Comparison of chosen nursing in prevention of immobility syndrome in a senior nursing home in Germany and the Czech Republic for lying immobile patients.
ČERNÁ, Lucie
As the population is getting older there is increased number of geriatric patients. However there is also increased number of immobile seniors, who are not able to take care of themselves. Comprehensive health and social security is being provided in homes for elderly, which fulfil the role of long-term institutional care. Nurses must be trained for this highly professional and knowledgeable nursing care aimed at geriatric problems. For geriatric clients of home for elderly it´s important realization of prevention, treatment and social measures in the form of an individual approach. That these measures can be implemented it is necessary to evaluate the health status of seniors, their physical performance, self-sufficiency and mental functions in connection with the social situation and to formulate a nursing diagnosis. As a result of long-term bed rest can lead to health complications affecting all body systems, so-called immobilization syndrome. How to prevent secondary damage that threatens the patients White Theo arge already ill is focused rehabilitation nursing. Among rehabilitation resources in the prevention of immobilization syndrome in nursing practice arge included: positioning, verticalization, mobilization. Inseparable part of prevention measures is preventing health complications in case of immobile clients homes for the elderly arge suitably selected positioning aids, mobilizing resources, well-equipped bed with antidekubitus system. In the thesis there were two objectives. To Compare keeping preventive measures which prevent the formation of pressure ulcers and muscle contractures in nursing care for immobile patients in the Czech and German retirement homes. To map out the possibilities of aids in the prevention of complications in immobile clients in the Czech and German retirement homes. Bachelor thesis has been made by using qualitative research. For collection of data was used a method in-depth interviews with four Czech and four German nurses. This method involved non-standardized interviews through open-ended questions that were recorded by dictaphone and transcribed verbatim. The data analysis was carried out by hand coding or using a pencil and paper. By analyzing the data four categories were created, each divided into the home for the elderly in the Czech Republic and the home for the elderly in Germany: 1.Nurses knowledge ,2.Implementation of preventive measures , 3.Equipment for immobile clients, 4.General knowledge related to immobile clients. Research survey found out that there is not a big difference in adherence to preventive measures to prevent the formation of decubitus and muscle contractures in nursing care for immobile patients in Czech and German homes for the elderly and comparable are also the possibilities of using tools to prevent complications in case of immobile clients. The biggest gap in nursing care for clients is bad knowledge of contractures in both homes for the elderly. Also keeping documentation and records relevant to the implementation of preventive measures in the Czech home for the elderly is not satisfactory. Mobilization record is missing and positioning record should be more detailed. We therefore propose the creation of positioning and mobilization recording modeled according to the German retirement home and handing it to the leadership of retirement home in the Czech.
Nursing process and its significance to seniors according the perspective of retirement home nurses
FRÜHAUFOVÁ, Hana
Modern nursing care is characterized by seeking, planning and satisfying needs of a healthy or an ill person. All this is performed by means of the nursing care process. The nursing care process is considered the basic scientific method of advanced nursing care leading to solution of problems. It has already become an inherent form of individual care planning. Thanks to the nursing care process a nurse has a holistic view of a patient and is able to provide quality care. Nursing care is provided in compliance with the nursing care process principles with regard to the specifics of geriatric care in senior homes. A senior home nurse takes into consideration the specifics of the individual phases of nursing care process in seniors. Senior homes provide residential services to people with lower self-sufficiency particularly caused by higher age and to those who need regular care in the field of self-sufficiency and personal care. Unfavourable social situation does not allow these people to live in their domestic environment. Complex care provided in senior homes substitutes for domestic environment. Seniors are also given health care provided by general nurses. The aim of the thesis was to examine nursing care process application in a senior home and the importance of the nursing care process from the point of view of senior home nurses. The following research questions were set: Which nursing care process phases are applied by nurses taking care about seniors in the senior home? How do senior home nurses feel the importance of nursing care process? Qualitative research by means of in-depth interview and questioning of nurses in a selected senior home in South Bohemia was chosen for collection of necessary information. The interviews were recorded and subsequently literally transcribed. The transcribed interviews were deeply analyzed by means of paper and pencil method by open coding. Four categories were created from the data analysis: Nursing care process, Nursing care process in practice, Nursing care process phases and Compilation of nursing care diagnosis and maintenance of nursing care documentation. Subcategories, where the core data are coded, are assigned to the individual categories, everything is illustrated by clear diagrams of the individual categories. The research shows that nurses have general knowledge of nursing care process. They were able to define nursing care process, to name its phases and to express their attitude to this method. They actively apply the process, but they say it is demanding for them from time and administration point of view. The fact that the knowledge of nursing care diagnosis is very poor among nurses is surprising. With regard to the limited awareness of nursing care diagnoses I propose a seminar explaining this issue.
