Attitude of midwives towards physiotherapeutic interventions during pregnancy, childbirth and puerperium

The purpose of this thesis investigates the attitude of midwives towards physiotherapeutic interventions during pregnancy, labour and puerperium. Two goals were defined for the thesis. The first goal was to investigate the midwives' interest in the inclusion of the selected methods to their practice and the second objective was to verify the midwives' interest in multidisciplinary cooperation with a physical therapist. The conducted research was based on a quantitative survey via a questionnaire of author's own construction. The questionnaire was distributed in electronic form. The research group consisted of 196 respondents, of which 195 were women and 1 of which was a man. Most respondents belonged to the age group between 25 and 36 years. 75 % of the respondents had a university degree, 11,2 % respondents stated that they graduated vocational school and 13,8 % respondents stated that that their highest reached level of education was highschool. The research showed that midwives are interested in inclusion of the methods in their practice and in multidisciplinary cooperation with a physical therapist. The midwives younger than 35 years showed higher interest in increasing their education than midwives older than 35 years. Correlation between the respondent's age and use of the individual methods was not proven by the research. The results showed that 94 % of the respondents considered multidisciplinary cooperation as important. 79,6 % respondents were interested in multidisciplinary cooperation with a physical therapist. The physical therapists that worked on the respondents' workplace mostly dealt with verticalization and scar care in women after caesarean section and in women after gynecological operations. The respondents themselves used the excercises of getting up sideways the most (93,9 %), verticalization (86,7 %) and breathing excercises. Respondents stated that they felt the greatest increase in the involvement of midwives in rehabilitation care is neeeded in birth injury and scar care (115x). The results can be presented at professional conferences for midwives.
The forms of regeneration and its contemporary trends

The thesis comprises a comprehensive summary of particular types of regeneration and their contemporary trends. The introduction focuses on regeneration and relaxation in general, and then the attention is given to the mechanotherapy- massage, its types, health effects and history. The heat therapy which is specialized in sauna and its new trends is connected with the mechanotherapy. To the heat therapy belong also packs and compresses. The hydrotherapy and its several types are characterized in the next part. Further this work attends to the electrotherapy and phototherapy. The last part of the research deals with the cupping therapy, acupuncture, oxygen therapy, salt chamber and vacuum carbonic acid bath. The result of the research is a complete list of types of regeneration designed for the general public.
The role of nurses in the care of chronic wounds by using alternative methods and materials of moist wound healing

This bachelor?s thesis deals with the actual classification of wounds and the issue of the chronic wound treatment. In the preface of the theoretical part of the thesis, the term wound is defined and the wounds are subcategorized in the terms of their pathological traumatological origin. In the next chapters, the process of the wound healing is analysed from molecular biological aspect, the terms acute and chronic wound are defined and newly introduces the term non-healing wound for the chronic wound. In the special part, the particular pathological-surgical types of chronic wounds are described from their molecular biological, surgical anatomical and therapeutic point of view. Next, the paper specifies risky factors influencing wound healing process aiming their therapeutic influenceability. These factors are further divided into the system ones and the local ones. The thesis continues with the analysis of the therapeutic and nursing possibilities in the chronic wound treatment describing traditional bandages including sterile pieces of gauze and disinfectant solutions for the materials of the moist wound healing. This is followed by the chapters discussing modern methods of the debridement of the chronic wound such as ultrasonic hydrotherapy, negative-pressure wound therapy or larval therapy. Each chapter describes both the indications and the contraindications for the use of these methods, their advantages and benefits for the patient, aiming the role of the nurse in the chronic wound healing process using these methods. The end of the theoretical part of the thesis is devoted to the nutritional support and the systemic enzymotherapy, which ensures a sufficient energy intake to the organism and both all the macronutrients and micronutrients necessary for building the body tissues. The empirical part of the thesis is in the form of a qualitative research focused on the documentation of the role of the nurse in the care of the patients with a single type of the chronic wound ? more precisely the patient after laparotomy with an extensive resection of the digestive system and a subsequent secondarily healed chronic wound. The method of using case studies was used to accomplish this goal. The record of the chosen group of the patients and the process of the systemic therapy with the support of the cardiovascular, ventilatory and nutritional functions regarding the surgical performance is acquisited and it is completed with the description of the local therapy of the chronic wound, with the description of the role of the nurse in the healing process and with the enclosed photo documentation. Casuistries describe the regression, the stagnation or the progress of the wound healing process; they map the role and the participation rate of the nurse in this process and the use of alternative equipment in this process and the art of improvisation. The summary of the results brings up the conclusion that the scientific and technological progress opens up new possibilities to the patient with the chronic wound and introduces modern technological procedures into the chronic wound therapy though there is a group of patients after laparotomy surgeries with the conducted extensive resection of the digestive system and with the subsequent anastomosis dehiscence, which is contraindicated for the use of these methods. These patients are left to depend on the holistically provided care of a multidisciplinary team with irreplaceable role of the nurse. Biopsychosocial needs of the patient puts great demands on the nurse regarding both the personal and the professional side. The reasoning at the end weighs up and suggests the possibilities of the participation of the nurse in the preoperative preparation of the patient for the planned resection of the digestive system mainly in the form of the education in the support of the nutritive functions and the systemic enzymotherapy.
Experience of midwives in water births

