The preparedness of medical operation center of medical rescue service as a subject of critical infrastructure for electric power failure in selected regions

This thesis deals and discusses about the preparedness of the medical operations centre of the medical rescue service in selected regions, which include the Pilsen Region and the South Bohemia Region, for an electrical outage. The theoretical part describes the medical emergency service, the system of its provision of pre-hospital emergency care and with a focus on the medical operations centre, critical infrastructure, including the critical infrastructure and crisis preparedness entity, and an integral part is the electricity and power outage. The research part is made by a form of controlled interviews with specialist medical emergency services who deal with the issue. Based on the data obtained from the interviews, the SWOT analysis is performed and the results are subsequently interpreted. Research has shown how prepared individual health operations centres are for power outage, and in which areas they have deficiencies. Among the pros are a higher number of self-built diesel aggregates used to deal with power cuts, as well as the delivery of diesel aggregate from the fire brigade of the region. Based on the data identified and the SWOT analysis performed, a procedure is chosen to improve the preparedness for an electrical outage and to eliminate or mitigate weaknesses and, where appropriate, eliminate certain threats. The outcome of the discussion is the presentation of shortcomings and a proposal for possible improvements that would help to improve the readiness of health operations centres. These proposals will be presented to the experts concerned.
Evacuation of objects and equipments Emergency medical services of the capital city of Prague during special floods

The aim of this thesis is to analyse the readiness of the Prague Emergency Medical Services in the event of special flood. The theoretical part of this work provides general information about special floods, facilities evacuation and addresses legislative issues in this area. In the research part, the threats of the special flood in the capital city - Prague were identified. The individual water works were evaluated, and by using flood maps it was determined which properties of Prague Emergency Medical Services are potentially exposed to the special flood. Furthermore, due to the complexity of the crisis preparedness evaluation, a semi-structured interview was conducted with the leaders of the crisis management of the Prague Emergency Medical Services. This semi-structured interview was assessed using a SWOT analysis, which revealed further findings that can be used to improve the crisis preparedness of the Prague Emergency Medical Services in the threat of the special flood. Based on SWOT analysis results, the "Strategy of an alliance" was selected in order to mitigate weaknesses and seizing opportunities. All conclusions are listed in tables and they are used in the research part in terms of new proposals and measures for the evacuation of the objects of the Prague Emergency Medical Services during the special flood. The aim of the thesis - evaluation of the readiness of the Prague Emergency Medical Services was fulfilled. The biggest and main benefit of the thesis are proposals for improving the crisis preparedness of the organization. This work brings new knowledge, procedures, and expertise in this matter. All proposals and their possible implementations in practice with potential inclusion in the crisis preparedness plan will be consulted at the next crisis management meeting.
The Preparedness of Emergency Medical Services in the Pilsen Region and The Upper Palatinate for Emergency Situations

The aim of this diploma thesis is to present and analyze the preparedness of ambulances of the Pilsen region and the Upper Palatinate to solve emergencies with a focus on cross-border cooperation. The theoretical part describes the integrated rescue system of both territorial units with emphasis on the medical rescue service and its system of providing pre-hospital emergency care and the system of dealing with emergencies affecting more people. The important part is also the legislation of possible cooperation and an important project of cross-border cooperation, thanks to which this activity even started. The research part is performed in the form of controlled interviews with managers of ambulances and managers of the cross-border cooperation project. These workers regularly participated in joint exercises and their awareness of this issue is at a high level. Based on the data obtained from the interviews, a SWOT analysis is performed, and the results are interpreted. The research demonstrated problems in the communication of the ambulances of the Pilsen Region and the Bavarian Red Cross, the problem in the language barrier and the diversity of procedures in solving emergencies. The advantages include quality training of employees and benefits from previous exercises in the form of new planning documentation for cross-border cooperation. Based on the data obtained and the SWOT analysis performed, an alliance strategy is chosen to take advantage of opportunities to eliminate or mitigate the weaknesses of cooperation. In the discussion, is the result of presenting shortcomings and a proposal for possible improvements that would help to improve cross-border cooperation in the future. These proposals will be presented to the employees concerned at the next meeting of the cross-border cooperation project.
Analysis of crisis preparedness of selected municipalties with extended powers in South Bohemian Region

