer from the disease (Jirák, Holmerová et al., 2009). Most nurses meet clients with dementia at their wards, which is why they should be educated in the field of specific needs in caring about these clients.The goals of the thesis were: 1. To indicate the specific problems of nursing care about a client with old age dementia. 2. To map the possibilities of educating hospital nurses in the problems of clients with old age dementia. 3. To find the negative factors affecting the psyche of nurses caring about clients with old age dementia. Four research questions were set to reach these goals. 1. What are the most frequent problems dealt with by healthcare staff caring about clients with old age dementia? 2. In what way a nurse communicates with a client with old age dementia? 3. What are the possibilities to educate nurses in the issues of old age dementia? 4. What are the negative impacts on the psyche of healthcare staff members working with clients suffering from old age dementia?The method of quantitative research by means of interviews was applied to elaboration of the thesis on the topic Problems of Nursing Care of a Client with Old Age Dementia. The interviews were semi-structured and were prepared for nurses from various standard departments of Strakonice Hospital. The interviews consisted of seventeen questions and were recorded with the consent of the nurses and consequently literally transcribed. The research was performed from 22 February to 1 March 2013.The research results have shown that all nurses often meet clients with dementia and they know the specific problems related to nursing care of these clients. The results also show that although they are aware of various forms of education in the issues of old age dementia they do not educate themselves and they consider other nurses uninformed. We found during the research that no forms of education are provided in this field at the departments. Despite the lack of information nurses try to solve problem situations with clients upon their experience or collecting information from other healthcare staff, which is usually insufficient. Because of these relations communication between a nurse and a client with dementia worsens, which leads to poor cooperation with both, a client and his/her family, and negative impacts in nurses? psyche like fatigue, stress and tiredness often occur, which is also reflected in their family lives.We think that nurses should be more strongly motivated to education in the issues of old age dementia. Education would bring nurses theoretical knowledge of dementia, it would help them create better cooperation with a client suffering from dementia and they would be able to prevent and duly solve possible problematic situations in the treatment and they would also learn how to communicate with a client with old age dementia efficiently. My bachelor thesis results in a draft of a seminar intended not only for nurses from consequent care departments, which contains the basic information on the issues of nursing care of a client with old age dementia (Annex 5).
