Bakalářská práce je zaměřena na problematiku predikce předčasného porodu. Předčasný porod je jedním z nejzávažnějších problémů v porodnictví a příčinou mnoha perinatálních komplikací. Incidence se dnes v České republice pohybuje kolem 7,2 %. Problémem predikce této závažné patologie je falešná negativita nebo naopak falešná pozitivita, kdy jsou ženy zbytečně hospitalizovány a léčeny. Je velmi důležité najít metodu, která by dokázala odhalit co nejvíce hrozících předčasných porodů a zároveň vykazovala co nejnižší falešnou pozitivitu. To je ovšem velmi obtížné. Předčasný porod je zapříčiněn multifaktoriálními vlivy. Cílem práce bylo zjistit predikovatelnost předčasného porodu pomocí vaginální cervikometrie ultrazvukem, její kombinace s vyšetřením hladiny fetálního fibronektinu ve vaginální tekutině a účinnost algoritmu QUIPP v predikci předčasného porodu. Tyto cíle byly doplněny o 6 hypotéz, které pomohly jejich splnění. Od ledna 2019 do března 2020 byly vyšetřovány asymptomatické ženy s vysokým rizikem předčasného porodu v týdnu těhotenství 22+0 až 25+6. Do práce byly vybrány ženy, které zároveň následně porodily v Gynekologicko-porodnické klinice 1. LF UK a VFN v Praze. Shromážděná data od těchto žen byla následně zpracována, statisticky spočítána a popsána pomocí tabulek a grafů. Z výsledků práce... This bachelor thesis focuses on the issue of premature birth prediction. Premature delivery is one of the most serious problems in obstetrics and the cause of many perinatal complications. The incidence in the Czech Republic today is around 7,2 %. The problem of prediction of this serious pathology is false negativity or converselyfalse positivity. Regarding the latter, women are unnecessarily hospitalized and treated. It is very important to find a method that could detect as many impending premature births as possible and at the same time show the lowest possible false positivity. However, this is very difficult. Premature birth is caused multifactorial effects. The aim of the study was to determine the premature birth prediction using vaginal ultrasound cervicometry in combination with the examination of fetal fibronectin levels in the transvaginal fluid and the effectiveness of the QUIPP algorithm in predicting the premature birth. These goals were accompanied by 6 hypotheses that helped to fulfill the goals. From January 2019 to March 2020, asymptomatic women with a high risk of premature birth in the week of pregnancy 22+0 to 25+6 were examined.