The aim of the thesis is to analyze the crisis preparedness of selected municipalities with extended powers in the South Bohemian region in the crisis plans of the municipality with extended powers with the help of a statistical survey with the determination of the necessary parameters. The theoretical part contains basic information about crisis management, crisis plans and crisis events. Part of the theoretical part is also an analysis of threat analysis, where a list of types of hazards with unacceptable risk was determined, from which municipalities with extended powers choose according to their own risk analysis events potential for their administrative district. After explaining the concepts and their connection, the practical part follows. In the practical part there is a statistical survey of crisis preparedness of selected municipalities with extended powers. Specifically, these are Jindřichův Hradec, Dačice and Třeboň. From the crisis plans, 3 types of possible crisis events were selected, which will provide a basis for the result. These are floods, disruptions to large-scale drinking water supplies and disruptions to large-scale electricity supplies. The mentioned events were divided into several parts and subsequently evaluated by means of multi-criteria analysis with determination of the weight of the monitored parameters. The result of the thesis is an analytical evaluation of selected parameters in crisis plans of selected municipalities with the following competencies. As a result of the practical part, it was not possible to confirm the hypothesis of the absence of statistical differences between the crisis preparation of selected municipalities with one competence. The author therefore rejected this hypothesis.
Ways of conducting an evacuation of critical clients from health facilities

The diploma thesis dealt with the issue of evacuation of critical clients from health care facilities. Extraordinary events are events, unpredictable, affecting objects, areas without distinction, including health care facilities. Crisis preparedness of health care facilities focused on evacuation as one of the tasks of civil protection is insufficiently enshrined in legislation and it is up to each health care facility how to approach the topic and issues of evacuation. The thesis focuses on the theoretical approach to this issue through the literature in the field of population protection and crisis preparedness of health care. The main of the thesis is to design a procedure for the evacuation of critical clients who are provided with health care in these facilities, which I deal with in the practical part of the work. This part is focused on creating a specific proposal for a methodical procedure for the evacuation of critical clients from health care facilities. This proposal was created on the basis of information and data that I obtained by analyzing selected specific representatives of health care facilities in the Czech Republic focused on the nature of health care provided in departments, workplaces of health care facilities and the health status of clients. The synthesis of the data was then used to create the individual steps in carrying out the evacuation. Evacuation groups were created according to the health status of clients and subsequently, according to evacuation groups, individual evacuation approaches. Individual evacuation groups had designated specific evacuation centers or target facilities. The proposal is determined to medical staff who are able to classify the client and then determine the evacuation procedure. The diploma thesis can be served as an educational material for students, but mainly as a basis for planning for emergencies of health care facilities with regard to evacuation.
Comparison of traumatological plans of health care facilities in the Czech and Slovak Republic

This diploma thesis analyzes trauma planning in health care facilities in the Czech and Slovak Republics. The aim of this work was to compare and propose measures to optimize trauma plans of selected health care facilities in Czech and Slovak Republics. The thesis is divided into theoretical and practical part. In the theoretical part of the thesis was used a search of available literature and there were characterized health care facilities in the Czech and Slovak Republics, crisis preparedness of health care, related legislation, traumatological planning and other related terms such as triage- prioritizing treatment and mass admission of patients. In the practical part of the diploma thesis were used qualitative methods such as observation and analysis of research objects, therefore trauma plans of selected health care facilities in the Czech and Slovak Republics, which had been selected on the basis estimation of experts, followed by comparison of already used trauma plans and SWOT analyzes had been processed and evaluated. After that overall evaluation of the obtained results and on their basis had been prepared proposals for the optimalization of trauma plans. The results of this diploma thesis pointed out the importance of updating trauma plans and the importance of detailed and clearly processed trauma plans and the necessities of exercising and training employees from health care facilities, which would contribute to be more effective in crisis preparedness of health care facilities.
Preparedness of Medical Facility to Emergency Events of Medical Character

