Evaluation of effect of mobile application CF Hero on compliance of inhalation treatment and respiratory physiotherapy of pediatric and juvenile patients with cystic fibrosis

This diploma thesis deals with the adherence to inhaled treatment, its measurement, telemedicine and telerehabilitation and the possible use of the CF Hero mobile app in paediatric patients with cystic fibrosis. In the theoretical part, it presents knowledge about cystic fibrosis, adherence to treatment in patients with cystic fibrosis and possibilities of its measurement, telemedicine and telerehabilitation. The practical part describes the CF Hero mobile app, which has been the target of measurements on several patients. At the same time, a questionnaire was developed focusing on satisfaction with the use of this mobile application. Methods: The practical part was conducted as a multicentre prospective cross-over study investigating the effect of the CF Hero mobile app on adherence to inhaled treatment, respiratory quality of life and chest excursion The evaluation was conducted over a 6-month period, with the primary objective being to measure the amount of exhaled solution relative to the prescribed amount. Simultaneously, each patient underwent 3 identical examinations. Martina Havlová Opponent: Title of bachelor's thesis: Techniques of Respiratory Physiotherapy in inhalation Therapy of Chronic Pulmonary Disease Abstract: This paper is focused on the techniques of respiratory physical therapy in inhalation therapy of chronic pulmonary diseases. The paper is divided into two parts - theoretical and practical. The aim of the theoretical part is to briefly describe anatomy and physiology of respiration. Next, the theoretical part deals with chronic obstructive pulmonary disease and possibilities of its treatment. The emphasis is put on the inhalation therapy and the description of particular techniques of respiratory physical therapy which are used in the therapy of chronic pulmonary diseases. The practical part includes two case histories of patients who are being treated for chronic obstructive pulmonary disease. To evaluate the state of the patient, spirometry testing and the MMRC dyspnea scale are used. The aim of this paper is to show the influence of respiratory physical therapy on efficacy of inhaled medicine concerning the patients with chronic pulmonary diseases. Issue of nursing care within patiens reliant on domestic oxygen therapy and inhalation therapy.

Theory basis: Oxygenotherapy is a method using the oxygen for the treatment. In present time is not usable only in the medical facilities, but also in the home enviroment. Important fact is, that there's a certain life quality improvement at all aspects due to home oxygen therapy. This thesis is focused on issues of the nursing care of the patients dependent on home oxygen therapy and inhalation treatment. Theoretical part of thesis starts with enlightenment of oxygen therapy, utilization of the oxygen and its source. Other chapters of thesis are focused on terms such as oxygen therapy, diseases treated by this method and patients indication for the home oxygen therapy, followed by available sources of the oxygen in the Czech republic, nurse procedures for oxygen application, oxygen therapy equipment and important regulations and policies for oxygen manipulation. There's also mentioned procedure to approach a proper effect of the oxygen therapy and knowledge given by a nurse with the patient release into the home environment. One chapter is dedicated to the nursing care of the patients dependent on home oxygen therapy and inhalation treatment. There's a section in this part touching a care problems with patients. Theoretical part of thesis is closed by a chapter about inhalation treatment, which is used by the patients dependent on home oxygen therapy. Two objectives of the research, and two research questions were defined for this thesis. First goal was to determine which informations are given by nurses to the patients dependent on home oxygen therapy and inhalation treatment. Second goal was to determine what are the most common problems of patients dependent on home oxygen therapy and inhalation treatment. The method of research: Empirical part of this thesis is done by qualitative method in the form of half-structured interview. Results: Three main categories were defined based on data retrieved from interviewees. Categories are divided into subcategories. In the first main category, named "Nurses cooperation", was found, that nurses are cooperating with patient's family, practitioner, pulmonologist, physiotherapist, technical support and homecare agency. From the second main category, named "The range of the patient's awareness", research show us, that nurses inform patients about policies for oxygen manipulation, strict smoking prohibition and the need of periodical check from practitioner. Subcategories in this category are "Introduction to oxygen concentrator", "Oxygen therapy equipment", "Policies for oxygen manipulation" and "Methods of breathe rehabilitation". In subcategory "Introduction to a oxygen concentrator" was found, that nurses are not informing patients. In subcategory "Oxygen therapy equipment" was found, that most interviewed nurses are using informational leaflets except one of the nurses, which is not using any helpful materials. In subcategory "Policies for oxygen manipulation" was found, that nurses are informing patients about restrictions to work with greasy hands, avoiding open fire and strict smoking prohibition. Some of nurses also mentioned policies for the oxygen concentrator, which was a surprise for us. In last category, named "Methods of breathe rehabilitation", research show us, that nurses are recommending aids such as Treshold PEP, Accapela and breathe trainer. Nursing of the patient with asthma bronchiále

This Bachelor thesis deals with the asthma bronchiale which is a chronic inflammatory disease of the upper air passages connected with their structure changes. This disease afflicts all age categories and its occurrence is increasing worldwide. The theoretical part of the work describes causes, symptoms, diagnostics, treatment of the disease and education. A part of the work is devoted to the role of the nurse and to needs of clients suffering from asthma. The advantage of the disease is the fact that its development is possible to be influenced to a large extent by early diagnosis, by proper treatment and by following regime precautions. Therefore clients suffering from asthma need not only a lot of quality information about the disease and its treatment, but also proper education in the field of inhalation techniques and regime precautions. On the other hand the nurse who has good knowledge of specific needs of a client suffering from asthma bronchiale can help him a lot in the course of her nursing. The aim of the Bachelor work was to find out if the clients suffering from asthma bronchiale have some specific needs and on the basis of the research results to prepare information brochure for the clients with this disease. The research questions were answered by means of the qualitative research using the form of the semi-structured interview which was elaborated into casuistry. The essential data are written in the categorizing charts. The research set consisted of eight respondents suffering from asthma bronchiale. Results of this Bachelor work show that the clients have enough information and they get them particularly from their doctor and from the Internet. Nurses do not provide patients with information quite a lot. The found out specific needs can help the nurse to understand problems of this disease better and to improve the nursing process. As a result of this research there was compiled an information brochure which can provide both the asthmatics and the general public with the most important information. This work can be a good source of information not only for clients suffering from asthma bronchiale but also for medical students and for nurses.
