Use of AED at OHCA in Central Bohemian Region
Jalovcová, Kateřina ; Knor, Jiří (advisor) ; Peřan, David (referee)
Absktrakt Aims: This work deals with the use of an Automatic External Defibrillator (AED) in outpatient circulatory arrest in the Central Bohemian Region. The aim of this work is to map who most often activates an AED in a detected circulatory arrest. Another goal is to compare meaningful activations, ie activations where an electrical impulse was initiated and to compare them with AED activations when an eletrical impulse was not initiated. Methods: The research part of the work was performed by a conducting retrospective analysis. The presented data was provided by the Emergency Medical Services of the Central Bohemian region for the years 2019 and 2020. The data included recorded trips to outpatient circulatory arrests and activation of the First responders system and the use of AED for outpatient circulatory arrests. The hypotheses were tested using Fisher's exact test with a significance level of 5%. Results: In 2019, a total of 21 activations of First responders were recorded and in 2020 there were 27 activations. In 2019, AED was activated and defibrillation was initiated in 6 cases by First responders and in only 1 (20%) case the Return Of Spontaneous Circulation (ROSC) occurred. In 2020, EAD was activated and an electrical impulse discharged in 6 cases and in 2 (33.33%) cases ROSC occurred....
The Influence of Timely Initiation of CPR on the Prognosist of Cardiac Arrests due to Cardiac Causes
Beňová, Eliška ; Knor, Jiří (advisor) ; Peřan, David (referee)
The thesis deals with the influence of timely initiation of the cardiopulmonary resuscitation in cases of cardiac arrest due to cardiac causes. In the research part of the thesis, I mainly gave my attention to the factors connected with time and quick initiation of the cardiopulmonary resuscitation, so I created contingency tables from the input data where I was interested in the success rate of the non-hospital cardiopulmonary resuscitation given by a bystander compared to the cases where it is not performed. From the total of 1315 cases, I selected 872 individuals where the cardiac arrest had cardiac causes. The observed period of 2019-2020 shown that in the cases where a layman initiates the resuscitation before the arrival of a professional team the return of spontaneous circulation occurs at 24.78% cases. Meanwhile when the cardiopulmonary resuscitation is not given, the restoration of circulation occurs in 25.11% of individuals. This points out that the result of cardiopulmonary resuscitation does not depend on whether the massage was given by a layman or not. I also dealt with the influence of the time of arrival of the professional team at the place of the event. I found out that the most common travel time in the Central Bohemian Region is 5.01-10 minutes and that the time of arrival...
Standardization of pre-hospital care as a tool to prevent patient harm
Peřan, David ; Marx, David (advisor) ; Šeblová, Jana (referee) ; Šín, Robin (referee)
The theme of the dissertation thesis is "Standardization of pre-hospital care as a tool to prevent patient harm." The aim of the work is to identify individual areas and procedures of pre-hospital emergency care suitable for standardization, and subsequently the creation or adaptation of procedures, including their verification. The handover of patient information and the area of clinical procedures (basic examination of the patient, evaluation of the electrocardiogram (ECG) and differential diagnosis of urological problems) were selected for evaluation. The first study focuses on the effectiveness of information handover using the acronym ATMIST (Age - Time - Mechanism - Injury / Illness - Signs and Symptoms - Treatment) in a prospective observational study. The result is the finding that despite the use of a structured tool, only 80% of the information is correctly transmitted. A recommendation for practice is an electronic form of information handover that prevents the loss of information. At present, the electronic form of the pre-notification was already introduced at the Prague Emergency Medical Services. The second study focuses on the benefit of a structured examination of a patient using the ABCDE approach (Airway - Breathing - Circulation - Disability - Exposure) in a simulation...
Organization of Advanced Life Support Provider Course in the Czech Republic
Peřan, David ; Vrzáček, Petr (advisor) ; Lejsal, Matěj (referee)
The theoretical part of this thesis titled "Organization of Advanced Life Support Provider Course in the Czech Republic" deals with adult education from different perspectives. Firstly, it briefly discusses the history of adult education and then deals with emerging trends in human resource management and knowledge management (e.g. management by competencies, learning organization, etc.) in relation to education. Then clearly describes the area of adult education with a focus on training and mainly training of health care professionals. The last part of theoretical block describes the Advanced Life Support Provider course and teaching methods that are used during the course. The practical part presents the action research project, which includes quantitative methods, which represents two questionnaire surveys relates to the satisfaction of the course participants with the organization and practical usage of the taught techniques. The results of these surveys are high rate of satisfaction with ongoing organisation of the courses and also high rate of practical usage of the taught techniques. The qualitative method aimed at creating a handbook for the organization of the ALS courses in the Czech Republic and its implementation in practice. It has focused on the acquisition of specific know-how from...
The importance of volunteer service in the protection of inhabitants, focusing on the Czech Red Cross
PEŘAN, David
The topic of this bachelor thesis is ?The importance of volunteer service in the protection of inhabitants, focusing on the Czech Red Cross? and how it deals with volunteering and the volunteer service, which is provided in the Czech Republic, especially in the capital city Prague. The theoretical part of this study is about the history of the volunteer service in Europe, Czech Republic and Prague. The basic legislation about the volunteer service, specifically Act. No. 198/2002 Coll. on volunteer service and on amendments to certain Acts (Volunteer Service Act), is analyzed in detail. The next document, which was used in the theoretical part, is ?The statues of Czech Red Cross organization? (CRC), which adjust the position of volunteers inside this organization. The Law on volunteer service, describes volunteer service, as a service, which is provided by person without employment relationship or relationship based on staff or membership. One of the basic principles of the Red Cross (RC) organization is volunteer service, which means, that everyone, who cooperates with the RC organization provides a volunteer service, but in the Czech Republic individuals can provide services based on employment, staff or membership. Here is the conflict between national law and the principles of RC. This thesis works on the concept, that everyone, who cooperates with the Czech Red Cross is a volunteer, including individuals, who are staff, members or employees. The statues of the Czech Red Cross organization present three groups that can provide services: employees, members and volunteers. Employees have contracts of employment and this contract delegates them into leadership positions, for example in a crisis staff at the strategic and operational level. Members have rights and responsibilities and their activity is used in a emergency crews and especially at tactical level in humanitarian units. Volunteers are used during an extraordinary event to support humanitarian units and other activities, which are mentioned in Regional Concept of Emergency Preparedness. Basic training is parallel for all groups and after this training they start special trainig according to performed activities. The research question is: Does the the ?Concept of emergency preparedness of the Czech Red Cross up to the 2015? improve situation of CRC's volunteers in the field of protecting inhabitants? The response was a qualitative analysis of collected data about the volunteer service and comparative analysis of basic legislative documents of crisis management, protection of ihabitants and the Czech Red Cross organization. Objects of comparison are legislative documents. Comparison is to find out the answers to the criteria analysis from the fifteen selected legislative documents and two concepts. The analysis containes the answers to five questions. In the analysis were used synchronous comparison of monitored issues in the current legislation of the Czech Republic. Thanks to the results of the comparative analysis was prepared the Plan of CRC to help the capital city of Prague according to the applicable Methodology of the Plan of CRC to help a region. The result of the comparative analysis is the fact that the seven documents deals with volunteer services directly and one indirectly. Five documents deals with the funding of volunteer services directly and one indirectly. The real output is the Plan of CRC to help the capital city of Prague. I think that these disputed issues will not be solved soon. The question is, how will the new CRC documents (Plan of help and Concept of emergency preparedness) be applied.
