Cíle: Radioterapie je při terapii zhoubných nádorů hlavy a krku hojně využívanou léčebnou modalitou z důvodu poměrně vysoké radiosenzitivity těchto malignit. Zlepšuje léčebné výsledky a umožňuje zachování funkcí a kontinuity tkání. V minulosti byla však prokázaná celá řada nežádoucích účinků radioterapie. Protože v ozařovaném poli je velmi často zahrnuta i štítná žláza, jedním z důsledků může být hypothyreóza. Navzdory tomu jsou poruchy funkcí štítné žlázy zřídkakdy uvažovanou komplikací radioterapie. Cílem této retrospektivní studie bylo zjistit četnost thyreopatií (zejména hypofunkce) u pacientů po radioterapii, porovnat hladiny hormonů štítné žlázy u těchto pacientů s hladinami hormonů pacientů z kontrolní skupiny a pokusit se odhadnout vývoj hypothyreózy v čase. Materiál a metodika: Měřením hladin thyreostimulačního hormonu (TSH), volného thyroxinu (FT4) a volného trijodthyroninu (FT3) byly stanoveny thyreoidální funkce u 43 pacientů s nethyreoidálními karcinomy oblasti hlavy a krku léčených radioterapií, eventuálně v kombinaci s jinou léčebnou modalitou. Tato data byla porovnána s hladinou hormonů u 40 kontrolních pacientů léčených pouze chirurgicky. Výsledky: Ve středním čase sledování 34 měsíců po radioterapii byla hypothyreóza diagnostikována u 35 % ozářenách pacientů. Při srovnání s... Objectictives: Radiation therapy is often applied to patients with head and neck cancer because of a high sensitivity to these cancers. It improves treatment outcomes and permits the preservation of functions and physical form. However, it has been shown that various complications can result from radiation therapy. Hypothyroidism can be one of them as the thyroid gland is frequently included in radiation fields. In spite of that, thyroid hypofunction is not commonly considered a complication of radiotherapy for head and neck cancer. The purpose of this retrospective study was to determine the frequency of thyroid dysfunction in patients after radiotherapy, to compare thyroid hormone levels in irradiated patients with control group and to predict development of thyroid hypofunction in time. Material and method: Thyroid function was measured by means of thyroid stimulating hormone (TSH), free thyroxine (FT4) and free triiodthyronine (FT3) in 43 patiens who had nonthyroid head-neck carcinomas treated by radiotherapy or radiotherapy in combination with other treatment modalities. These data were compared with hormone levels of 40 control group patients treated solely by surgery.
