Education of nurses in intensive care

For my diploma thesis I chose the topic of education of nurses in intensive care. In the field of nursing education, it was necessary to respond to new trends in the European Union. In recent years, the situation in the field of education has changed in many ways. Act No. 201/2017 Coll. it has brought a novelty that the credit system of education has been abolished and it is therefore possible to conclude that the motivation of nurses to education may have fallen. The theoretical part describes the history of intensive care in the world and in the Czech Republic. Mentioned is the history of nursing education in intensive care and the history of nursing education in the Czech Republic. The next chapter points to the current situation of intensive care, the requirements for qualification education of a nurse and the competence of a nurse specialist. The possibilities of intensive care education, including lifelong learning, are being announced. An integral part is the motivation chapter. The main aim of my thesis was to find out the motivation of nurses to specialize in intensive care. Educational skills of nurses - theory and practice

In my Bachelor's thesis I am approaching the theme of "Educational abilities of the nurses - theory and praxis". The target of the work is to approximate the the issue of education, to establish the level of theoretic knowledge of the nurses working in the clinics of the Royal Vinohrady Teaching Hospital, regarding the education of the patients and to try to determine the quality level of the education being performed, based on the interview survey and patients' satisfaction. The thesis is divided into theoretical and empirical parts. The theoretical part contains an insight into the history of the pedagogy and the development of the education, describes the basic educational terms and the educational determinants to help us to understand the issue. It also approximates the challenging task of the nurses educators, who, in order to provide the educational activity, also have to master the process of education, understand the area being educated and the art of the communication with the patient. The empirical part is focused on a quantitative exploration in the form of a interview survey, which was carried out in the Royal Vinohrady Teaching Hospital. Mentally challenging moments of intensive care's nurse

The profession of general intensive care nurse is specific in its high demands on physical, technical, ethical and mental abilities of each individual. The thesis focusses on the mental stress of the nurse, analyses potentially high-risk professional situations the nurse encounters and proposes methods for their management. The topic of mental stress and its handling has, and will, have important impact on the field. The goal of the thesis is to map the psychologically challenging situations that all of the 129 nurses approached in a smaller local medical facility considered as difficult to handle and thus have impact on their ability to cope with a demanding job. The situations are defined and evaluated using predefined hypotheses. The results of an anonymous quantitative questionnaire clearly show that the largest levels of stress and discomfort are caused by situations which are "directly connected to medical care provided by the general intensive care and accident and emergency nurse". The least discomfort is perceived in situations described as "indirectly connected to intensive care / A&E nurse duties", such as irregular working patterns and work during public holidays. Prevention of errors during drug administration by nurse

Current status: Currently, the safety of patients is a key element in the provision of safe and high quality care. The drug administration is the main danger fro the patiens which is neccessery to be mentined. (Buchini and Quattrin, 2012). Each provider of health care services should follow specific internal regulation of medication process. Morover, it is necessary to support employee to report possible misconduct in the adverse event to the reporting system, which is used to create functional corrective measures. However, we have to realize that it is impossible to forget the role of the patient in the prevention of medication errors. Goals: The main aim of the research was to find out whether the nurses know the recommended nursing procedure of medication to patients.Not only, mentioned the most common misconduct during the administration of drugs, but also identify the most common causes of these misconductions. After that to describe preventive mechanisms for reducing the risk of medication errors. In conclusion it was necessary to point out the cooperation of patients in the process of safe medication administration. Methodology: The research was conducted using a qualitative research method. The data were collected the polling method, along with in-depth interview technique. The qualitative data was coded using ,,paper and pencil" and semantically organized into groups along with the categorization schemes. Research file: In order to archieve the highest level of objectivity the research was carried out with a group of nurses and patients only from the Surgical Department of a particular nursing unit. First, the research consisted of six nurses working in the surgical Department at the hospital České Budějovice a.s. and the second research file consisted of six clients admitted to the same Department. Due to the fact that the respondents were genrally reluctant to cooperate the researched sample is limited.
Specifics of nursing care for patients with spinal muscular atrophy

This thesis is focused on defining the specifics of nursing care for patients with disease spinal muscular atrophy. SMA is classified as an incurable genetic disease that causes progressive muscle atrophy and deformity of the body but sensory perception and intellect remain preserved. The incidence of the disease in the neonatal and childhood prevails, however the outbreak of the disease in adulthood is not any exception. Suitable nursing care is of great importance in maintaining the health of the patients for the longest possible time and delaying the onset of worsening health. The thesis is divided into theoretical and empirical part. The theoretical part deals with SMA disease, incidence, classification, treatment options and nursing care. In connection with the theme of this thesis, we choosed three targets. 1.: Map out the specifics of nursing care of pediatric patients with spinal muscular atrophy from the perspective of nurses. 2.: Map out the specifics of nursing care of adult patients with spinal muscular atrophy from the perspective of nurses. 3.: Comparison of the specifics of nursing care of pediatric and adult patients with spinal muscular atrophy. We used qualitative research method using a semi-structured interview to achieve our goals. To assess quality goals, we used the following research questions: How nurses specify the nursing care provided to a pediatric patient with spinal muscular atrophy? How nurses specify the nursing care provided to an adult patient with spinal muscular atrophy? With which needs of patients suffering with spinal muscular atrophy have the most experienced nurses experience? The research group consisted of six respondents, three of which respondents had experience with care of pediatric patients and the other three with care of adult patiens. As a processing technique we used open coding, through pencil and paper. The information obtained was divided into categories. The research shows that for nurses caring for pediatric patients the age is important at first occurrence of the disease as that influences the possibility of communication. Prevails ensure physiological needs. For nursing care, throughput of the tracheostomy cannula and status check of the oral cavity is important. In terms of nutrition the nurses prefer serving food through the PEG. They also agreed on the need for positioning. Emptying urine is ensured by the PMK or urinating in diapers. The nurses agreed on serving laxatives and manual defecation. The survey of nurses caring for adult patiens explains that the most common barrier is a state of consciousness, mental status and impaired verbal and nonverbal communication. For the needs of the nurses they state the physiological needs, as well as the fulfillment of psychological needs. In the nursing care of respiratory ways nurses report caring for tracheostomy cannula and a suctioning. In the case of nutrition they prefer per os (orally) and after that they use the help of NGS or NJS. The most common way of serving the diet they indicate the PEG. In case of immobility of the patient they all agree on positioning. For emptying urine they use PMK, one also mentioned epicystostomy. For defecation they agree on serving of laxatives, enemas and manual defecation. The results of our survey highlight the differences in nursing care provided to children and adults. The most relevant in the category of barriers in communication, is the age of the children and adults state of consciousness, the willingness and the ability to communicate. Adult patients desire to fulfill even higher requirements,while with children it is about fulfilling the physiological and social needs. In the nursing care, significant differences are not found. Importance of nurse´s work providing nurishment to senior patients

