Souhrn Úvod: S problematikou léčby ztížené nosní průchodnosti se setkává ve své praxi každý otorinolaryngolog. Jednou z nejčastějších příčin oboustranné nosní obstrukce různého rozsahu je chronická hypertrofická rýma. Využití diodového laseru k ošetření sliznice dolních skořep je jednou z možností chirurgické léčby. Cíl práce: Ověření vlivu operační techniky subepiteliální laserové koagulace na nosní sliznici. Sledovat rozsah poškození nosní sliznice na histologických preparátech laboratorních zvířat. Zjistit, zda preparáty s obsahem benzalkonium chloridu, ovlivňují kvalitu mukociliárního transportu. Materiál a metodika: Pro část experimentální bylo zvoleno laboratorní zvíře prase domácí. Sliznice nosních skořep byla ošetřena diodovým laserem - vláknem - v kontaktním režimu aplikovaným subepiteliálně. Bylo ošetřeno celkem 16 zvířat. Odběry k histologickému vyšetření byly provedeny 3. a 8. den. po výkonu. V části klinické byl ke konečnému zpracování použit soubor 89 pacientů s diagnózou chronické hypertrofické rýmy. Soubor jsme rozdělili na 3 skupiny. První skupina zahrnovala všechny pacienty užívající topický kortikoid. Druhou skupinu tvořili pacienti užívající preparát s obsahem benzalkonia chloridu (BCH). Třetí skupinu vytvořili pacienti s neúspěšnou konzervativní terapií a následně ošetřeni chirurgicky... Introduction: any ENT specialist meets in his praxis a treatment dilemma of impaired nasal flow. Chronic hypertrophic rhinitis belongs among the most frequent cause of bilateral nasal obstruction. The evaluation of diode laser for treatment of nasal mucosa is a possibility of surgical approach. Aim of the work: examination of subepithelial laser coagulation damage on nasal mucosa. Follow-up of the damage of the nasal mucosa on histological specimen at laboratory animals. Determination of effect of benzalconium chloride remedies on the quality of mucocilliary transport. Material and methods: domestic pig as a laboratory animal was chosen for the experimental part of the work. The mucosa of the turbinates was treated with a diode laser (wire) in a contact mode, which was applied under the epithelium. 16 animals in total were treated. Histological take-off was performed the 3rd and the 8th posttreatment day. For the clinical part a group of 89 patients was collected with a diagnosis of chronic hypertrophic rhinitis. The group was dividend to 3 subgroups. The first subgroup comprised of all patients taking a nasal steroid. The second group was formed with patients taking some of benzalconium chloride preparate (BCH). The third group was collected of patients after unsuccessful conservative treatment of...