Childbirth is a very important moment in the life of every woman. Expectant mothers very carefully plan and think about the way and place of delivery of their baby nowadays. They start to be interested in more natural and more alternative childbirth. One of the options is also a water birth. The theoretical part is focused on a summary of knowledge in the area of childbirth, alternative methods of childbirth and water birth, which the Czech Gynaecological and Obstetrical Society accepted in 1999. The practical part is focused on knowledge and experience of midwives in selected hospitals practising water births. For analyses of the relevant matter and finding out the set targets there was chosen a combination of quantitative and qualitative research investigation. The quantitative form of the research was carried out in form of an anonymous questionnaires intended for midwives in the Hospital of České Budějovice, a. s., Český Krumlov, a. s. and the University Hospital in Plzeň. The qualitative form of the research was carried out in form of non-standardised interview with midwives experienced in water birth in the Czech-German Mountain Hospital Krkonoše, s. r. o. There were set 5 targets and related hypotheses. The aim of the work was to find out, whether midwives are informed about water births in selected hospitals, whether they are provided the necessary equipment of delivery rooms for practicing water births and further to find out attitudes of midwives towards water births in selected maternity hospitals. The aim of the work was also to find out whether midwives in the selected hospitals are interested in assisting water births and last but not least whether they are experienced in water births. All targets were met. Hypothesis 1, which is called ?Midwives are informed about water births in selected hospitals?, was proved. Also hypothesis 3 about midwives having negative attitudes towards water births in selected hospitals was proved. Other hypotheses that midwives have the necessary equipment of delivery rooms for practicing water births in selected hospitals, that they are not interested in assisting water births in selected hospitals and that midwives are experienced in water births in the selected hospitals, did not prove. This Bachelor?s Thesis may serve as a source of information and study material for other classes of midwives. This thesis could be further used as an education material for midwives in practice as well as for expectant mothers considering a water birth.
Swimming for disabled children in České Budějovice and in Jihlava

Abstract The bachelor thesis, which is called {\clqq}Swimming for children with disability in České Budějovice town and in Jihlava town`` is aimed to a swimming realization for the children with disability in the civic associations named KONTAKT without barriers (hereinafter only: KONTAKT wB) and Vodomílek Sport Club (hereiafter only: Vodomílek SC). The theoretical part characterizes an impact of the water surroundings, the importance and the use of swimming for the children with disability. Furthermore the hydrotherapy, the hydro-kinesiological therapy, the suckling swimming and the swimming classification are specified in that part. The aim of my work has been to find out, how the swimming for the children with disability in both the already mentioned associations is realized. The aim was reached by means of a structured conversation to parents and to the children from KONTAKT wB and from Vodomílek SC, and also by means of semi-structured conversations to specialists of the given sphere. Furthermore the research was completed by watching of the children with disability during their swimming lesson. Comparison of the theoretical part and the practical part show the swimming itself enables the children with disability their multilateral development and it increases their self-activity. It also contributes to their socialization and to their resocialization. It teaches the human being to get to know himself, his body and his possibilities. Furthermore it increases his fitness and his organism resistance. It is proved the swimming lesson realization in both associations corresponds to the parents´ideas of the children with disability. The swimming itself is mainly realized by volunteers, who have to be empathetic, persevering and firm. That can also be practised by the parents themselves last but not least. It is clear from the results that the way of the swimming realization in KONTAKT wB and in Vodomílek SC is not the same. The common feature is their individual approach to the children as well as their typical swimming lesson configuration. The difference rests in the teaching method, which both the associations purvey. KONTAKT wB proceeds according to their own methodical line, Vodomílek SC combines various teaching methods. Results of that research will be used as the better understanding of the swimming purpose for the children with disability and will help promote that sports activity. The theoretical part and the research can be used as the knowledge summary related to problems of the children with disability. It can be used by parents, volunteers, swimming coaches and others, who want or need to get more detailed pieces of information related to that theme.
