Pro léčbu povrchových mykotických infekcí (dermatofytóz) je k dispozici relativně široké spektrum antimykotik z různých chemických tříd. Tyto látky cílí především na různé kroky syntézy ergosterolu, čímž narušují buněčnou membránu (allylaminová, azolová a morfolinová antimykotika) nebo na funkci mikrotubulů (benzofuranová antimykotika). Navzdory tomu, že léčba dermatofytóz je často spojena s dlouhodobou expozicí houby antimykotikům (týdny až měsíce), antifungální rezistence u dermatofytů byla donedávna považována za vzácnou. Současné studie však ukázaly rostoucí výskyt chronických infekcí, reinfekcí a případů selhání léčby v důsledku objevující se rezistence k některým běžně užívaným antimykotikům. Nejzávažnějším problémem současnosti je šíření rezistence vůči terbinafinu, jejíž molekulární podstata spočívá v mutacích v genu pro enzym skvalen epoxidázu (SQLE). Nárůst výskytu této a dalších rezistencí je v současnosti alarmující zejména v Indii, zatímco situace Evropě či Americe je z pohledu citlivosti dermatofytů stále relativně příznivá. Cílem této bakalářské práce je shrnout dosavadních poznatky o antifungálních rezistencích u dermatofytů a jejich molekulárním podkladu. Práce dále shrnuje epidemiologickou situaci z hlediska výskytu a šíření těchto rezistencí ve světě. Klíčová slova: antifungální... For the treatment of superficial fungal infections (dermatophytosis) is available a wide spectrum of antifungals from various chemical groups. These antifungals are mainly focused on various steps of ergosterol synthesis, thereby disrupting the cell membrane (allylamine, azole and morpholine antifungals) and on microtubule function (benzofuran antifungals). Despite the fact that the treatment of dermatophytosis is often associated with long-term exposure of the fungus to antifungals (weeks or months), until recently antifungal resistance in dermatophytes has been considered rare. However, current studies have shown the incidence of chronic infections, reinfection and treatment failures due to emerging resistance to some commonly used antifungals. The most serious problem today is the spread of resistance to terbinafine where the molecular principles are founded in the structural changes in the squalene epoxidase enzyme (SQLE). The increase in the incidence of this and other resistances is currently alarming especially in India, while the situation in Europe and America is in terms of dermatophyte susceptibility still quite favorable.
