Analysis of Selected Indicators of Hospital Jindřichův Hradec, a.s. Using Time Series

Subject of this bachelor‘s thesis is analysis of selected indicators of Hospital Jindřichův Hradec, a.s. using time series. In the theoretical part are explained basic backgrounds, which are needed to make successful analysis. Practical part of the work is focused on the analysis of particular indicators, together with the possibility to provide their prognosis for the future period. Contribution to this work and results of the analysis are summarized at the end.
Physical examinations for adults performed by nurse in practitioner´s ordination.

This bachelor thesis deals with physical examination, the monitoring of physiological functions and the recording of cardiographs by nurses in private practice examination rooms. The thesis contains two parts. The first, and theoretical, part of the thesis is focused on the explanation of certain terms and a description of what it means to provide a physical examination, monitoring of physiological functions and recording cardiographs. The physical examination is focused around human senses. Sight (inspection), hearing (auscultation), touching (palpation), knocking (percussion), per rectum examination, and smelling are all considered methods of examination. Body temperature, breathing, pulse and blood pressure are considered physiological or vital functions. The electrocardiogram signifies cardiac activity in the form of a line, which is drawn by a gadget called an electrocardiogram (ECG). Activities executed by nurses in a private practice exam rooms can be divided into four groups: preventative, diagnostic, medical and administrative group. A nurse provides patients with medical educative materials, samples biological material, monitors clients physiological functions, gives injections, changes bandages, sends preventative examination invitations, orders medical material and equipment etc. The second part contains investigative research. The empirical investigation was performed using the qualitative method in a form of a semi structured interview and direct observation during the interview. In an interview with eight nurses, who all work in private practices, the nurses were asked 15 basic questions. The nurses were observed as they each examined a separate patient. The observation was done based on 32 observation criteria. The observation results were compared with the results of the interviews. Based on their close relation to physical examination, monitoring of physiological functions and cardiograph was added.
Specificity in the provision of nursing care before and after thyroid surgery.

After diabetes mellitus we encounter thyroid disease as the most common disease of the endocrine glands. The most common diseases of endocrine glands are inflammations, tumors, struma or thyroid enlargement, thyroid hyperfunction and hypofunction. Indications for surgery are among others malignant and benign thyroid diseases. In the postoperative care both general and specific principles are applied, which are conformed to the condition of the patient, the type of surgery and the surgeon requirements. Providing the right nursing care should also help the prevention of possible postoperative complications. The bachelor thesis named "Special features in providing nursing care before and after thyroid surgery" is divided into two parts, theoretical and empirical. The theoretical part of the bachelor thesis deals with thyroid anatomy and physiology and its surroundings, its need for iodine, it describes individual thyroid diseases, thyroid disease diagnosis and treatment. Furthermore, it also deals with the nursing care before and after thyroid surgery, the emergence of possible complications and last but not least it deals with home care after the patient is discharged from hospital. The aim of this bachelor thesis was to detect the pecularities of providing nursing care before and after thyroid surgery and to determine regime measures and the knowledge of these in patients after thyroid surgery at the surgical department. Research questions were created to achieve the objectives. Q1: What are the pecularities of the nursing care before thyroid surgery? Q2: What are the pecularities of the nursing care after thyroid surgery? Q3: What are the regime measures in patients after thyroid surgery? Q3: What knowledge do the patients have about the regime measures after thyroid surgery? To obtain necessary data a semistructured interview method was used. To process this thesis a qualitative research was carried out. Semi-structured interviews were used as a technique for data collection. Categorization was compiled on the basis of interviews. A research survey was carried out in the surgical ward, where there are patients with thyroid disease. The research showed that the nurses are aware of care principles and they are sufficiently informed about treating a patient with thyroid disease. The interview showed the principles of nursing care, which should be followed throughout the whole period of hospitalization of patients with thyroid disease. In the interviews the nurses emphasized the differences in providing nursing care in a patient before and after thyroid surgery. Furthermore the research showed that nurses would be in favour of further training seminars that would deal with the given issue, about treating a patient with thyroid disease. Awarness and knowledge of the regime measures was also recognized in patients after thyroid surgery. Based on the data obtained from the interviews with patients, those report that their awareness and also knowledge of regime measures after thyroid surgery is minimal and insufficient. The result of the bachelor thesis was to determine the principles of care for patients with thyroid disease. Principles of nursing care were detected in patients before and after thyroid surgery. Furthermore, regime measures were identified in patients after thyroid surgery and also the knowledge of those regime measures in patients after thyroid disease. This bachelor thesis outlines the pecularities in nursing care of patiens with thyroid disease, policy of regime measures after a patient's discharge from hospital and keeping to those is among others a condition for early patients recovery and his return to a normal life. The role of nurses in meeting dietary guidelines for adult patients with coeliac disease.

