Case study of physiotherapeutic treatment of a patient after total hip endoprosthesis

Title: Case study of diagnosis of total hip endoprosthesis Author: Maria Stylianidou This Bachelor Thesis includes the treatment of the case of my patient who had an operation of total hip endoprosthesis 3 years ago, caused by osteoarthritis. Focusing in kinesiology and biomechanics of the hip after surgery and discuss its correlation with the low back. My Bachelor Thesis consists of two parts: theoretical and practical parts. The theoretical part includes the anatomical, kinesiological and biomechanical aspects of the hip joint as well its connection with the lumbar after hip operation by possible complications which affect the low back. In practical part I analysed every procedure I have done with my patient, all examinations and their conclusions and day to day therapy procedures and progress. At the end of the therapy sessions, the patient had some important changes concerning her pelvis and spine, fascias, muscles as well as her gait pattern. More details about the progress of the patient included on the final kinesiologic examination in the special part of this Bachelor Thesis. The effects of physiotherapy for female stress incontinence in working age

The bachelor thesis deals with impacts of physiotherapeutic methods on woman stress incontinence in working age. Stress incontinence is an uncontrollable leakage of urine during the increase of intrapelvic pressure in activities such as sneezing, coughing, laughter, lifting of heavy weights etc. Middle-aged women are mainly affected by this symptom.The first part of the thesis refers to the anatomy and physiology of the urinary tract, pelvic floor and core stabilization system, which the pelvic is part of. Subsequently, the incontinence and its distribution is outlined, focusing on stress incontinence and its Etiology, diagnosis and treatment. Since the stress incontinence has an unpleasant experience for most of the women, it is also mentioned its negative impact on the quality of life. The second part of the thesis is practical, using qualitative research in the form of three casuistries. Patients are three women aged from 42 to 57 years old suffering from stress incontinence. The goal of the thesis was to point out the effect of physiotherapy on stress incontinence treatment and describe the physiotherapy impact on the quality of life.The exercise unit with patients consisted of entry check-up and then the individual therapy was created including exercise methods such as Mojžíšova's, Kegel's and development positions according to Mr. Kolář. Patients used for strengthening the pelvic floor the Kegel8? system. Finally, the examination and evaluation was performed. The quality of life was determined through interview and CONTILIFE questionnaire, which was submitted at the beginning and the end of the therapy. Final results showed that the chosen therapy was not as successful as expected. Only one patient felt the improvement of her incontinence. Other patients did not feel any difference. Following to the results, the quality of life remained almost intact. The reason behind the result could be the inactivity of patients and short duration of the therapy. However, all patients experience improvement in their physical condition, including individual problems that seem to be unrelated to stress incontinence.
Influencing of stress urinary incontinence of women by different physioterapy methods

The topic of my bachelor's thesis is affecting the women stress incontinence using different physiotherapeutic methods. The thesis consists of a theoretical part and a practical part. The theoretical part covers the problematic of physiology and anatomy of the urinary system and pelvic floor. It also explains the theory of incontinence, diagnosis and treatment of the stress incontinence. Stress incontinence is a state in which one cannot intentionally regulate the urine drain using increased intrapelvic pressure. It is one of the most common women diseases. It is a multifactorial problem, because it causes not only physical, but also mental, social and even economic problems. Stress incontinence is a symptom which can have several causes. Under normal circumstances, the urethra reacts on increase of the intrapelvic pressure by increasing its own pressure and thus it is closed. Therefore it cannot leak the urine. If, however, the urethra is not in its physiologic place, which can be caused by a pelvic floor muscle malfunction, the urine is being leaked uncontrollably. The pelvic floor muscles are crucial part of the core stabilization system. They are very important stabilizers of the whole torso and together with diaphragm they ensure the proper breathing. The stress incontinence can be treated also using conservative and surgical methods. In most cases, the incontinence is treated using only passive methods, by using medicines and applying incontinence tools. In the worst cases, they are treated by a surgery. Women, however, have a possibility to treat the incontinence by physiotherapy. Using this treatment, the symptoms of incontinence can be eliminated or at least reduced to a manageable level. The incontinence can be prevented by doing particular exercises. The goal of the physiotherapy is to teach the women how the urinal system and mainly the pelvic floor work. To help them with the training of the activation and relaxation of the pelvic floor muscles and how to include them during the breathing together with the diaphragm. It is important to focus on the woman mentality. We are trying to support the proper self confidence of the women. The women have to be treated comprehensively and individually. The practical part of the thesis was done using qualitative research. Two patients suffering from stress incontinence were treated during the research. Based on the entrance kinesiologic analysis, an individual exercise plan was created for each of the patients. In the end, the output kinesiologic analysis was done. Mainly, the postural therapy, the flat foot therapy, techniques by Capova, therapy by Ludmila Mojzisova, etc. were used. It is clear from the results that the used therapy was successful and it was rated very well by the patients. According to the patients, the exercises had influence not only on their incontinence problems, but it also helped to improve their overall condition, both physical and mental.
