v Abstract Title: Case study of physical therapy treatment of a patient after ischemic stroke with right hemiparesis Objective: The aim of my bachelor thesis is to provide a comprehensive theoretical overview of ischemic stroke and to present a case study of physiotherapeutic care for a specific patient with this diagnosis in the special section. Method: This thesis comprises a theoretical part and a special part. The theoretical section focuses on compiling knowledge about ischemic stroke using scholarly literature and current resources. The special part addresses a case study of physiotherapeutic care for a specific patient who has suffered an ischemic stroke. It includes anamnesis, initial kinesiological analysis, short term and long term physiotherapeutic plans, daily records of therapies, and final kinesiological analysis, as well as an assessment of the therapeutic outcomes. Results: Based on theoretical foundations, physiotherapeutic care was applied, which led to an improvement in self-sufficiency, increased muscle strength, enhanced stability, greater range of motion, and reduced pain in the shoulder joint. Methods: It will be a pilot qualitative research, where it will be applied empirical method using Rize Joint-structured interviews with subjects after stroke. Influencing the gait pattern in patients after stroke employing Vojta reflex locomotion

The title of the thesis: Influencing the gait pattern in patients after stroke employing Vojta reflex locomotion Abstract: This bachelor thesis provides insight into Vojta reflex locomotion method and it's influence on gait quality amongst patients after stroke. Vojta reflex locomotion method is used quite rarely amongst such patients, yet it's field of application is expanding continuously. Use of this method makes it possible to affect complex and almost inaccessible muscular interactivities amongst patients suffering from central paresis, so it can positively affect gait or other parameters necessary for patient's locomotion. In this thesis two patients were studied. Both of them had completed eight forty-minute sessions of therapy during one month. Therapy was performed in two coordination complexes - reflex creeping and reflex rolling. The results were evaluated using certain tests, such as Timed Up and Go test, Rivermead visual gait assessment and Footscan gait analysis. Stabilometric platform Nintendo Wii and program HomeBalance were used during the therapy and enable the visual biofeedback to the pacient. Another specification of this therapy is that the whole process was situated to home environment. The therapy lasted one month and every patient should exercise every day. Time of one exercise depended on the patient's skills. The patient's task was to change position of the center of pressure to the specific place. Every day the patient exercised identical therapeutic scene consisting of a balanced position sequence in the space, that the patient should have achieved. The success of the therapy was evaluated objectively by intrumentation (FootScan) and objective tests, but also by patients' subjective feeling. Cooperation nurses and families of patients after stroke

Acute cerebrovascular accident (CVA) is the third most frequent cause of death in developed countries and its occurrence is increasing even in the productive age. A cerebrovascular accident involves a rapid loss of brain function. It occurs in two variants: ischemic or haemorrhagic. Ischemic CVA is caused by blockage of blood flow into the whole brain or its part. Clinical image of a patient with ischemic CVA is variable, there are mild, serious or fatal conditions differing in terms of the extent, seriousness and time interval of brain tissue ischemia duration. Haemorrhagic CVAs are caused by bleeding from damaged wall of some brain artery. Intracranial haemorrhage may be split or bordered and depends on the artery wall rupture extent, blood pressure and resistance of the adjacent brain tissue. Split haemorrhage occurs as a consequence of chronic arterial hypertension that causes vessel rupture. Haemorrhagic CVA is a subsequence of intracerebral haemorrhage or bleeding into subarachnoidal space. Cerebrovascular accident affects not only the patient, but it also influences further functioning of the family from the sensual, physical and financial points of view. A nurse becomes an irreplaceable assistant and educator for the family in the care of the patient suffering from this disease. Nursing care is impossible without contact with the patient and quality nursing care requires mutual confidence, sympathy, and cooperation within interaction between the nurse, the family and the patient. Nurse?s role in everyday contact also involves so called productive behaviour, which is a part of quality care. The nurse explains all unknown things to the patient. She shows her interest, affection and sympathy. She eliminates anxiety, fears and creates optimistic atmosphere in the patient. She always patiently listens to the patient appreciates his/her efforts, supports and helps him/her. It is important that the nurse creates conditions for the closest contact between the patient and his family, who affect positively his/her condition. All restraints like that care should solely be provided by professionals have to be eliminated and the family should not be perceived as an unimportant factor. The aim of the thesis was to map the level of mutual cooperation between nurses and families and to find what the approach of families to cooperation with nurses of the neurology department of the Písek Hospital was. The research for this bachelor thesis was elaborated in the form of qualitative research. Selection of respondents was completely individual, the sample was divided into two groups (nurses x families). After clarification of all the reasons for the required interview and assurance of maximum anonymousness the groups of respondents ? nurses and relatives were arranged. The dialogues were based on questions prepared in advance. Written records of the interviews were analyzed and the results then categorized into categories and subcategories. The research brought facts about the level of cooperation between the staff and relatives at present. The results have shown that cooperation between the nurses andfamily members is at a very good level at the neurology department. The replies of the respondents were mostly definite, that the cooperation between the family and the nurse is positive and beneficial for both the parties. The research into the approach of a family to cooperation with nurses at the neurology ward has brought positive conclusions. The respondents showed strong interest in the patient. They assumed realistic attitude to the situation occurred, when someone close to them had been hospitalized. There was no rejection of offered cooperation, however uncertainty and fear of the unknown was obvious in all the family respondents. This is why a simple manual based on the research conclusion has been elaborated.
Communication problems of nurses in pacients after stroke

