Hippotherapy, a part of the medical treatment of children with cerebral palsy

My bachelor's thesis called Hippotherapy, a Part of the Medical Treatment of Children with Cerebral Palsy focuses on the influence and effects of hippotherapy on the locomotive apparatus and emotional feelings of three children's patients. The research took place from January to March 2011 on the premises of the Sociotherapeutic Farm of the Mental Hospital Bohnice in Prague. Nowadays the awareness of hippotherapy is growing both among specialists and laymen. However, it is important to realize that hippotherapy does not only mean horse riding which make people feel emotionally good, but it is also a purposeful and efficient therapeutic method. In my bachelor's thesis I emphasize the positive impact of this therapy not only on the emotional state of mind, but also on the locomotive apparatus of clients and the muscle tone. My bachelor's thesis is divided in two parts ? theoretical and practical. The theoretical part is an overview of my knowledge of hippo-rehabilitation with focus on hippotherapy from professional books and other available sources. I also present my knowledge of children's cerebral palsy (DMO), possibilities of physiotherapy of cerebral palsy and types of muscle tone. The practical part focuses on the research regarding effects of hippotherapy on patients suffering from DMO. By using the hippotherapy evaluation test (Hollý, Hornáček, 2005) I evaluated the influence of hippotherapy on the locomotive apparatus of patients sitting on a horse, by measuring the distance of epicondylus lateralis femur of the right and left lower limbs in the maximum abduction (Drhovský, 2006) I evaluated the influence on spasticity of patient's lower limbs, by using a verbal scale of feelings of happiness (Hollý, Hornáček, 2005) I tried to find out whether horse riding makes children with DMO feel happy and based on the secondary analysis of data I tried to outline the effects of hippotherapy in the medical treatment of children with DMO. The objective of my bachelor's thesis is to provide new information on hippotherapy both to professionals and public and to emphasize its positive effects on humans.
Possibilities to use the canistherapy for children with special educational needs in the rehabilitation centre for people with disabilities of ARPIDA, o.s.

This bachelor thesis is focused on possibilities to use the canistherapy in the centre of ARPIDA, o.s. An aim of the research was to find out an effectiveness of the canistherapy for children with special educational needs in the context of a comprehensive rehabilitation system. To collect data, I chose following methods: a direct participant observation, an uncontrolled interview and a content analysis of the data collection. I led interviews with physiotherapists, a psychologist, a head of the day care centre and with parents of the children from the research sample. The observed sample consisted of four children with special educational needs aged 11, 14, 15 a 21 years. All the children attended the day care centre of ARPIDA, o.s. Children´s case studies, an actual course of the canistherapy including a picture file, expressions of individual experts including an evaluation of the effectiveness of the canistherapy at each child with the special educational needs, are recorded in the practical part of my bachelor thesis. The most effective method at the children with the cerebral palsy is an AAT method in the form of a positioning. The effectiveness of the canisterapy was recorded here mainly in the motor area. The most significant indicator of the effectiveness of the canistherapy at those children was releasing of their spastic muscles. Results of the observation then confirmed positive emotional moods of children during the canistherapy. At a child with ADHD syndrome the AAA method was used, through which it came, during the canistherapy, to an improvement of the ability to concentrate an attention and to an overall pacification. The effectiveness of the canistherapy was evident even in the area of the fine motor and communication abilities of children. I suppose that findings of the thesis can become a guidance and an inspiration for more frequent visits of future canistherapeutic teams, which will decide to work with children in the centre of ARPIDA.
Supportive therapy with dog´s assistance as a part of the coherent physiotherapy for the physically disabled person

ABSTRACT The use of animals as man{\crq}s helpers or companions is as old as domestication itself. Contact with an animal, if accepted positively by the person, relaxes, slows down heartbeat and lowers blood pressure, releases tension, intermediates communication, compensates for missing abilities of people. Canistherapy as a supporting rehabilitation method contributes to the relaxation of muscle spasms, development of fine and gross motor skills, stimulates verbal and non-verbal communication, orientation in space, enhances motivation to participate in one{\crq}s own healing process, and improves the client{\crq}s interaction with other clients and nurses. The theoretical part of the Bachelor thesis is dedicated to the definition of canistherapy within comprehensive rehabilitation of people with a motor apparatus disability. Particular attention is paid to the issue of individuals affected by cerebral palsy and achondroplasia. The goal of the Bachelor thesis is to determine the influence of a dog-assisted therapy on people with a motor apparatus disability. A partial goal is to define the standing and position of a dog-assisted therapy within integral comprehensive rehabilitation of the client. The research is mainly focused on the use of the Animal Assisted Therapy method. Qualitative techniques of data collection were used within the research part of the Bachelor thesis. The following data collection techniques were used: anamnesis, observation (kinesiology examination), secondary data analysis and photo documentation. Results of the action of AAT method canistherapy prove in different areas a positive influence on both monitored clients permanently living in a diagnostic social care institution. The research was carried out in a client suffering from a quadriplegia form of cerebral palsy and in a client with achondroplasia. Casuistries of both mentioned clients describe the way of dog assisted therapy conduct, progress and results.
Posibilities of Physiotherapeutic interventions in children with cerebral palsy

