MUDr. Yvona Angerová, Ph.D., MBA Thesis Title: Physiotherapy after surgery for upper limb spastic dystonia in patients after brain damage Abstract This bachelor's thesis overviews surgical treatment for upper limb spastic dystonia and subsequent post-operative physiotherapy. The thesis consists of two sections, theoretical and practical. Theoretical part describes upper motor neuron syndrome and definitions of two relevant symptoms, which are spasticity and spastic dystonia. This section also contains quantitative estimation methods for spasticity and spastic paresis. This is followed by description of surgical procedures classified based on different upper limb segments. Finally, physiotherapeutic techniques recommended after these surgical procedures are described. Practical section consists of description of two case reports, in which the brain injury led to spastic dystonia that was treated by surgery of upper limb. Each case contains past medical history, entrance and exit kinesiological analyses, functional examination of upper limb using modified Frenchay arm test and physiotherapeutic sessions reports. Using means of physical therapy to influence spasticity from the physiotherapist 's point of view

The theme of the bachelor thesis is using means of physical therapy to influence spasticity from the physiotherapeutist's point of view. Theoretical part describes upper motor neuron syndrome and spasticity as one of its symptoms. Further it states methods of evaluation and options of influence of spasticity including latest research methods. Separate chapter is dedicated to the physical therapy, its impact on human organism and means used to treat spasticity. In the practical part the influence of cryotherapy on spasticity of upper limb of two patients after brain injury is examined. Efficiency is measured by Tardieu scale and goniometry. Main contribution of the thesis lies in gathering and analyzing of present findings, mainly on the field of physical therapy. Title: Therapy of spasticity with botulinum toxin and assessment of the effect by Tardieu Scale Abstract: This paper discusses paralysing effect of botulinum toxin which is used to treat spastic muscles in contemporary medicine. The paper focuses on spasticity in patients with disorder of the central nervous system caused most commonly by stroke. At first it deals with theoretical knowledge about physiology of movement, then describes active ingredient and explains the mechanism of its effect. It dedicates a lot of attention to follow-up rehabilitation as an integral part in the management of spasticity. This paper is based on two case histories. A practical part describes in detail patient's condition before and after application of botulinum toxin. A progress of the effect was assessed during periodic meetings. These instruments were used to assess: Tardieu Scale of spasticity and Frenchay arm test. In connection with the application of botulinum toxin is emphasized active co-operation of the patient. It is an essential precondition for positive results in the management of spasticity. The effect of prolonged stretching on spastic muscle contracture before and after Botulinum toxin A application

A large proportion of patients with central motoneuron lesions is at risk of spastic paresis and formation of contractures. One of the therapeutic techniques used in patients with spastic paresis is prolonged stretching. The theoretical part summarizes the knowledge regarding spastic paresis, its clinical evaluation, the development of contractures and therapeutic interventions, especially botulinum toxin (BTX) and stretching. The aim of this study was to compare the effectiveness of prolonged stretching on spastic muscle contracture before and after BTX application. In our retrospective comparative study there were included 30 patients divided into two groups. The first group consisted of 15 patients performing prolonged stretching of the m. rectus femoris (m. RF) for three months according to Guided Self-rehabilitation Contract of Professor J. M. Gracies. The second group (15 patients) performing the same procedure, but at the same time BTX was injected into the m. RF. We monitored the change in passive knee joint range of motion into flexion and the change in 10MWT time.
