Bakalářská práce se zaměřuje na návrh a rozvoj obchodního modelu. V teoretické části jsou vymezeny postupy a nástroje pro zhodnocení podniku. V analytické části jsou poznatky o současném stavu podniku a vnějšího okolí. Tyto poznatky jdou poté vyhodnoceny podle důležitosti a jsou podle nich formulovány návrhy na rozvoj podniku.
Types of emergencies and the specific role of the medical rescue service during emergencies with mass disabilities

The aim of the present diploma thesis was forming a general list about emergencies focused on specifics of emergency medical service with mass disability. The theoretical part includes basic terms and legistative. The individual chapters include summary information about the integrated rescue system and the position of the emergency medical service in dealing with emergencies with an emphasis on an emergency with a mass disability. The work deals with the acceptance of the call at the medical operating center, the reporting of departure crews from the place of occurrence and the emergency management structure. The research was done using a qualitative method using a semi-structured interview focusing on the issue of emergencies with mass disabilities, patient classification and personal experience of the staff of the Medical Rescue Service of the South Bohemia with emergencies. After a detailed analysis of the interviews, we realized that the majority of informants have a sufficient to above-average amount of theoretical knowledge in this area. The knowledge of paramedics regarding the general overview and sorting methods (START, Identification and sorting card) was at a very good level. On the contrary, lower knowledge was reflected in the issue of trauma planning. Most of the interviewed paramedics pointed to good theoretical teaching, in preparation for dealing with an emergency, but the respondents emphasized the need to increase the frequency of practical training. Adapting training and practical exercises for the current needs of paramedics would certainly help.
Problems of triage during an emergency involving disabled people

Mass casualty incidents and their occurrence are a real threat not only to the society as a whole but also to emergency medical service providers, who, at the moment of the occurrence of a mass casualty incident, must completely change their approach to providing care and switch into the emergency regime. At that moment, high demands are placed on the entire integrated rescue system, both with regard to ensuring safety, preventive measures and organizing activities at the site of the mass casualty incident, andto providing quick and expert assistance. This bachelor thesis concerns mainly providers of emergency medical services during mass casualty incidents and problems of sorting of casualties. In case of a mass casualty incident, the situation requires a completely different approach to the treatment of the casualties than everyday work. In this situation, the individual medicine model when the emergency medical crew provides care to one patient only, from the initial contact to the transfer of the patient to a medical facility, does not apply. In a regime of mass casualty incident, it is necessary to set priorities for treatment, to carry out such treatment and transport the patient to a medical facility. The priority in such a situation is not to save everyone's life, but to eliminate the extent of injuries of the people to the minimum. The treatment of casualties at the site of a mass casualty incident is complicated by many factors that negatively affect the situation. In particular, there is a lack of time for decision-making and treatment, chaos, stress, panic, and in the initial phase also a lack of resources and forces, concerning not only the providers of emergency medical services, but the entire integrated rescue system. Since the on-site situation is optimized within thirty and sixty minutes of the start of operations, it is crucial in the initial phase to focus on the organization and management of the integrated rescue system units. The bachelor thesis is devoted to the issue of classification of the casualties on the site of amass casualty incident. The theoretical part deals with the topics of disaster medicine, the integrated rescue system and in the last part it is devoted to the classification itself. The theoretical part is based on expert literature and current legislation. In the part focused on the classification of the casualties, the individual methods for classification of casualties and scoring systems are analysed in detail, especially the START method and classification identification card, which dominate in our country. The practical part of the thesis is done in the form of qualitative research and compares the answers obtained by a semi-structured interview with two groups of respondents. Respondents were non-doctor medical personnel of the emergency medical service, one group from the Vysočina Region and the other from the South Bohemian Region. The content of the interview was divided into four categories, each focusing on a particular area of casualty incidents. The individual categories dealt with general and specific procedures at the site of a mass casualty incident, cooperation of the integrated rescue system and the issue of classification of patients. The aim of this thesis was to find out how the rescue workers are prepared for a mass casualty incident both theoretically and practically, how they evaluate the cooperation within the whole integrated rescue system. The aim of the thesis was also to verify the theoretical knowledge of the respondents concerning this area of interest and the procedures. The results obtained by the interview and their analysis are included in the chapter Vyhodnocení rozhovorů. The research questions and their results are discussed in the chapter Diskuze. The overall conclusion is included in the chapter Závěr.
The role of emergency medical services in the incident with mass health disabilities

