Metabolism of immunosuppressants in children with inflammatory bowel disease

Metabolism of immunosuppressants in children with inflammatory bowel disease Abstract Thiopurines are still used as the main immunosuppressants in the maintenance therapy of paediatric Crohn's disease. According to official guidelines, in case of failure, biological therapy (mainly infliximab or adalimumab) is generally commenced. The present thesis focuses on the possibilities of therapy optimization in children in whom the conventional Crohn's disease therapy has failed based on the metabolism of administered immunosuppressive medications. We confirmed that the knowledge of thiopurine metabolites concentration in red blood cells may help to monitor patients' adherence to therapy. However, according to our observations in children on combination therapy of infliximab and thiopurine lower concentrations of 6- thioguanine (the active metabolite of thiopurines) seems to be effective compared to what has been expected. We presented a web application designed to partially substitute for the thiopurine metabolite measurements based on easily available laboratory data. Last, but not least, we confirmed that the clinical effect and safety profile of both adalimumab and infliximab are similar in the treatment of paediatric Crohn's disease. Only in case of family history of atopic dermatitis adalimumab may be preferred.
Impact of the Gastrointestinal System on Metabolic Control in Children with Type 1 Diabetes Mellitus - Gut Peptide Hormones

Our objective was to evaluate plasma levels of gut hormones and hormones associated with glucose metabolism in children with type 1 diabetes mellitus (T1DM), and to correlate plasma concentrations of gut hormones with blood biochemistry, markers of metabolic control and anthropometric parameters. A multiplex assay kit (LINCOplex® ) was used for the determina- tion of postprandial plasma levels of specific gut peptide hormones. Amylin, glucose- dependent insulinotropic polypeptide (GIP), active glucagon-like peptide 1 (GLP-1), active ghrelin, insulin, leptin, pancreatic polypeptide (PP), and polypeptide YY (PYY) were assessed prospectively in 55 subjects including 19 T1DM children (mean age: 13.4 years) and 21 healthy reference controls (mean age: 13. 4 years) and 15 patients with functional abdominal pain (FAP) (mean age: 10.5 years). In total, 440 plasma hormones samples were assessed in 55 patients. Entered data were examined using a non-parametric Wilcoxon's test. Further- more, statistically significant correlations were assessed by stepwise regression analysis. A analytické části nejprve představuji společnost Škoda Auto a.s., včetně její historie, aktuálních výsledků a jejího přínosu pro ekonomiku ČR. Poté se přesouvám na zahraniční trh (Malta) a pokouším se definovat pozici značky Škoda v tamním tržním prostředí, sleduji její posilování a podporu ze strany místního dovozce a prodejce a práci uzavírám výzkumem znalosti a vnímání značky.
