Nosní polypóza je sice onemocněním benigním, ale výrazně ovlivňuje kvalitu života. Zdá se, že není onemocněním per se, ale že se spíše jedná o výsledek nebo o vyjádření chronických zánětlivých procesů probíhajících na sliznici dutiny nosní a VDN. Patogeneza NP zůstává přesto neznámá. Prevalence výskytu Staphylococcus aureus u pacientů s nosní polypózou je vysoká. Nové studie prokázaly výskyt IgE protilátek ve tkáni polypu, zaměřených proti stafylokokovým enterotoxinům (SAE). Avšak role Staphylococcus aureus v rozvoji tohoto onemocnění je s největší pravděpodobností minimální. Za základ terapie považujeme lokální kortikosteroidy. Při neúspěchu nebo rozsáhlé nosní polypóze se velmi často uchylujeme k chirurgickému řešení. Nejčastěji prováděnou operací nosní polypózy je FES. V naší studii jsme se zabývali vlivem pooperační terapie antibiotiky u pacientů po prodělané FES. Výsledkem studie byl negativní nález na kultivaci pro SA u 30% pacientů, nezávisle na tom, zda pacienti dostávali pooperačně placebo nebo ATB. Je tedy pravděpodobné, že samotná přítomnost polypu podporuje kolonizaci Staphylococcus aureus.

Rhinosinusitis is a heterogeneous group of diseases, with different underlying etiologies and pathomechanisms, and may indeed represent an umbrella, covering different disease entities. It is currently not understood whether acute recurrent rhinosinusitis necessarily develops into chronic rhinosinusitis, which then possibly gives rise to polyp growth, or whether these entities develop independently from each other. Nasal polyposis is an inflammatory condition that affects from 1% to 4% of the general population. Polyposis is believed to be multifactorial disease that is frequently associated with asthma and aspirin sensitivity. The studies show that more polyps were found in nonatopic that in atopic patients. A global evaluation of NP must include, together with nasal endoscopy, symptoms assessment, and CT-scan, the measurement of the quality of life. In my study I used the Sino nasal outcome test (SNOT-22), with 22 questions to ask the patients about the quality of life. They were asked to fill up the questionnaire before and 6 month after the surgical procedure. Twelve patients were included to this study, than they were divided into two groups, patients with and without the factors of the risk (asthma, aspirin sensitivity and allergy). Prevention of recurrent nasal polyposis in patients with detection of enterotoxi n producing Staphylococcus aureus strains

Enterotoxins produced by Staphylococcus aureus can act as superantigens and thus, regardless of the antigenic specificity, influence the course of chronic rhinosinusitis with nasal polyps (NP). The aim of this study was to determine if antibiotic treatment administered after endoscopic sinus surgery (ESS) for NP can positively influence the course of the disease compared to placebo. After ESS, 23 patients which tested positive, in a perioperative culture, for SA strains producing enterotoxins A - E and TSST-1, were randomized into 2 groups. Group A (13 patients), which in addition to treatment with intranasal steroids and saline douches, received oral anti-staphylococcal antibiotics (according to the minimal inhibitory concentration) for 3 weeks. Group B (10 patients) received a placebo. Both groups were compared preoperatively, then again at 3 and 6 months after surgery using a symptom-specific score, an endoscopic score and the SNOT-22 quality of life questionnaire. Slightly better results were achieved in patients who received antibiotic therapy. However, the differences were not statistically significant. Impact of pharyngofixation and timing in cleft palate repair surgery on the development of chronic adhesive otitis media and cholesteatoma in cleft palate patients.

Introduction: A significant number of pediatric cleft patients suffer from OMS and its consequences as hearing loss, adhesive retraction changes and cholesteatoma. The aim of our retrospective study was to find out whether these effects can be minimized by selecting an operative technique to correct the palate - using pharyngofixation or by surgery timing . Material and methods: Retrospective study involving163 patients - 325 ears (1x external auditory canal atresia), operated at the age of 5 months - 8 years, follow up time 36-84 months. Results: 125 (38%) ears had an epitympanic retraction and 45 (14%) had a superior posterior quadrant retraction, 10 (3%) had cholesteatoma, together 5 epitympanic and 5 sinus cholesteatoma. No significant relationship was found between the use of pharyngofixation and the severity of the otologic finding, respectively of retraction rates in the epitympanum and the posterior superior quadrant (p 0.53). Analogous no significant relationship was found between the surgery timing and the severity of the otologic finding (p 0.48) Conclusion: In our findings, the use of pharyngofixation and timing of palate correction surgery is not related with the severity of subsequent otologic findings.