Medical rescue workers are routinely employed in emergency medicine. In emergencies where a large number of people are affected, knowledge of medical disaster practices is necessary. Within a second, the intervening crews should switch to another mode with a different approach to the patients than they use daily. The diploma thesis has set the following goals: to map the level of knowledge of medical rescuers in the field of emergency response with mass health impairment, their personal experience and, last but not least, education and training. Three research questions were asked to reach these objectives. What are the skills of medical rescuers in dealing with emergency health issues with mass health impairment? How do medical rescuers assess incidents with mass health impairment at which they were present and what is their attitude towards training and education in the field of dealing with emergencies with mass health impairment? The research part consists of interviews with two groups of participants: eight medical rescuers of the Medical Rescue Service of the Pardubice Region and the same number of medical rescuers of the Medical Rescue Service of the South Bohemian Region. These interviews supplemented the information provided by the crisis preparedness managers of both medical rescue services. The survey used a qualitative research strategy, conversations were recorded on a dictaphone, then processed into categories, subcategories, tables and evaluated. From the results of the work, a great difference in the knowledge of health rescuers in the field of emergency health problems can be seen. Participants of the South Bohemian Medical Emergency Rescue Service showed shortcomings in both the basic terminology and the solving of extraordinary events in particular, from reporting the situation with a report form to the classification of the disabled. On the contrary, the majority of participants in the Paramedical Rescue Service of the Pardubice Region would be able to file an initial emergency report and categorize and transport patients correctly. On the basis of the evaluated results, this difference in knowledge can be attributed mainly to the frequency and content of the training provided. In the case of South Bohemian participants, we can find the cause in the absence of practical training. The alarming results were noted in the participants' ability to use the START sorting method, from all of the sixteen respondents only one was able to answer there. The elaborated case report of an extraordinary event points to the fact that theoretical shortcomings can subsequently be transferred into practice.
Education system of paramedics Czech Republic and Slovak Republic to deal with emergencies caused by accidents with mass disability of people

Accidents with mass disability of people occur more and more. No matter if they are of a natural or human origin they are still a burden for both the rescuers and the civilian inhabitants. They have a huge impact on the social and economic aspects of the society. It si inevitable to be ready for such incidents in a sufficient way and thus keep the impact on the human health, lives and property as small as posssible. The purpose of this thesis is to analyse the education of paramedics in the Slovak Republic and the Czech Republic in three dimensions. First of all it is a complementary overview of the school education within the bachelor college study. The second level is addressing the providers of paramedical care and services and compiling an overview of the education at both the teoretical and practical level carried out for their employees. The third part consists of a summary of the current possibilities of individual education provided by conferences, competitions and trainings within associations or organizations. The thesis consists of 4 theoretical chapters and 3 chapters in the empirical part. In the theoretical part we define the medicine of disasters, massive accidents, incidents with a massive inflictions of persons, the sorting system START and others. We talk about the history and current practise of interventions within massive incidents. We also describe the occupation of a paramedic from the perspective of his/her competence, operation of a rescue service and methods of education. We focused mainly on trainings which are the most frequent form along with lectures. In the practical part we analyse the education of paramedics within schools by the means of the content and time. Based on the received questionaires we compare activities which are provided by paramedical care providers for the purpose of education, training and readiness of health care workers for the interventions with a large number of victims. The practical usage we illustrate in casuistics. We summarise a database of congresses, competitions and trainings carried out in given areas in the CR and SR. This thesis can be used as a base in the area of improving the education of paramedics in the area of emergency incidents for schools and paramedical services providers or the needs of the Ministry of Health, Ministry of Education. The differences found between the subjects or republics can be motivating for further progress of education. There is also space for removing all the found imperfections or development of new methods of educating.
Knowledge of members of the emergency services of South Bohemia about issue of Disaster Medicine and mass disasters.

Mass accidents and catastrophes do not occur on a daily scale, they occur irregularly and unpredictably and always have devastating consequences that negatively affect the society or nature. They are usually accompanied by a large number of casualties or damage whose removal cannot be managed using usual local means. To cope with such events, it is necessary to have a professionally trained staff in the Integrated Rescue System and an appropriate coordination of rescue and liquidation. In recent years, there has been an increase of such incidents which have resulted in a mass disability of people. The events of recent days have brought a series of terrorist acts, for example in Paris or Brusselles, of suicide bombings that claimed 130 dead and numerous wounded. These acts, which are associated with the current migration crisis on the European continent, cause considerable tension in the society and evoke questions like whether the readiness of IRS to eliminate the consequences of such events is sufficient. Due to these facts, the thesis tries to find out what knowledge the members of the emergency medical service in South Bohemia have on the issue of disaster medicine and mass accidents. The theoretical part of the thesis titled Knowledge of the Members of the Emergency Medical Service in SB on Disaster Medicine and Mass Accidents defines disaster medicine and all tasks and subjects it deals with. It also mentions the Integrated Rescue System including its basic and other components, its powers and organizational structure resulting from the legislation. Furthermore, it describes the security system and planning, specifically crisis and emergency planning, up to the individual partial plans related to this issue. Finally, the thesis deals with the organization of emergencies with a mass disability of people and a detailed procedure of medical emergency regarding the sorting and evacuation of the wounded. The aim of this thesis is to map the knowledge of the members of the emergency medical service in SB, which concern the issue of disaster medicine and mass accidents, where the level of knowledge is later analyzed to confirm or disprove the hypothesis that the knowledge of the SB IRS members corresponds with a normal distribution frequency. The practical part uses a quantitative method based on questionnaires completed anonymously, where 50 questionnaires were selected randomly as a statistical set. The survey focuses on a group of emergency workers in South Bohemia. The meetings with them were organized during previous practical studies. The results of the survey were analyzed using statistical investigation. The statistical investigation used the methods of scaling, measurement of the absolute, relative and cumulative frequency. Later on, the empirical parametres used for non-parametric testing were counted. The results of this pilot study indicate the current state of the knowledge of the SB IRS members on disaster medicine and mass accidents. The thesis will be a beneficial study material for current and future paramedics and students of the field of population protection.
Readiness of the urgent reception to mass disasters