Theoretical Foundations Ageing and old age are natural parts of every individual during their lives. Most of the population wants to live an active life of the best quality possible. Nutrition is a factor that influences healthy aging most significantly. Adequate nutrition plays an important role in maintaining the quality of life and good physical condition. Nutritional needs of the elderly are highly specific. The nutritional needs of the elderly are defined mainly by changes in the organism and a wide range of influencing factors resulting from a person's lifestyle. An important role is played especially by eating habits, social and cultural environment, education, social status, economic standards and, ultimately, health and psychological condition of a person. Proper nutrition of the elderly contributes to their health and wellbeing; it is also important for recovery from the illnesses to which the elderly are more vulnerable. When providing nutrition to the elderly in hospitals, the role of nurses is important. Objective of the Thesis Four objectives were specified for the thesis: 1 To find out whether nurses follow the principles of providing nutrition to the elderly. 2 To identify the most common factors that affect nutrition of the elderly in hospitals. 3 To map the most common nutritional disorders in the elderly occurring in hospitals. 4 To find out what diet is the most common in the elderly staying in hospitals. Methodology The research qualitative inquiry was conducted in the form of covert observation. The objects of observation were the nurses working at a department of aftercare, which were observed in serving food to the elderly. The covert observations were complemented with depth interviews, also with the observed nurses working at the department aftercare. The observations were processed in Microsoft Excel in the form of tables and the interviews were processed in the Atlas.ti programme. Results The observations of the nurses in serving of food to the elderly showed that all the observed nurses failed to comply with the principles of providing nutrition to the elderly in the hospital. More or less each of the observed nurses made some mistakes. The most frequently occurring mistakes were as follows: While serving food, the nurses did not use aprons or serving overalls; the nurses did not prevent direct hand contact with the food. Almost half of the observed nurses did not provide the patients with sufficient time and the support to the patients while serving the food by the observed nurses was also inadequate. Further, nearly half of the observed nurses did not wish a Bon appetite to the patients. Also noteworthy is the latest most frequent mistake: nearly half of the nurses did not put away their aids after serving food. In the interviews, the nurses surveyed identified the most frequent factors influencing nutrition of the elderly in hospitals as follows: biological changes in old age, various types of dementia, psychological condition of the individual, co-morbidities, medication use, eating habits of the individual, the state of dentition (dental prosthesis), and the influence of the family. The most common nutritional disorders identified were as follows: absence of appetite, dehydration, excess weight, obesity and malnutrition. The most common diets eaten by the elderly in hospitals were identified as follows: diets no. 3, 9, 4, 0 and also diets in ground form, puree form and food served through an NGS (nasogastric tube). Conclusion Based on the research survey and the review of expert literature, a standard of nursing care, called "Serving food to patients with deficient self-care in nutrition", was designed for nursing staff. Nursing staff will have the opportunity to use the manual that will serve them as good practice when serving food to patients with deficient self-care in nutrition, and nurses will not consider the activity as a routine procedure.
Psychosocial working load of nurses and social workers. Quantitative survey.

I deal with problems of psychosocial working load of nurses and social workers in this final bachelor thesis. As usual the bachelor thesis is divided into a theoretical part and practical one. In the theoretical part I focused on a loading characteristic, on its types, on individual load situations. Furthermore some notions are described here such as working load, psychosocial load. I did not forget to apply herself to a nursing concept, nurse profession and as well to a social work characteristic and social worker profession. In the practical part I focused on a rate deduction of psychosocial working load and on identification of causes of psychosocial working load of nurses and social workers. A questioning method by the help of a non-standardized questionnaire was used to detect set targets. I have made up the questionnaire on the basis of my own experience and personal assumptions. The group of questioned people was formed by nurses and social workers from South Moravian Region, district Znojmo. The results have been arranged into a diagram. The research has proved that in both professions it is possible to identify the rate and causes of psychosocial working load. By choosing of this theme I wanted to pint out to psychosocial working load which nurses and social workers are daily accompanied by. This work has brought me a lot of information and thinking about these problems. I hope that it will also benefit nurses, social workers and as well their supervisors. Thinking about these problems could lead many people to a solution or even elimination of some issues connected with this given theme.
