Abdominal wall activity in patients indicated for total hip replacement compared to the population without advanced coxarthrosis

Introduction: The aim of this study was to explore the relationship between postural stabilization insufficiency and severe coxarthrosis indicated for total joint replacement. Postural stabilization insufficiency was assessed by abdominal wall activity. Methods: 51 participants were recruited. 26 patients with hip arthrosis (13 males and 13 females) were included in the experimental group (mean age 70.5 years, SD 9.1) and 25 participants (10 males and 15 females) in the healthy control group (mean age 70.5 years, SD 7.7). The Ohmbelt device can be used to assess the intra-abdominal pressure indirectly via abdominal wall expansion measurement. The pressure sensor within the Ohmbelt device was attached to the abdominal wall at the area above the inguinal ligament (anterior sensor) and contralateraly at the trigonum lumbale (posterior sensor) by an adjustable belt. The pressure exerted by the abdominal wall wS monitored by the sensors. All subjects were measured twice (once with the anterior sensor, the second time with the posterior sensor) in 3 postural situations - resting breathing in the upright sitting position, the diaphragm test, and holding a 2kg dumbbell in front of the body in the upright sitting position. Correlation between evaluation of functional postural tests and ability to expand abdominal wall in low back pain population.

The theoretical part: The theoretical part of thesis shortly describes anatomy and kinesiology of lumbar spine. The second chapter contains the causes of LBP and alerts to red flags. Muscle stabilization system of spine and intraabdominal pressure are themes of other chapters. Techniques of intraabdominal pressure measurement and questionnaires of quality of life were described at the end of the theoretical part. Methodology: 36 patients (22 female, mean age = 41.86, 16 male, mean age = 40.07) with low back pain were evaluated in three positions. Every proband filled Oswestry Disability Index (ODI) before the examination. Trunk orthosis DNS Brace evaluated 8 seconds intraabdominal pressure in three tests - breathing stereotype test, diaphragm test and hip flexion test. DNS instructor evaluated the same three tests by aspection and palpation. The first half of the probands was evaluated first with DNS Brace, the second half of probands was evaluated first with DNS instructor. Results: A statistically significant correlation (r=0,479, p=0,003) between evaluations DNS Brace and DNS instructor was demonstrated in one out three DNS tests - diaphragm test. The objectification of postural function of abdominal muscles

This thesis evaluates the possibility of using objective measurements to assess the postural function of abdominal muscles. The theoretical part provides an overview of findings regarding postural function of the abdominal muscles and the context for their co-activation using intra-abdominal pressure. The results are summarized in the form of research studies relating postural function and low back pain (LBP). The overview of the methods used to measure intra-abdominal pressure and the abdominal muscle activity is the main focus. In this section, we present a new methodology for measuring the activity of the abdominal muscles by using pressure sensors attached to the abdominal wall in the areas of the groin and the lumbar triangle. Methodology: In the experimental part, we tested 35 healthy subjects (average age 21.26t, SD ± 1,62) in 3 posturally different seated scenarios: 1. resting breathing, 2. with added external load (the subjects held a dumbbell 20% of their body weight) and 3. with maximum voluntary increase in intra-abdominal pressure (the diaphragm test). Results: An independent samples t-test indicated that the pressure created by the abdominal wall for both sensors in situation resting breathing increases in situation external load (HA1: upper sensor p=0.0079, lower sensor p=0.0009).
