Vestibular and postural biomarkers in spinocerebellar ataxias and peripheral vestibular lesions Abstract Vestibular dysfunction can be a part of clinical impairment in spinocerebellar ataxias. Together with cerebellar symptoms, it can lead to imbalance and also to impairment of dynamic visual acuity (DVA) in these patients. The aim of the work was to objectify the involvement of the vestibulo-ocular reflex in patients with SCA using modern diagnostic methods. At first the innovative method of video head impulse test (vHIT) was introduced in patients after vestibular schwannoma surgery, in whom a clearly defined vestibular impairment is present. We investigated DVA in SCA patients, the degree of the DVA impairment correlated proportionally with the impairment of vestibular function measured by vHIT. The best predictive values for DVA impairment were obtained by using a model that combined impairment of vestibulo-ocular reflex measured by vHIT and degree of cerebellar dysfunction according to SARA (scale for assessment and rating of ataxia). Title: Evaluation of Cerebellar Ataxia from the Perspective of an Occupational Therapist Abstract: The bachelor's thesis deals with the issue of occupational therapy in patients with cerebellar ataxia, especially with the possibilities of examination and evaluation of ataxia using assessment tools. The theoretical part of the thesis describes the types of ataxia. Further focuses only on cerebellar ataxia. The paper defines the individual terms characterizing the symptoms of cerebellar lesions. It also describes how these individual symptoms are examined. Further details on the tools found to assess cerebellar ataxia are included. One chapter also focuses on the role of occupational therapy in patients with cerebellar ataxia. It describes the possibilities of occupational therapy intervention in these patients and presents some therapeutic approaches. The aim of the bachelor thesis is to create a translation of the "Scale for the Assessment and Rating of Ataxia (SARA)" tool from English into Czech using the so-called "back translation" method and to evaluate its applicability in practice. The use of biofeedback for stability training in patients with degenerative cerebellar disease

The graduation thesis deals with the issue of balance disorders due to a cerebellar damage. It deals in detail with the treatment of patients suffering from degenerative cerebellar ataxia and with the use of biofeedback technologies as means of postural stability treatment. The aim of practical part of this work was to investigate the effectiveness of therapy using biofeedback for patients suffering from progressive ataxia due to cerebellar neurodegeneration. The research sample consisted of 8 patients. The patients underwent a total of 18 therapy sessions within a twelve-day rehabilitative program. The therapy contained a training of stability in various positions with the elimination of visual control. A device provided an additive information about head sways for the patients via a tongue electrotactile stimulation. The effects were assessed by means of posturography, functional clinical tests (BESTest, Dynamic Gait Index) and questionnaires (Activities-specific Balance Confidence Scale, Dizziness Handicap Inventory). Three assessments were performed: immediately before, immediately after, and 30 days after the rehabilitative program. Electrotactile sensory substitution in patients with degenerative cerebellar ataxia

Introduction: Biofeedback based on sensory substitution is a new method to treat patients with postural disorders. BrainPort provides tongue electrotactile stimulation. This innovative technique was developed for vestibular dysfunction of peripheral origin. Primarily for patients who had no benefits from conventional rehabilitation. Tongue electrotactile stimulation seems to be useful for central vestibula dysfunction as well. We are dealing with effect of BrainPort in therapy of degenerative cerebellar ataxia. We are interested in whether the effects persist for one month after the rehabilitation program. Methods: The methods are applied in 6 patients with degenerative cerebellar ataxia, verified by clinical testing, posturography, and genetic testing or verification of cerebellar lesions by MRI. Patients underwent an 12-day intensive rehabilitation program with BrainPort. Treatment was carried out 2 times a day. Patients were learning appropriate postural corrections to maintain signal in the middle of the tongue. For the therapeutic program 4 patients with cerebellar ataxia at the age of38 - 74 years were chosen. Subjects have partieipated in the therapeutic program with the application of biofeedback, they were training postural stability while visual control was excluded and they were using electrotacti le signal on the tongue for orientation. I used neurological tests BESTest and Dynamic Oait Index, questionnaire The Activities- Specific Balance Confidence (ABC) Scale, Dizziness I Tandicap Inventory (D ll) a Visual Analogue Scale (VAS), and examination by stati posturography for evaluation of postural stability in the patients. Physiotherapeutic approaches in cerebelar syndrome

Cerebellar ataxia syndrome is treated in different ways depending on source of its origin. In the case of an acquired disease the primary treatment is focused on eliminating the basic disease and physiotherapy influences its aftermaths. In the case of genetically determined disease physiotherapy is an essential treatment, although it only treats symptoms, and it can distinctly affect the patient's life quality. The work is designed as a basis for a planned guideline for the rehabilitation of cerebellar disorders in the Hereditary Ataxia Center FHN Motol. It summarizes more than eighty, mostly foreign, studies that deal with the possibility to influence cerebellar symptoms with physiotherapeutic procedures and discusses the approach of 5 specialized worlds' centers for cerebellar diseases.
