Zjištění podílu levátorového a fasciálního traumatu pánevního dna na rozvoji symptomů sestupu pánevních orgánů - vliv vaginálního porodu Abstrakt Poranění svalu levator ani (LAM) je významným rizikovým faktorem prolapsu pánevních orgánů (POP). Subdivize puborektální (PRM) a pubviscerální (PVM) jsou na Level III vaginální podpůrné struktury. Nulová hypotéza - nebylo významných rozdílů ve vzorcích svalových poranění mezi podskupinami LAM. Tato retrospektivní studie magnetické rezonance (MRI) zahrnovala: 64 kontrol nulligravid bez dysfunkce pánevního dna (PFD) a 526 POP symptomatických žen různé parity. Primárním výstupem byla morfologie PVM a PRM v axiálních rovinách 1/ místo připojení ke stydké kosti, 2/ viditelné oddělení/hranice mezi PVM a PRM. Připojení bylo hodnoceno jako "normální" nebo "abnormální". "Abnormální" bylo rozděleno do dvou typů: a) "Typ I" - ztráta svalové hmoty, ale zachování celkové svalové architektury, b) "Typ II" - oddělení svalu od stydké kosti. Subdivize puboanalis (PAM) byla vyhodnocena jako reprezentativní část PVM. Uchycení PAM a PRM a rozdělení bylo rozlišeno ve všech kontrolních případech. Připojení PAM a PRM se významně nelišily. Charakteristiky skupiny POP byly: parita: 1,9 (0,8, instrumentální porod: 5,6 %, hysterektomie nebo operace POP: 60 %, všechna stadia POP-Q, defekt... Determination of the proportion of levator and fascial trauma of the pelvic floor in the development of symptoms of the descent of pelvic organs - the influence of vaginal birth Abstract Injury of levator ani muscle (LAM) is a significant pelvic organ prolapse (POP) risk factor. The puborectalis (PRM) and pubvisceral (PVM) subdivisions are at Level III vaginal support structures. Null hypothesis - there were no significant differences in muscle injuries patterns among LAM subdivisions. This retrospective magnetic resonance imaging (MRI) study included: 64 nulligravidae controls without pelvic floor dysfunction (PFD) and 526 POP symptomatic women of different parity. Primary outcome was PVM and PRM morphology on axial planes 1/ the place of the attachment to the pubic bone, 2/ visible separation/border between PVM and PRM. The attachment was scored as "normal" or "abnormal". The "abnormal" was divided in two types: a) "Type I" - loss of the muscle substance, but maintenance of the overall muscle architecture, b) "Type II" - muscle detachment from the pubic bone.