Physical therapy plays an important part in the process of treatment of this disorder. Which in this case for life. The objective of my thesis was to sum up theoretical knowledge and information on CCP and on physical therapy of children suffering from CCP. The first task was to set up an overview of options and alternatives of physical therapy for children with CCP. The other task was to work out case reports of two children with the CCP diagnosis. Then a therapy plan, based on the knowledge from the theoretical part and on the patient´s examination, was made up. A combination of therapy techniques was proposed (the Reflection Method of Locomotion {--} a method developed by M.D. Vaclav Vojta, Proprioceptive Neuromuscular Facilitation (PNF), Basal programmes and sub programmes) suitable for a client with regard to his current state, his needs and abilities. The results showed that combination of methods in therapy is beneficial for a positive influence of motoric skills of a child, improvement of motion in particular joints and for a positive mental condition of a patient This thesis might be used as a set of helpful information for physiotherapists in clinical practice, who deal with CCP, and also as a guide for parents of children with the diagnosis of children cerebral palsy.
Chances and Difficulties of Family Upbringing of a Child with Polio

The thesis deals with chances and difficulties, which a family bringing up a child with polio has to cope with. In the theoretical part a child with a polio is characterized, the influence of the birth and upbringing of a child with polio on the life of its siblings and parents is described. Next, the cooperation with experts, organizations and institutions is considered as well as help from the society in the form of special advantages and social benefits. In the practical part the experience, opinions and feelings of parents bringing up a child with polio are presented on the base of a performed research. The main objective of the research was to find out how the birth and upbringing of a child with polio influences the family life, which difficulties and chances in various forms the parents met with and what is their opinion of them.
The importance of the comprehensive rehabilitation system of children with a combined handicap

The bachelor work investigates the integral rehabilitation system of care for children with a severe complex handicap. A partial aim of the work was to map out this problem in the ARPIDA Centre, a.c. in České Budějovice which provides services in this field. The research proved hypothesis no. 1 which presumes that when applying an integral rehabilitation system, close cooperation between interested professionals and the parents of a handicapped child is necessary. Hypothesis no. 2 was also proved. It states that the basis for complex care of children with severe complex handicap within the integral rehabilitation system is therapeutic physical education, especially the Vojta{\crq}s reflex method. Therapy is a long term and team effort and, as proven by the research, the parents are an integral part of the team. The results of the research proved the parents´ satisfaction with the activities of ARPIDA centre, including the opportunity to cooperate with professionals working at the centre. In my opinion there is also significant room for appropriate education for parents of children with a severe handicap with regards to the difficult task of caring for their children. This work can be used as study material for those interested in work in helping professions.
Integration children suffering from infant cerebral palsy into schools in Vysočina region

In my bachelor thesis I deal with the possibility of integration of children suffering from infant cerebral palsy into primary schools.My paper aimed at finding out what problems are encountered by parents of the integrated children.I was interested in their opinion, what they think about integration and how their child was integrated or not in the collective of their "healthy" schoolmates. Since I decided to sum up the current situation of the issue in the Vysočina region where I come from, I used the qalitative research in my paper.
Influence of combined disability of children looked after the ARPIDA center on lifestyles of their families.