The bachelor thesis "Problems in nurses´ communication with patients after cerebral vascular accident? deals with the knowledge of nurses in communication with patients with aphasia after an ictus. Right communication belongs to key points of the subsequent treatment of this disease. A cerebral vascular accident ? a stroke - can negatively affect the life of a person affected by the disease. Other negatives symptoms are provoked by acquired disorders of cognitive function. Phatic failure is of a concern in the nurse-patient communication. Patients are not usually able to express themselves properly or understand others. During their hospitalization the role of a nurse is therefore very important. Nurses should be sufficiently educated in communication and to know the specifics of this disease. Compensation aids to facilitate and improve communication with patients after a stroke should also be available and used by nurses. The objective of this thesis was to determine whether nurses follow the correct procedures in communication with patients after a stroke, and to map the most frequent communication problems between nurses and patients after a stroke. The empirical part of this study consisted of quantitative research, using an anonymous questionnaire that was designed for nurses in neurological and internal wards of the hospitals in Příbram, Písek and České Budějovice. The questionnaire contained a total of 24 questions. 107 questionnaires were distributed and the research was processed with 90 questionnaires, i.e. 84% of the original number. Three hypotheses were stated. The first hypothesis assumed that nurses know principles of good communication with patients after a stroke. This hypothesis has been confirmed. The second hypothesis assumed that nurses use aids to improve communication with patients after a stroke. This hypothesis has been confirmed. The third hypothesis assumed that nurses are interested in further upgrading skills in communicating with patients after a stroke. This hypothesis has also been confirmed. The objectives have been met on the basis of confirmed hypotheses. This bachelor thesis can serve as educational material for nurses working with patients with aphasia after a stroke.
Social services used by the elderly after having a cerebral hemorragie

My bachelor thesis focuses on senior citizens after a stroke, their family members and their opportunities to use social services. The thesis examined also the awareness of such services and their availability in the Příbram district. The topic is very pressing as the occurrence of strokes worldwide is fairly high and in developed country it is causing a serious medical, social and economic problem. Senior citizens and their family members should know about the possibilities to utilize social services, so that they can achieve the highest possible level of independence and self {--} sufficiency in the home environment or so that their carers are provided with professional assistance. The theoretical part covers the issues of senior citizens and old age, stroke related issues, conditions, provision, funding and remuneration of social services, methods of care provided by the family and an overview of providers of social services for senior citizens in the district of Příbram. The research part of the bachelor thesis used a qualitative research, method of interviewing by means of questionnaires and standardized interviews. The research sample consisted of senior citizens or their family members who were approached by means of questionnaires. 120 questionnaires were distributed and 102 of them were returned, i.e. 18 questionnaires were not filled in. The return rate was 85 %. Interviews were conducted with 15 senior citizens at my workplace in the centre of care for long {--} term bedbound patients. The purpose of my work was to map the availability of the social services in the district of Příbram and the level of awareness of such services and their utilization among senior citizens. Hypothesis H: Senior citizens after a stroke are informed about the social services they can utilize. H1: Senior citizens after a stroke with the level of dependence 3 and higher prefer social services provided in residential facilities. It was found out that certain social services are missing in Příbram and some of the existing ones do not have sufficient capacities. The availability of social services is sufficient. The level of awareness among senior citizens and their family members is also at a good level. The evaluated questionnaires and interviews imply that senior citizens with the level of dependence 3 and higher mostly use the services provided in residential facilities. Based on the results the postulated hypotheses were positively confirmed and thus also the objective of this thesis.