This thesis deals with the education of patients with coeliac disease.Two objectives were set within the survey. The first objective was to ascertain the degree of awareness of patients with coeliac disease educated by a sister. The second objective was to explore the educational methods and the scope of information provided by a sister to a patient within an education about a gluten-free diet. The method of qualitative research using the technique of in-depth interview was used in this work. The research sample consisted of 5 nurses and 5 patients. The investigation was conducted in the Capital City of Prague on outpatient departments of gastroenterology and on departments of allergology and imunology. Through the detailed analysis of the data obtained from the respondents, 6 categories were created: the use of educational methods from the perspective of nurses and patients, the use of educational aids from the perspective of nurses and patients, the time range of an education from the perspective of nurses and patients, the content of education by a sister from a patient's perspective, the content of education on a gluten-free diet from a patient's perspective, education of nurses in the area of coeliac disease.
Factors influencing the satisfaction of patients with provided nursing care

Current state: The satisfaction of patients is an important factor for the quality of nursing care. Nursing care should be mainly focused on meeting the needs of patients and many other factors. This greatly affects the patient's subjective feeling of satisfaction or dissatisfaction with either the proper nursing care, or other factors. The patient is an active participant in the treatment, not a passive recipient of nursing care. Measuring the satisfaction of patients is one of the requirements for the accreditation of hospitals. Research objective: The aim of the research was to determine the satisfaction of patients with the provided nursing care. Another objective was to demonstrate a statistically significant relationship between the satisfaction with nursing care and the impact factors: sex, age, education, environment and the routine on a standard ward, communication with patients and nurses attitude of nurses to patients. Methodology: The research was realized using quantitative methods. For my research I used a standardized questionnaire called Quality seen by patients - evaluation of inpatient care, which included 34 questions. The questionnaire for the assessment of patient satisfaction was compiled into several areas: satisfaction of patients with the hospital stay, with the nursing care provided, with leaving the hospital, with the general assessment of the health care provider. The survey covered the period from January to March 2015 at the standard surgery and internal medicine department in the hospitals of České Budějovice and Český Krumlov. For the survey I selected the patients who were hospitalized in these departments for at least 3 days. The data obtained by a quantitative research were later tested by SPSS software. For statistical testing we selected the chi-square and the tick chart. The significance level was set at 5%. Participants: As part of the survey, a total of 200 patients were approached. 100 patients were interviewed in the hospital of České Budějovice in surgery and internal medicine departments. The remaining 100 questionnaires were distributed to patients in the hospital of Český Krumlov, again on the internal and surgical wards. The total return of the questionnaires was 100% (200). Results: The research shows that men are less satisfied with the received care than women. An important factor in patient satisfaction showed to be the education of the respondents. University-educated patients were significantly less satisfied with nursing care than respondents with secondary and primary education. Another examined factor was the sleep schedule. Approximately one fifth of the surveyed patients (22%) stated that noise caused by other patients was disturbing. The respondents who indicated that were disturbed by the noise in the department, rated the satisfaction with nursing care significantly worse. Conversely, the patients expressed satisfaction with the wake-up time. In the area of the patient satisfaction with the cleanliness of the rooms and toilets, the research survey showed that satisfaction decreases significantly if the patients are not satisfied with the cleanliness of the toilets and rooms. The environment, in which the patients are hospitalized, greatly influences their satisfaction with nursing care. Another important factor for the patient satisfaction is also the communication and the attitude of the nurses to them. If the attitude of the nurses is compassionate, soothing and they pass information to the patients in an understandable way, the patients have more confidence and are more satisfied. This is true when the patients meet their needs and feel that they are treated with dignity and respect. Conclusion: The research shows that these factors affect the satisfaction of patients with nursing care and in the context of these results the importance of highly individualized care is increasing. Nurses must significantly contribute to improving patient satisfaction.
Nurse?s role in the care of patiens suffering from disfunction of lower urinary tract.