Theoretical part of the diploma thesis is focused on preparedness and crisis management of healthcare services during occurrence of emergency event with mass health disability. The first chapters provides simple and complete overview of various types of catastrophes, the explanation of emergency events, their causes and outcomes. Theoretical part deals with analysis of Integrated rescue system and puts emphasis on Medical rescue service and its role in mass health disability control in pre-hospital phases. The whole chapter is devoted to recommended procedure of resolving emergency event with mass health disability in the medical facility. Other chapters emphasize on medical facility crisis preparedness focused on traumatological plan. The aim of the diploma thesis was to find out the contemporary state of crisis preparedness of Nemocnice Písek, a.s. emphasizing on traumatological plan and to identify its potential deficiencies as well as to find the best possible solutions that would eliminate the problem to minimum or would remove the problem completely. The second mainstay of the diploma thesis was to evaluate knowledge of medical staff in Nemocnice Písek, a.s. concerning the issue of traumatological planning and evaluate their preparedness to admission of patients during mass health disability and providing quality medical and nursery health care. In my diploma thesis I also looked into the case whether the researched medical facility organises practices of crisis preparedness of traumatological plan in collaboration with the Integrated rescue system, which make the major presumption for quality and professional managing of emergency event with mass health disability. SWOT analysis was used to achieve the set goals, a managerial tool which aims at evaluation of internal and external environment of medical facility through strengths and weaknesses, opportunities and threats, was used. Quantitative research was also used in the thesis, particularly the method of questionnaire survey which gathered data about traumatological planning awareness and attitude of researched group of respondents. The results of questionnaire survey were surprising. Medical staffs are theoretically aware enough of procedures necessary for handling with emergency event with mass health disability according to the traumatological plan. On the other hand, the negative finding is the fact that 74% of medical staff do not participate in practice to handle mass admission of injured people, which in my opinion shows underestimation of the preparedness of the medical facility for emergency event or crisis. The traumatological plan of the chosen medical facility follows the regulations given by the valid legislation and is regularly updated but the interview with the head doctor of the surgical department MUDr. Marta Horáková reveals the weaknesses of traumatology planning. The traumatological plan contains practical deficiencies so that it is recommended to make it more specific, detailed and adapted to technical disposal of the hospital. In particular, the system of calling out the staff in case of traumatological plan activation during out of working hours has not been fully resolved; furthermore, direction points for the Integrated rescue system and the public have been marked insufficiently or have not been marked at all. The last major weakness is the absence of contact point for the media or the public. In the final part of my thesis I suggested measures for elimination or at least reduction of the crisis points.
Prague Emergency Medical Services Preparedness for Medical Intervention During Chemical Threat in Metro

The goal of this theses is to analyse the preparedness of Prague Emergency Medical Services (hereinafter Prague EMS) for chemical attack in subway (metro). The theoretical part states general information about chemical terrorism and the Integrated Rescue System's solution. The research part focuses on procedures following from Integrated Rescue System Common Type Activity during joint action - Reaction on chemical attack in subway (hereinafter STČ 13/IZS). This document was subjected to content analysis to meet the goals. The work then focuses on the evaluation of Prague EMS training Metro 2014 whose goal was to practice the procedures and joint deployment of Integrated Rescues System members and other subject according to STČ 13/IZS. For the evaluation complexity, the results were supplemented by SWOT analysis which pointed at another usable knowledge. All conclusions are clearly organized in charts and they are efficiently worked with to set new measures and recommendations which would increase the preparedness of Prague EMS for chemical attack in metro. The goal of this work to assess the level of preparedness and to submit final protocol to organisation's crisis management was fulfilled. The main benefit of this work is a recommendation for the reinforcement of crisis preparedness at Prague EMS. Furthermore the work brings new expert findings to solved problematics.
Proposal of EMS of the Central Bohemian region preparedness for managing multiple victim incidents and disasters