This diploma thesis deals with the care of a child with combined handicaps in the family. I investigated how the lifestyle of a family caring for a child with combined handicaps is affected. A sub-task was to explore this issue in a family caring for a child with combined handicaps who is a client of a nongovernmental non-profit facility, the centre ARPIDA, o.s., in České Budějovice. I used qualitative research methods in my diploma thesis. The chosen technique was a semi standardized interview with the parents and siblings of children with combined handicaps in care at the centre ARPIDA, o.s. A content analysis of data from individual client documentation from the centre ARPIDA, o.s., was used as a complementary technique. Based on these materials, I created five case studies of clients with combined handicaps. The theoretical part describes the important concepts and situations that are related to this issue. Particular attention then focuses on individual means of a comprehensive rehabilitation system which offer a comprehensive interdisciplinary care to families caring for a child with disabilities. In the practical part, I analyse the interviews carried out with the parents of children with combined handicaps. In particular, I examined their views of the changes that they arose in connection with the birth and care of a child with disabilities. I also had interviews with the healthy siblings of the children with disabilities. Thanks to these interviews, I gained interesting insights on this issue from the perspective of other family members. In conclusion, I state that the lifestyle of the parents and siblings of a child with combined handicaps is significantly affected by the birth of a disabled child. The parents often lose the opportunity to follow their professional careers and there is sometimes a negative impact on their health. They feel insufficiently appreciated by the state for their burdensome care for a child with combined handicaps. Due to this fact, healthy siblings become more mature and more sensitive individuals who are aware of critical life values. Based on the research part of my thesis, I defined three hypotheses in the end. Hypothesis 1: The parents are adversely affected in their health due to long care for a child with combined handicaps. Hypothesis 2: The most important quality in the care of a child with disabilities is patience. Hypothesis 3: Nongovernmental non-profit organizations are an important source of assistance to families with a child with disabilities in its natural environment. The diploma thesis should highlight the importance and unsubstitutability of families caring for a child with combined handicaps. This thesis may serve to professionals and students focusing on this issue, but also the parents of children with combined handicaps as their feedback.
Integration of children with multiple handicap

In my assignment ``Integration of children with multiple handicap{\crqq} I focused on the findings what problems parents of children with combined disability within school integration have to face, especially primary schools in the region Karlovy Vary. I also focused on teachers with these children integrated in their class. Performing the research, the main methods I used were questioning and analyses of documents. As to the questioning, I chose the form {\clqq}interview``. Collecting data from analyses of documents, I used the form {\clqq}secondary data analysis``. The research has revealed many hypotheses. My assignment should serve as a valuable source of information on this issue that has been discussed very often in the last time.

ABSTRACT Model housing like a part of self-contained rehabilitation of children with combinated handicap In my thesis I deal with the following subject: Model housing like a part of self-contained rehabilitation of children with combinated handicap. The model housing is totally new project which Children´s Centre ARPIDA in České Budějovice ( hereinafter {\clqq} DC ARPIDA ``) took up as part of its services.The project has been running for two years and its implementation is realized in the second grade of vocational tranining school of DC ARPIDA in the building of The model protected housing. The aim of the project is to teach children with physical and combined handicap predefined activities.These predefined model activities should enlarge their independence from other persons and increase the extent of their self-sufficiency in common and extended daily activities connected with housing. The model protected housing is designed for children who attend the two-year vocational tranining school in DC ARPIDA where they go through three month´s course of the model protected housing.At the beginning of the course the children go through ergo-therapeutical examination to determine the rate of their independence and self-sufficiency. After finishing the course they are reviewed to find out whether they have improved their results and how they have profited from passing through the course. The main goal of my thesis is to investigate whether The model protected housing contributes to the increasing of the client´s level of seff-sufficiency and why it has a great importance in the system of education and physiotherapy. Another goal which should be followed is to show how the children´s parents see the project as a whole, whether they want their children to take part in this project, if they support the idea of a separate living and how they want their children to live in the future. I used the questionnaire method and the secondary data analysis to verify the given hypotheses. The interviewed group was made up of the parents of children clients of DC ARPIDA. My main aim was to investigate how much they are informed about the possibilities of integration of their children to the society, in what ways the parents are able to support them, their willingness to assist in this proccess and what is their idea of their children´s future life.The proccess of integration is not only the result of school classes or experts´ and school staff´s effort but first of all it is the parents who are supposed to be the most important link in the proccess of integration . Their position in the proccess of successful integration is irreplaceable. The input and output physiotherapy results were used for the secondary analysis. On the basis of data obtained from this analysis it is possible to claim that the model activities which the children performed in the model protected housing help to increase the rate of independance from an other person and of everyday self-sufficiency. With respect to explicit results of the research it is possible to draw the conclusion that in all cases the course of model protected housing increases the function rate of self-sufficiency and independence from the others in the model activities. Thereby the basis hypothesis has been clearly confirmed.
