Úvod: Značná část dětských rozštěpových pacientů trpí OMS a jejími následky jako nedoslýchavost, adhezivně retrakční změny a cholesteatom. Cílem naší retrospektivní studie bylo zjistit, zda lze tyto následky minimalizovat výběrem operační techniky při korekci patra - použitím faryngofixace. Materiál a metodika: Retrospektivní studie 163 pacientů - 325 uší (1x atrezie zvukovodu), operovaných v 5 měs. - 8 letech, doba sledování 36-84 měs. Výsledky: 125 (38%) uší mělo retrakci v oblasti epitympana a 45 (14%) mělo retrakci v oblasti zadního horního kvadrantu středouší, bylo 10 (3%) cholesteatomů 5x epitympanální a 5x sinus cholesteatom. Nepodařilo se prokázat signifikantní vztah mezi použitím faryngofixace a závažností otologického nálezu resp. četností retrakcí v epitympanu a zadním horním kvadrantu(p 0,53) Závěr: podle našeho zjištění nesouvisí použití faryngofixace a timingu koreční operace patra s tíží následného otologického nálezu. Dle našich zkušeností a v souladu s literaturou zatím zůstává nejlepší prevencí adhezivně retrakčního procesu a cholesteatomu dispenzarizace s otomikroskopickým sledováním a včasná indikace k chirurgickému řešení podle závažnosti změn a dynamiky procesu. The aim of our retrospective study was to find out whether these effects can be minimized by selecting an operative technique to correct the palate - using pharyngofixation or by surgery timing . Material and methods: Retrospective study involving163 patients - 325 ears (1x external auditory canal atresia), operated at the age of 5 months - 8 years, follow up time 36-84 months. Results: 125 (38%) ears had an epitympanic retraction and 45 (14%) had a superior posterior quadrant retraction, 10 (3%) had cholesteatoma, together 5 epitympanic and 5 sinus cholesteatoma. No significant relationship was found between the use of pharyngofixation and the severity of the otologic finding, respectively of retraction rates in the epitympanum and the posterior superior quadrant (p 0.53). Analogous no significant relationship was found between the surgery timing and the severity of the otologic finding (p 0.48) Conclusion: In our findings, the use of pharyngofixation and timing of palate correction surgery is not related with the severity of subsequent otologic findings.