This thesis deals with issue of preparedness of EMS Central Bohemia on multiple victims' emergencies. In addition to preparedness of EMS Central Bohemia the thesis also analyses the system of training and education of employees in the management of mass casualty. The theoretical part discusses the legislation on EMS, files from Catalogue of model activities related to this issue, coordination of parts of Integrated Rescue System (IRS) on site of event and role of EMS on-site emergencies. Further, the specifics of EMS Central Bohemia are described, the development of EMS Central Bohemia since its establishment (inception) to the present, organizational structure, mass casualty's emergencies preparedness, EMS dispatch (medical operations center), system of emergency calls management and finally Traumatology plan of EMS Central Bohemia. Aim of the thesis was based on analysis of the current status to propose optimal system of deployment and activation of forces and resources, and to propose system of employees' education and system of testing exercises. Based on research, SWOT analysis was developed (composed) and the results were compared with pre-established hypotheses. From obtained findings the comprehensive overview of forces and resources of EMS Central Bohemia region for mass casualty management was prepared. In particular, the number of available resources on duty, system of backups, modules and containers for mass casualties and their deployment. When evaluating resources for mass casualty management also an economic aspect was taken into consideration, as well as ambulance crews' knowledge on possible use of these resources. Based on this review, it was found that in terms of securing forces and resources, the readiness of EMS Central Bohemia is appropriate. Hereby the hypothesis nr.1, which presumed that EMS Central Bohemia has sufficient number of forces and resources to deal with emergencies with multiple victims has been confirmed. The hypothesis nr.2, which presumed that forces and resources deployment is not ideal, on the contrary has not been confirmed. In this thesis the current status of employees' preparation for mass casualty was also discussed. It was found that the base of this preparation is Course for leading employee of rescue service, organized by Department of crisis preparedness. At the same time the conception is set that after first four pilot courses this education system will be applied to particular areas, thereby should be secured sufficient education of crews of EMS Central Bohemia region in this issue. Other education options were also defined, which are now already available in the organization. The hypothesis nr.3, which presumed that system of preparation and training focused on mass casualty is not appropriate, have thus managed to disprove. However it was found that although a clear conception and education options in this field exist, it's not well known to ordinary employees. One of the conclusions is therefore also a recommendation to managers of EMS Central Bohemia to more inform their employees about these options. One of the goals of this thesis was to suggest a system of learning and testing exercises. This proposal is described in the conclusion the proposed number of testing exercises is at least 4 times a year. The total number of employees of the organization and the number of those who may have the capacity and operational reasons to participate on similar exercises has been taken into consideration here. The education system was found appropriate, only with above mentioned objection regarding to awareness of ordinary employees about it.
Emergency readiness of the South-Bohemian Region inpatient healthcare facilities and their medical staff

In the theoretical section of my master thesis I focused especially on the role of the health services within the crisis management. In particular chapters I tried to write down a simple and comprehensive summary of the role of the healthcare within the integrated rescue system, I dealt with the function of the Ministry of Health, the hierarchy of the healthcare in solving the emergencies, the valid law and the crisis preparedness in the healthcare. In one part of my master thesis I also mentioned the importance of testing the professional and practical readiness of particular units of the integrated rescue system by emergency exercises and the characterization of their individual types. Of course I could not omit the crisis planning and a brief description of some chosen types of plans that are used during emergencies.The goal of my thesis was to map the preparedness of the South-Bohemian Region inpatient healthcare facilities for possible emergencies in the respective facility or out of it and the medical staff´s awareness of possible risks and of possible solving the emergencies in the respective facility.In the practical section of my master thesis I used the methods of quantitative research. The technique of this research is based on the questionnaire survey with bounded, half-bounded and open questions. The questionnaire designated for medical staff consists of 23 questions. The first three questions are focused on the characterization of the respondents; the remaining twenty questions are to find out the stock of knowledge related to the crisis preparedness. The respondent gets one point for each correct answer to a knowledge question. This part of the questionnaire is composed as a multiple-choice test and one can get 43 points at most. The questions were created using the crisis plans that are valid in the České Budějovice Hospital. After our mutual agreement I took over the questionnaire designated for healthcare facilities from Mgr. Jolana Němečková, who analysed the crisis preparedness of the South-Moravian Region healthcare facilities in her master thesis. I did not change the method of questionnaire analysis either to enable a subsequent benchmarking of the results in the South-Moravian Region and the South-Bohemian Region. The questionnaire consists of 28 questions. One can get one point for a dichotomous question and two points for a closed-ended question. The highest possible score was 22 points. The results of the questionnaire survey for medical staff show that the preparedness of the České Budějovice Hospital´s medical staff is at a very good level. The average score was 37 out of 43 points and that complies with the classification 2. When analysing the relation of the preparedness with the highest achieved education level and the preparedness with the length of the practice, the positive correlation was confirmed. That means that the higher achieved education and the longer practice, the more was the knowledge of the respective area. The results of the questionnaire survey for the inpatient healthcare facilities management are not so positive. The average score was 12.3 and that complies almost with the classification 4, which means a bad preparedness. In comparison with the South-Moravian Region healthcare facilities, the preparedness in the South-Bohemian Region is one level worse. For the purposes of my master thesis I set four hypotheses that were proved or falsified during a follow-up statistical assessment. Hypotheses No. 1 and No. 2 cannot be accepted. Hypotheses No. 3 and No. 4 were accepted.
