Introduction: Hyperlipidemia is a serious condition whereby the blood levels of lipids, cholesterol and triglycerides are abnormally elevated. This condition is generally asymptomatic but may lead to atherosclerotic heart disease and other types of cardiovascular pathology if not treated effectively. Various risk factors can affect the onset and severity of hyperlipidemia and those include genetic predisposition, hypertension, diabetes mellitus, obesity, poor diet, lack of regular exercise, smoking etc. Measurement and monitoring of blood levels of triglycerides, cholesterol and lipoproteins can be used as prevention method for assessing the risk for an individual to develop hyperlipidemia. Once a patient is diagnosed with the condition several approaches exist for the treatment. The main goal of the treatment is to reduce the risk of...

Charles University in Prague, Faculty of Pharmacy in Hradec Králové Department of: Social and Clinical Pharmacy Candidate: Mgr. Tereza Hofmanová Consultant: prof. RNDr. Jiří Vlček, CSc. Title of Thesis: Dosage errors in psychiatrically diagnosed patients The aim was to capture, evaluate and statistically process the pharmaceutical misconduct of the psychiatrically ill patients. From the available documentation (D-courses), I analysed the complete drug therapies for 137 patients (59 males and 78 females) hospitalized in The Psychiatric hospital in Opava, 29.10. -1.11.2013, which was approx 15,75% of the total amount of patients. Patients were diagnosed according to ICD-10 disease according to F00- F69 and G20, G308. All patients were logged polypharmacotherapy, where each patient was treated with 2 or more drugs, the maximum quantity of drugs per person was 12. On average, the amount of drugs per patient was 6.4. The most commonly represented by the drugs belonged to antipsychotics. Used antipsychotic medications have the potential to cause a number of adverse effects as extrapyramidal symptoms, QT-interval, hyperprolactinaemia and induction of metabolic syndrome. To determine drug interactions there have been used three different programs; Infopharm ©, Micromedex ® and Drugs.com. Each has seen a...

vii ABSTRACT David Čechlovský‡ , Stefanie Kraff*, Ulrich Jaehde* *Institute of Pharmacy, Clinical Pharmacy, University of Bonn, Germany ‡ Department of social and clinical pharmacy, Faculty of pharmacy in Hradec Králové, Charles University in Prague Title of diploma thesis: Individualized pharmacokinetically guided dosage adjustment of fluorouracil (5-FU) using population pharmacokinetic modelling. Objectives: To develop a method to increase the efficacy and tolerability of fluorouracil (5-FU) with pharmacokinetically-guided dose adjustment based on a target AUC. Methods: Blood samples were collected from 90 patients with the diagnosis of colorectal carcinoma treated with fluorouracil (5-FU) administered at various infusion durations. Several versions of compartmental pharmacokinetic models were fitted to the plasma concentration data, using nonlinear mixed effect modelling (NONMEM). Different error models were evaluated. The potential effect of patient covariates was evaluated using a stepwise method. Model evaluation was performed by using the bootstrap method. Results: The one-compartment linear model was chosen as a base model as it was successful in fitting to the data collected..The final model contained Additive Residual Error. A covariate BSA>CL and IIV on CL were significantly correlated to the...
Patient Perspective of Medication Information Desired
Kalaitzidis, Panagiotis ; Vlček, Jiří (advisor) ; Zimčíková, Eva (referee)
Title: Patient perspective of medication information desired Student: Kalaitzidis Panagiotis Tutor: Jiri Vlcek Department of Social and Clinical Pharmacy, Charles University in Prague, Faculty of Pharmacy in Hradec Králové Background: Even from the decade of 1970, patients wanted more information about drugs and did not want to be so depended on the information the physician provided them. Today patients are more than ever informed about their disease and the available and appropriate treatment, due to the high volume of information that can be acquired through the Internet. The traditional role of pharmacists was the provision of medicines to the population. However, nowadays, this role has been changed so as to include other tasks, such as management of prescribed medicines, and promotion of health, since pharmacists can serve as a source of expertise knowledge to the customers. Due to the fact that there is no relevant research for the case of Greek patients, this pilot research could help to fill this gap, by trying to find out what kind of information patients want to know about medicines in several pharmacies in Greece. Aim: The aim of this research is to examine what kind of information patients want related to medicines. More precisely, this research will try to identify the type of...

ASSESSMENT OF POTENTIAL DRUG INTERACTIONS IN TWO INPATIENT WARDS IN UNIVERSITY HOSPITAL IN HRADEC KRÁLOVÉ Author: Maříková M.* Tutor: Vlček J.** *Clinical Pharmacy Department University Hospital Hradec Králové **Social and Clinical Pharmacy Department, Faculty of Pharmacy in Hradec Králové, Charles University in Prague, The Czech Republic Keywords: drug interactions, clinical pharmacist, drug-related problems, geriatric patients, intensive care Background: Medicine and pharmacy are two different but related disciplines that complement, overlap and need each other. The first work, which has looked at medication errors was published in the nineties of the 20th century by the American Institute of Medicine "To Err is Human: Building a Safer Health System's" issued in November 1999. The report is based on analysis of several different studies, concluding that 44.000 to 98.000 people die each year because of preventable drug errors. In comparison, fewer than 50.000 people died from Alzheimer's disease and 17.000 died of illicit drug use in the same year. Each administration of the drug to the patient is a kind of experiment, which rely on available population data of pharmacokinetic and pharmacodynamic properties of drugs. The same situation is in a topic of drug interactions. Number of drugs in use...