Quality of Senior´s Life in Residential Facilities of Social Services
MATLASOVÁ, Kateřina
Today´s society starts to be intensively interested in life in general and old people´s quality of life, therefore I decided to focus this diploma thesis on seniors´ quality of life in social service facilities. The given goals were to map and assess the quality of seniors? life in chosen social service facilities in a former Jindřichův Hradec district, to compare the quality of seniors´ life in three facilities of the same social services in the frame of the former Jindřichův Hradec district and also to compare the seniors´ quality of life in different social service facilities. The research of this work was made due to a qualitative form. A semi-standardized (semi-controlled) interview survey method and a qualitative observation method, which was used to get the assumed goals of the whole work, were used. To get the aims I used an interview method which was done in Senior Apartment in Jindřichův Hradec, České Velenice and Budíškovice as well as in an apartment with day care in Jindřichův Hradec. It was also used a scale questionnaire called WHO DAS II as a complementary technique in all the social service facilities. The following hypotheses were made out in keeping with the given goals of this diploma thesis. The first hypothesis ? the more social active actions were offered in the frame of provided social services, the higher is the users´ quality of life of this service. The second hypothesis ? users of this service have higher quality of life in the district town than users of this service provided in a municipality with inhabitants less than 10 000. The third hypothesis ? users? quality of life in day care facilities is higher than users´ quality of life of a senior apartment. The given hypotheses were confirmed. We can say old people´s quality of life has been improved in the comparison with last years. I would recommend solving this problem with good communication and feedback which is necessary to find out how seniors are satisfied with services in facilities. It is also very important to keep each client´s private which leads to the feeling of certainty and safeness.
Communication as a tool for helping seniors in senior care providing institutions to an active life
KLIMKOVÁ, Lenka
Communication is one of the most important needs of man whatever his age. A good communication is the basis of relation between the nursing staff and clients. By means of good communication an agreeable atmosphere is created in senior homes, mental state of clients can be influenced and physical hardships are relieved. My paper aimed at describing principles of successful communication with seniors applied within complex care for clients in social and health institutions, including free time activities in the Senior Home Máj in České Budějovice and in Senior Home of Mistr Křišťan in Prachatice. The first one presuming that one of the general principles of effective communication with seniors applied in complex care for clients of health and social institutions is an interested, personal relation of the staff towards the client, accepting his/her personality, empathy and listening to him/her, creation of a friendly and optimistic atmosphere, recognition and encouragement of the client's efforts. This hypothesis was confirmed. The second hypothesis presumed an important role in encouragement of activity in seniors, the clients of health and social institutions, is played by communication methods applied in free time activities, entertaining therapeutical techniques and animation games aiming at encouragement, motivation and empowerment of clients. This hypothesis was confirmed as both institutions offer many free time activities encouraging communication, encouragement and motivation of clients. The staff realize that existing communication is not ideal as it is nurses who communicate most often with seniors when providing nursing and medical care and they would need more time. However, seniors realize and understand the lack of time on the part of the staff. Information for the practical part of the bachelor thesis was collected by means of a quantitative research in the form of a group structured interview, applying mainly the method of a directed interview and analysis of documents.
