Alternative methods in managing postoperative pain in living donor renal transplantant recipients
Nová, Michaela ; Hocková, Jana (advisor) ; Dynáková, Šárka (referee)
The main criteria for managing good postoperative care include the effective and appropriate choice of pain therapy. Experiencing pain is a purely individual feeling for each of us, so it is very important to have adequate and sufficient analgesia during this period. Modern management of post-operative pain therapy uses preparations of various pharmacological groups that potentiate and thereby more effectively reduce pain. By this mechanism, we can reduce the total dose of analgesics given, and more particularly, reduce the dose of opioids that have a higher risk of side effects. In this project i want to show how the analgesic catheter could be useful as alternative possibility in algorithm therapy of postoperative pain. Analgesic catheter enable continual local anesthetics administration, exactly 0,5% bupivacaine, which is operating in surgical wound. Main goal of the study is to find out and verify if analgesic catheter is method which provides continual analgesia strong enough to results in lower use of opioids. The research data will be determined by quantitative research using a questionnaire survey. Non standardized self-production questionnaire. will be given in a paper form to non-medical healthcare staff, who perform nursing activities The spectrum of patients is very specific; they are...

Bohuslavová, Monika ; Dynáková, Šárka (advisor) ; Pucholtová, Romana (referee)
Introduction: Burnout syndrome is a topical issue in the world and in the Czech Republic, not only in the healt sector. The diploma thesis is focused on finding the conection between burnout syndrome and sexuality of general nurses in emergency care. I chose the topic for my diploma thesis because it is close to me and I work in emergency care myself. In addition, the topic is not very well explored, and the research data will contribute to an international study examining the imapct od burnout syndrome on human health. This study in the Czech Republic is under the guidance of PhDr. Šárky Dynákové, Ph.D. and doc. PhDr. Jana Kožnara, CSc. Methodology: Data collection for research was conducted using two anonymous questionnaires. The first questionnaire focused on burnout syndrome using the Czech version of the Shirom-Melamed scale (Shirom, Melamed, 2006 In Ptáček, 2013). The second questionnaire examined the sexual function of a woman according to Mellan in Kratochvíl, S. (1999). The research sample consisted of general nurses working in emergency care at a teaching hospital. Results: The impact of burnout syndrome has been demonstrated by research in sexual desire (H1), self-assessment as a sexual partner (H2), and mood before sexual intercourse (H4). The results show that the area that achieved...

(v AJ) Introduction: The literature indicates that progressive aging of the population leads to increasing number of elderly patients with chronic kidney failure, initiating long-term dialysis treatment. ERA - EDTA (European Renal Association - European Dialysis and Transplant Association was issued in 2016 for treatment of elderly patients with advanced kidney disease. For easier orientation in the issue to withold or withdraw from dialysis therapy recommendations have been made in 2010 Renal Physicians Association (RPA). The issue of chronic kidney disease is also dealt with nurses organization The European Dialysis and Transplant Nurses Association/European Renal Care Association (EDTNA/ERCA). Aim: The aim of this research was to point out the reasons, for which patients with chronic kidney failure decide to withold or withdraw from dialysis therapy. Methodology: The thesis was performed through a qualitative research. Data were collected by individual guided interviews. The research sample included five participants. Four individuals decided to withold or withdraw from dialysis therapy and were further followed in the pre-dialysis nephrology outpatient clinic. One participant, who initially had started dialysis therapy acutely, later repeatedly stopped it and started it again with the...

Comparison of the efficacy of arterial flushing sets: heparinized flush versus saline flush
Strychová, Zdenka ; Di Cara, Veronika (advisor) ; Dynáková, Šárka (referee)
(v AJ) The aim of this diploma thesis was namely to find out whether the saline solution, intended for the continuous flushing of arterial catheters, is as effective in preventing arterial blockage as compared to saline solution with heparin. In this diploma thesis I also deal with the comparison of material consumption by using both methods of flushing arterial sets and the financial costs associated with them. Patients with diagnosis of sepsis or septic shock were included in the study. Pseudorandomization was used to group 52 patients into either experimental and control group. The testing was performed using a non-parametric Wilcoxon two-assay test and Fisher precision test. Based on the results of my study, regard to the occurrence of complications (catheter closure) an arterial catheter flushing by saline solution is as effective as flushing by a saline solution with heparin. My research also confirmed that the use of saline solution as a flushing solution is less costly (regarding material) and therefore it is advisable to use saline solution for flushing arterial catheters. Based on my study, saline solution is now used as a flushing solution of arterial catheters at an anesthesiology-resuscitation department, where only a saline solution with heparin was used for the flushing. Using saline...
Transfer of some aspects of palliative home care to an intensive care unit
Pešlová, Šárka ; Di Cara, Veronika (advisor) ; Dynáková, Šárka (referee)
(v AJ) According to research conducted by the STEM/MARK Agency in collaboration with Cesta domu organisation, 78% of the Czech population, wish to die at home. The fact is, however, that more than 60% of the chronically ill, who would benefit from a provision of specialized palliative care, die in beds of intensive, acute health care facilities or aftercare. The aim of this work was to determine whether, in what conditions and to what extent, it is possible to transfer some of the conditions for which the patients wish to die in a home environment, into intensive care and make the last period of life more bearable and pleasant for those who are dying here and find the most suitable solution to provide quality palliative care. This qualitative research was conducted in two steps, with the first phase led to the findings, for what reasons the sick desire to die at home and for what reasons their relatives agree to provide care to the sick. As a research method used was interpretative phenomenological analysis, the second phase was focused on the possibility of portability of these terms to intensive care and was based on the interviews with nurses serving in the Department of intensive care. The research method we had chosen was the method of grounded theory. The main result of this work is the...