The role of a nurse in the care of patients with dysfunction of lower urinary tract is one of the most urgent topics of modern urology, gynaecology and neurology. With regard to the latest results of epidemiological studies pointing out high prevalence in the population this problem has been in the centre of attention recently. Dysfunction of lower urinary tract is defined as a complex of symptoms. A quantitative research was performed to obtain the necessary information, employing the method of questioning with the technique of non-standardized interview. 8 patients with dysfunction of lower urinary tract and 8 nurses working at urological wards and outpatient departments in the South Bohemian Region were invited to participate. The following goals were set to obtain the necessary information: 1. To find out how patients with dysfunction of lower urinary tract gain information. 2. To find out how well patients with dysfunction of lower urinary tract are informed. 3. To point out the role of a nurse in the care of patients with dysfunction of lower urinary tract. The following research questions were set to reach the goals: 1. How do patients with dysfunction of lower urinary tract gain information? 2. What information is available to patients with dysfunction of lower urinary tract? 3. In what activities do nurses participate in the care about patients with dysfunction of lower urinary tract? The research has shown that the population is not well informed about this dysfunction. If the complex of symptoms occurs the affected persons seek help with general practitioners, gynaecologists or paediatricians and if the problems endure they are sent to a check up at a urological department. However there are lots of patients who procrastinate to go to a check up for months or even years. The role of a nurse in the care of these patients was also examined among experienced staff. The results may help nurses improve the quality of the care provided to patients with dysfunction of lower urinary tract. The obtained information may also point out the mistakes that should be avoided in cooperation of a nurse with a patient; the information from the research may serve for preparation of an educational plan.
Approach and responsibility of today's patiens to cataracta disease.

Cataract - Cataract is a disease of the eye lens and can affect all age groups. If left untreated, it can significantly impair vision or lead to blindness. In the theoretical part, I tried to clarify what the cataract actually is, what types of cataract do we have, examination methods, diagnostics, treatment, and nursing care in patients with cataract. In the second part, I tried to explain what the patient awareness is, and how important it is to inform patients about this disease, preoperative and postoperative care as well as after-care in home environment. In the practical part, quantitative survey in a form of questionnaire has been used. The survey took place at the Departmen of Ophthalmology, Jihlava Hospital. I have created two types of questionnaire, for nurses as well as for patients. Results were recorded in bar charts. Three hypotheses are determined for goals. Hypothesis No 1 - younger and younger patients undergo cataract surgery. Hypothesis No 2 - patients are more responsible and well informed. Hypothesis No 3 - level of cooperation with healthcare staff coresponds to anticipations of patient with ocular disease. All three hypotheses were confirmed. Even though the hypothesis No 2 has been confirmed, some patient answers do not indicate it. It is definitely necessary to continue putting stress on awareness, have enough time to inform patients and answer questions of all patients. Results of my work can lead to creation of time space for communication with patient; ensure synoptic and sufficient quantity of information material adequate to the patient needs.
Ombudsman status and his role in medical organizations

In my thesis I would like to inform the public not only about the office of ombudsman as such but also about the special ombudsman{\crq}s office for public health service, the tasks that he is in charge of and the fact how he can be helpful to people. The objective of my research was to find out how the public perceives the office of the ombudsman for public health service, what issues the ombudsmen deal with within the scope their offices in the health care facilities and to map the frequency of ombudsmen{\crq}s offices in the health care facilities within the Czech Republic. The hypotheses, which I have predetermined, were as follows. The public does not have enough information on the office of the ombudsman for public health service; the most frequent issues, which the ombudsman for public health service settles, are the grievances of patients concerning the behaviour of clinicians and nurses; and teaching hospitals have established an office of the ombudsman for public health service. Through my research I have learned the following facts. The office of the ombudsman for public health service has been established in six health care facilities of the Czech Republic. It includes four teaching hospitals and two non-governmental health care facilities. There is no legal regulation stipulating the necessity to establish an office of ombudsman in the health care facilities. The issues most frequently settled by the ombudsmen concern the improper communication of medical staff with the patients and their relatives or visitors. Another problem is the patients{\crq} dissatisfaction with the fact that the medical staffs does not properly or fully inform them, as well as the issue of too long waiting time in the emergency facilities. Using a questionnaire to address the public I also found out that almost three fourths of informants have never heard the term of ombudsman, they do not know what tasks he is in charge of and have no idea that such and office exists in some health care facilities in the Czech Republic. The same number of informants would like to obtain more information on the issue of the ombudsmen in the health care facilities and the public evaluates the current awareness of this issue as insufficient. More than a half of informants consider the office of the ombudsman for public health service as useful and think that establishing of such an ombudsman{\crq}s office results in a higher quality of health services.