Readiness of the urgent reception to mass disasters This thesis is dedicated to the readiness of the urgent reception to mass disasters. It deals with the history, evolution and the current state of the urgent reception, both in the Czech Republic in the first place and in the world. Within the scope of this thesis I focus mainly on the work scope of the urgent receptions in dependence to their structure, equipment and their actual preparedness to deal with mass accidents. I focus besides other things on technical equipment of the urgent receptions, which is in frames of the Czech Republic very heterogeneous, but totally crucial to handle possible mass accident. A very import aspect is the matter of education and further training of the medical employees on all levels. It concerns not only medical positions, but also nurses and auxiliary staff. The question of the conditions severity of the admitted patients, their classification according to these conditions and transparent processing of the information about them cannot be passed over. Furthermore the thesis discusses emergency medicine as a discipline and the direction it should, in my opinion, further be heading in order to improve the functioning of the present system of pre-hospital and hospital emergency care. Urgent receptions in the Czech Republic represent a relatively new thing, which results into certain ambivalence of their concept and structure. Each urgent reception is somehow original and adapted to the requirements of the founder and the local situation. There is one thing the urgent receptions have in common, overload with patients who fall into the competence of practitioners. This is associated with the financial loss-making of these departments within the medical facilities. This, especially in the case of the private hospitals, leads to a reluctance of the founder to invest to the urgent reception. In this thesis I mention legal framework, contingency planning in the Czech Republic, contingency plans, their content purpose and type plan with its importance for solving crisis events. Another plan that is resolved in my thesis and that has crucial importance especially, for urgent receptions is the trauma plan of medical facility whose application into practice in the Czech Republic in many health care facilities is very problematic. And the practice of the trauma plan activation itself is, according to my research, the prerogative of only a very small amount of medical facilities.
Emergency preparedness of the Integrated Rescue System and its bodies for sudden accidents associated with a large number of injuries and victims in Slovakia

The main topic of the thesis is the readiness for emergencies with a large number of injured and victims of the bodies of IRS in the SR. The work is divided into two main parts, theoretical and empirical. In the theoretical part of the work several bodies of the IRS in the SR, which participate during rescue and clean-up operations, are introduced. The basic bodies of the rescue system are described in detail, other bodies and bodies of the Police Force are described in brief. The work also includes categories of emergencies and brief information of disaster medicine as well. At the end, the theoretical part addresses the problem of sorting the injured by history through the categories and types of selection, sorting methods "START" and JumpSTART, a sorting card, and organization of activities in the place of an event associated with the "triage" procedures. Empirical part consists of research to determine theoretical knowledge of selected bodies of the IRS on the issue of triage. The main aim of this work was to determine the theoretical readiness of selected bodies of the IRS Fire and Rescue Service and Emergency Medical Service in the SR, confirm the need for continuous training and the organization of tactical exercises focused on mass disaster and work out and propose the same procedure of sorting the injured in incidents with a large number of injured and victims. Two hypotheses related to the theoretical readiness of selected bodies associated with sorting the injured in an emergency were established. The first assumption was that paramedics have better scope of theoretical knowledge about handling with emergencies, particularly sorting the injured than members of the Fire and Rescue Service. The second assumption was that the bodies of IRS have better knowledge about the system of sorting the injured "START" than the system "JumpSTART". Basic statistic method that allowed confirmation or refutation of hypotheses two-stage test is described in the chapter too. The methodology describes a method of data collection using a questionnaire, describes the statistical methods in detail and enriches it with the determination of basic empirical parameters and their comparison. The found discrepancies were expressed as a percentage. Data to confirm hypotheses were collected from paramedics and members of the Fire and Rescue Service. Only one hypothesis was confirmed. Empirical investigation showed that theoretical knowledge about the issue "triage" are at a much higher level in paramedics than members of the Fire and Rescue Service. Another observation was that the selected bodies do not show a difference in knowledge about the classification of adults and children, when considered together. There would be a big difference if they were compared individually. The results identified weaknesses in knowledge particularly among members of the Fire and Rescue Service and, therefore, detailed procedure for "triage" in emergencies was proposed. It will serve as a tool primarily for members of the Fire and Rescue Service as well as a study material for university graduates in the field of emergency health care. The need for emergency medical service was designed rescue bag with aids for primary triage. The need for continuous training was observed, particularly among members of the Fire and Rescue Service, who lack the information and skills. Organizing tactical exercises especially for paramedics with the possibility of cooperation with members of the Fire and Rescue Service was proposed as well. In the light of these findings, we recommend making effort to address the deficiencies in the theoretical preparedness through the organizing of continuing education, prepare similar questionnaires for verifying knowledge after the seminars, organize training of practical skills and verify knowledge through participation in exercises on dealing with catastrophe, together with the other emergency bodies.
