Clinical-pathological correlation of chronic oral lesions Summary Background: The dissertation deals with local and systemic factors of chronic oral epithelial lesions. It is focused on chronic hyperplastic candidosis. Describes features and behaviour of this disease, its complex diagnostics and therapy. Shows the options of correlation between clinical attributes of lesions, histologic image, laboratory results or microbiological cultivations during this disease. Possible stable correction measures for stabilization of patients are shown. For successful therapy is a cooperation of dentist, practitioners and immunologists mandatory. Methods: The study has 128 patients with histologically verified chronic hyperplastic candidosis. Anamnestic and clinical risk factors for lesion manifestation have been determined. Examination for subjective complaints was performed at 117 patients. Immunological status was evaluated in 61 patients for both humoral and cellular immunity. Through the dispensary cultivation checks and correlation of clinical status and subjective problems have been performed. Results: Correlations of risk factors for the course of disesase and its recurrence were evaluated. Multiple myeloma- diagnosis and treatment

Multiple myeloma is an agressive hemato-oncological disease the diagnosis and treatment possibilities of which have been developing for the last 15 years. The diagnostic methods include flow cytometry which uses antigen detection to distinguish between pathological and physiological plasmocytes. One of the monitored markers is marker CD45 which could be, according to our monitoring, of a negative prognostic value. 71 patients in our group were divided according to the intensity of CD45 antigen expression and the group with a lower expression showed a statistically higher risk of relapse within 12 months 62% vs. 25 % (p=0,0011). We did not find any connection with the influences of induction therapy or cytogenetics which are otherwise considered the most important prognostic markers. Multiple myeloma treatment involves a lot of combined protocols; and the repeated autologous transplantation is still considered to be the most efficient. We observed more positive results in planning the second autologous transplant early in patients who reached just partial remission after the 1st autologous transplant - significantly better TFS (treatment free survival) and overall survival (OS) were in the group of patients with early tandem transplantation: 18 months vs. 10 months (p=0.04) and the OS median was not reached...

Summary: In this thesis section "Oral Mucositis (OM) and oral Graft versus Host Disease (GVHD) in patients after allogeneic stem cells transplantation and with focus to FLU/MEL conditioning regimen" the author deals with following issues: 1. Literature overview: The overall negative impact of OM is mentioned and the complex pathogenesis, clinical characteristics, incidence and factors influencing both OM and GVHD are described. The histological features of GVHD and classification of it are mentioned, too. Conventional and reducedintensity conditioning transplant regimens, including FLU/MEL are presented. 2. Methodological section: Prospective observational study of 117 patients after allogeneic stem cells transplantation with BU/CY2/±ATG (26%), FLU/MEL (60%), FLU/CY (9%), Bu/FLU/ATG (2%) and HD-CY/ATG (2%) conditioning regimens. OM assed according to WHO criteria, GVHD assessed according to NIH criteria accepted by the EBMT. The statistical univariate analysis performed by means of GraphPad In Stat - Statistica Software (Man- Whitney, Fisher, t-test), p<0,05 considered significant.
