Souhrn V patogenezi syndromu diabetické nohy (SDN) se uplatňují nejen lokální tkáňové faktory a ischemie, ale i infekce, která může být současně příčinou reulcerací. Neuropatická porucha kostního metabolismu vede k Charcotově osteoarthropatii (CHOA). Cílem dizertační práce bylo posoudit efektivitu aplikace nových kožních krytů v experimentu, dále zhodnotit lokální vaskulogenezi při různých metodách buněčné léčby SDN, vymezit roli infekce u reulcerací a definovat scintigrafické parametry aktivity CHOA. Ve studiích zabývajících se problematikou lokálních patologických procesů u SDN jsme prokázali v experimentu, že želatinová nanovlákna akcelerují hojení rány a mohou být vhodným buněčným nosičem pro kožní regeneraci a že acellulární porcinní dermis je účinnější v procesu hojení ran v porovnání s xenotransplantáty. Ve studiích zabývajících se problematikou terapeutické vaskulogeneze jsme zjjistli, že účinnost kmenových buněk (KB) z kostní dřeně je obdobná jako účinnost KB z periferní krve po stimulaci. Při posuzování rizika systémové vaskulogeneze jsme nezjistili signifikantní vzestup sérových hladin pro-angiogenních cytokinů, což svědčí pro parakrinní efekt KB. Prokázali jsme signifikantní korelaci inhibitoru angiogeneze (endostatinu) s množstvím aplikovaných KB, což by mohlo odrážet lokální vaskulogenezi. Při... Local tissue factors, ischemia and infection (which are often the cause of re-ulceration) are the main pathogenetic factors for diabetic foot disease (DFD). Neuropathic bone metabolism disorder leads to Charcot osteoarthropathy (CHOA). The aim of this dissertation was to assess experimentally the effectiveness of new skin substitutes, evaluate local vasculogenesis in different types of cell therapy of DFD, the role of infection in recurrence of DFD and scintigraphic parameters of activity of CHOA. Our studies concerning local pathological processes in DFD experimentally proved that gelatine nanofibers accelerate wound healing and can be suitable scaffolds for cell transfer and skin regeneration and also that acellular porcine dermis is more effective in healing of chronic wounds then xenotransplants. Our studies concerning therapeutic vasculogenesis confirmed that efficacy of stem cells (SC) harvested from bone marrow is similar in efficacy to SC separated from peripheral blood after stimulation. We found no evidence for systemic vasculogenesis by means of a significant increase of pro-angiogenic cytokines, which confirms the paracrine effect of injected SC.
