Tématem této práce je screening protilátek u těhotných žen s ohledem na jejich krevní skupinu. Screening protilátek u těhotných žen slouží k určení potencionálně rizikových těhotesntví z hlediska hemolytického onemocnění novorozenců (HON). HON je stav, kdy mateřské nepravidelné protilátky třídy IgG procházejí přes placentu a destruují fetální erytrocyty. Protilátky mohly vzniknout následkem předchozí nekompatibilní transfuze nebo proniknutím fetálních erytrocytů do matčina krevního oběhu prostřednictvím fetomaternálního krvácení. To mohlo nastat v současném těhotenství, nebo v průběhu toho předchozího. Nepravidelné erytrocytární protilátky jsou detekovány pomocí nepřímého antiglobulinového testu (NAT). Citlivost testu můžeme zvýšit použitím média o nízké iontové síle (LISS-NAT). Protilátka, která nejčastěji způsobuje HON, je anti-D. Od té doby, co se zavedla rutinní prenatální profylaxe imunoglobulinem anti-D, výrazně klesl výskyt HON způsobený touto protilátkou. Po porodu se slabé formy HON léčí pomocí fototerapie ultrafialovým (UV) světlem. U závažné formy musí novorozenec podstoupit výměnnou transfuzi. Imunoprofylaxe imunoglobulinem anti-D je výrazně levnější, než výměnná transfuze. Screening protilátek je levné, ale efektivní vyšetření. The topic of this work is screening the pregnant women's antibodies according to their blood group. The screening of the pregnant women's antibodies is to state the potentially high-risk group of pregnancy from the point of hemolytic disease of newborn (HDN). HDN is a state when maternal irregular antibodies of group IgG go via placenta and destroy fetal red cell. The antibodies may have been formed as a consequence to a previous non-compatible transfusion or due to the penetration of the fetal red cell into the mother's blood circulation via fetomaternal hemorrhage (FMH). This may have developed during the current pregnancy or the previous one. Irregular red cell antibodies are detected by the indirect antiglobulin test (IAT). The sensitivity of the test can be increased by the medium of low ionic strength solution (LISS-NAT). The antibody causing the HDN the most is anti-D. Since introducing the routine-based antenatal prophylaxis by the anti-D immunoglobulin, appearance of HDN caused by this antibody has decreased noticeably. After the delivery, the weak appearance of HDN is treated with ultraviolet (UV) light phototherapy. In case of severe appearance, the newborn undergoes a exchange transfusion. Immunoprophylaxis by the anti-D immunoglobulin is much cheaper than transfusion. Screening antibodies is cheap, but effective investigation.
