Lucie Benešová Title: Functional movement disorders of the temporomandibular joint in women of working age Abstract: The bachelor thesis deals with functional disorders of the temporomandibular joint in women of working age. The thesis is a diagnostic work. The main aim is to evaluate the most common functional changes and general common features of patients with temporomandibular joint dysfunction by means of extended kinesiological analysis. The sub-objective is to map the quality of life and the presence of stress factors in the patients' lives. The second sub-objective is to elaborate the proposal and goals of the physiotherapy intervention. The theoretical part, in addition to short chapters describing the anatomy and biomechanics of the temporomandibular joint, deals with functional temporomandibular disorders. It provides a comprehensive overview of their causes, symptomatology and relationship to other body structures. It also summarizes the consequences of excessive psychological stress in the context of these disorders and describes conservative methods of treatment. Lucie Benešová Title: Total tempromandibular joint replacement physiotherapy Abstract: The bachelor thesis is designed as a theoretical-practical and deals with the issue of physiotherapy after total temporomandibular joint replacement. The aim of the bachelor thesis is to develop a proposal for diagnostic and therapeutic concept in physiotherapy for total temporomandibular joint replacement. The theoretical part is divided into four sections. The first is a brief description of the anatomy, kinesiology and biomechanics of the temporomandibular joint. The second deals with joint disorders, their etiology, classification, clinical manifestations and therapeutic intervention with a special focus on disorders that lead to surgical treatment. The third describes surgical process of total joint reconstruction, mentions the risks and contraindications of surgery, and describes postoperative care. Abstracts Title: Craniomandibular Syndrome - Diagnosis and Therapy from the Perspective of a Physiotherapist Objectives: The thesis deals with problems of diagnosis and therapeutic intervention in pain, that originates in craniomandibular area from the perspective of the physiotherapist, and especially functional disorders of myofascial substrate. The goal was to determine whether and to what extent can be the pain relieved, through selected therapeutic methods (soft tissue techniques, mobilization techniques, positive thermotherapy, individual LTV). Further more, if the pain control (to alleviate its intensity) is effective and whether it is achieving lasting effects even in periods without therapy. Another question was whether the method of pressure algometrie (PPT) correlates with subjective pain level marked on the VAS scale. Methods: The study itself was elaborated in qualitative form. The examined group consisted of 10 consenting women with pain in the craniomandibular area, who had no diagnosis by neurologist, a dentist and an otolaryngologist. The comparison group was not established, since it was not a comparison of different therapies, but the effect of therapeutic techniques on pain reduction, thus subjects with pain could not remain untreated. Physiotherapy as part of comprehensive treatment in patients with acquired temporomandibular joint dysfunction

The subject matter of my undergraduate thesis is acquired dysfunctionality of the temporomandibular joint. The concept incorporates a number of funcional and structural changes and pathological conditions affecting not only the mandibula as such but also the overall posture of the organism as a whole due to sequential spasms. Temporomandibular disorders have a multifactor etiology and very diverse clinical image, which often makes diagnostics quite difficult. Generally, increasing psychic tension, emotional stress, and occlusive relations are believed to count amongst the main causes. According to some studies, up to 50 % of the population suffers from some type of disorder of the temporomandibular joint. As treatment of these conditions can be problematic and time-consuming, both the pacient and the therapist have to be patient. The theoretical part of my thesis concentrates on mapping the problems arising from dysfunctionality of the temporomandibular joint, whereas the practical part contains causalistic elaboration of two female patients diagnosed with this condition. My research was conducted using the method of qualitative causalistics. The first female patient was diagnosed with degenerative changes in the right mandibular joint causing diminished mobility of the lower jaw, affecting the functionality of the patient{\crq}s right ear and body posture. The second female patient suffered functionality disorder in the form of dislocation of the mandibular joint{\crq}s disk, asymmetric movement of the jaw when opening her mouth, and irregular stereotype of breathing. In both patients, the therapy was adjusted to the diagnosis and each patient{\crq}s momentary condition. Primarily, their treatment was focused on their hypertonic muscles and sequential spasms, followed by relaxation of their chewing muscles, training of symmetrical mouth opening, breathing stereotypes, and sensomotoric elements. The patients were instructed how to do suitable autotherapeutic exercises. The therapeutic sessions took place twice a week over a six-week period. In both patients, the results were satisfactory. With the first patient, her ability to open her mouth and her overall posture improved. With the second patient, the unpleasant noise phenomenon when opening her mouth was removed and the asymmetry of mouth movement was diminished. Moreover, she learned to adopt a regular breathing stereotype. My thesis can be useful in the clinical practice of physiotherapists, physicians, and other medical personnel, as well as a source of information for patients.
