Evaluation of pulmonary and postural functions in patients with chest wall deformity (Pectus carinatum)

Objective: The paper deals with the evaluation of the status of pulmonary and postural functions of the patients with pectus carinatum. Further, it deals with the development of pulmonary and postural functions after Welch surgery. Methods: Between the years 2010 and 2011 20 patients (19 men, 1 woman) went through Welch surgery at the Pediatric Clinic of the Faculty Hospital, Prague Motol. The average age of the group was 16,2 ± 1,2 years. Before surgery, pulmonary functions, respiratory muscle strength and posturography were evaluated. The received values were compared with the predicted, i.e. the controlled group, and in order to find mutual dependencies, also among themselves. 7 patients were repeatedly checked for pulmonary functions and posturography in the interval of 8,1 ± 3,0 months after their surgery. Pre and post - surgery values were compared. Results: Dynamic and static hyperinflation and also an increased patency of periphery respiratory tracts was found in 20 pre-surgery patients with PC. Testing of respiratory muscle strength showed significantly lower values than the predicted. Checking for posturography revealed the differences in postural system resulting, especially with complicated postural demands, in medio-lateral instability of the body. Effects of Vojta's reflex locomotion on lung function

Introduction: The aim of this thesis is to answer a question if it is possible to influence ventilation parameters with an application of Vojta's reflex locomotion, which is one of the neurophysiologic techniques. The thesis confronts actual knowledge and relation between respiratory and postural function of "respiratory" muscles and the diaphragm and possibility of its reflex affection. Methods: With a spirometry we assess the impact of short-time activation of Reflex locomotion on the lung functions in a group of 21 healthy subjects in age 18 - 33 years. The study evaluates the effect of activation in two situations. First, it is instantaneous effect of Vojta's method on lung function. Second situation is the assessment of the time duration of evoked effect of activation and make out, if the changes are seen also after 25 minutes over the end of Vojta's stimulation. Results: Based on the interpretation of the both monitored situations, we set up an observation that short time activation of Vojta's reflex therapy has an effect on respiratory mechanics and thus on the spirometric values. At the other side we realize that not all of the changes were significant and some of them were unadvisable. Possibilities of physical therapy in females with muscular dysbalances of the type of the upper crossed syndrome

Upper-crossed syndrome is, according to Janda, described as a muscle imbalance in the upper part of the body, which significantly affects function of the cervical spine, the thoracic spine and the shoulder girdle. The non-physiological function and the position in these parts further influence, above all, respiratory stereotype - upper breathing - and body statics. Therapy is based on influencing the deep stabilizing system, followed by a therapy focused on the place of malfunction and the kinesiology count. Therapy of deep stabilization system complex infuences breath and postural function in their mutual coordination and it is program-controlled by CNS. Importance of close connection of breath and postural functions ensues from the fact, that respiratory muscles participate in internal stabilization with deep spinal muscles. After spinal cord injury occur a lot of complication, with resulting from neurological picture. These can be central, peripheral or mix paresis in some cases even plegie. Often an autonomic nervous system can be demaged, causing complication practice in nearly all body systems. Rehabilitation attend belongs to basic care of paretic patients and is closely connected with basic with basic nursing methods from which the physiotherapy of these patients arise. This survey gives a summary of procedures and methods of physiotherapy , which help the patient to prevent and better respond to the complication treatment e.g. on respiratory and movement apparatus. Complex physiotherapeutic treatment concentrates mainly on prevention of complications. The physiotherapist must anticipate formation of complications and adapt the comlexity of the treatment. The aim of the thesis is to prove that active techniques of respiratory physiotherapy together with aerosol inhalations contribute effectively to treatment of acute and chronic respiratory problems and also to prove that this therapy increases patient's motivation to realize the importance and effectiveness of respiratory exercises . Benefits of above-mentioned therapy influence patient's professional life. Quality of life has also impact on family and social relationships of respective patient. The influence of colostomy on the musculoskeletal system

Introduction. This work is a pilot study which might highlighte pathology of musculoskeletal system at the patients with colostomy. It suppose to evaluate quality of life and the importance of physiotherapy intervention. Methodology. The group includes 13 patients (6 men and 7 women) with left colostomy. Median age was 58 years. The median time from the creation of the stoma at the time of investigation was 33 months. Patients were collected anamnestic data, kinesiological analysis, focused on testing stand, scars, coccyx, muscular tonus of 6 muscle groups, the diagnosis of visceralsomatic relationship and dynamic neuromuscular system. In conclusion, questionnaire methods were used for quality of life and pain. Patients were instructed in two months of therapy, followed by control investigation. Results. The most significant abnormalities were observed in the dynamic neuromuscular system insufficiency, in expression of visceral patterns of colon, but also for the stomach and duodenum. Abdominal wall has hypotonia, the other groups (except for m. piriformis), however, show a left-sided hypertonia. Significant is also the coccyx pain and limited mobility of scars with its palpable sensitivity. Patients have a slightly reduced quality of life and affective component of pain tends to exceed the sensory...
