Mucosal immunity in upper respiratory tract diseases and autoimmunity diseases

Mucosal immune system comprises not only the major compartment of the immune system but also important interface with the outer environment. It is responsible in maintaining an intricate balance with the danger and non-danger stimuli of the outer world by employing specific anatomical features and unique functional mechanisms. Mucosal immune system has been long understudied, perhaps due to the limited accessibility, and its biological importance is thus still underevaluated. However, it has become evident that it is important to study mucosal immune system not only in local mucosal affections but also when uncovering pathogenic mechanisms and novel prevention strategies of organ specific autoimmune diseases such as type 1 diabetes. Thus, the first, more clinically oriented part of this thesis is focused on mucosal immune system of the upper respiratory tract in disease conditions - in nasal polyposis (NP). Because there is a substantial accumulation of eosinophils and neutrophils in the most frequent type of NP, we investigated and described increased expression of chemokine receptors CCR1 and CCR3 in NP versus nasal mucosa. Gut microbiota is important for our health and well-being, but when its composition is disrupted, it can induce or perpetuate several chronic inflammatory disorders, including inflammatory bowel diseases (IBD). The mechanisms which distinguish protective microbes from the deleterious or indifferent ones are largely unknown. The aim of this thesis was to study the interaction of the immune system with microbes that have different relationships to IBD pathogenesis. Escherichia coli is a predominant aerobic microorganism of the gastrointestinal tract. This species includes microbes implicated in induction of IBD as well as in its therapy. Four E. coli strains with different relations to IBD were selected for our experiments: E. coli Nissle 1917 (EcN), which has been successfully used in IBD therapy, E. coli strains LF82 and p19A, which have been implicated in the pathogenesis of IBD, and E. coli strain K6, which has neither been implicated in pathogenesis nor in protection from this disease. Adverse reactions to metals is relatively frequent in sensitive individuals, and may lead to the development of various diseases, such as lichen planus and lichenoid reaction and was also described effect of amalgam to the pathology of autoimmune diseases. In sensitive patients with known reactions to metals leads to atrophy of the oral mucosa and discomfort (burning, cutting, excessive salivation). Adverse effects of dental materials are described as an allergic reaction type IV delayed hypersensitivity. It disrupt the immune system, increased vascular permeability, tissue is often damaged by chronic inflammation and it is also activated antigen- nonspecific component of immunity. Monocytes are the key cells of the immune system, which act as antigen presenting cells, in particular after conversion in the tissues into macrophages are directly involved in the activation of immune responses and their stimulation is therefore crucial for the development of adverse reactions to dental materials. The fetus develops in the water environment and the newborn is 75% composed of just water. aquatic homeostasis of the organism is very tightly regulated , water is essential food component . Water is also used for recreational purposes. but important to remember that water is not our natural environment in postnatal life of a newborn is not equipped with perfect swimming reflex such as in the offspring of other mammals, and even so it is necessary to early develop swimming skills to gain a sense of security and satisfaction in water. Their severity is except in life-threatening to reduce the loss of self-sufficiency. It has subsequently negative impact on quality of life of old man. Accidents seriously threaten the health and life of the affected individual, but they also have far-reaching impact on its immediate social environment. Are costly to society. Cleft lip and palate is one of the most common congenital developmental defects . Morphological character defect has severe negative functional effects on swallowing , breathing, chewing and speech formation . aesthetically , the This is a significant handicap facial triad that affects the most overall facial appearance . Approximately 15 % of children have a defect associated with other disabilities , but isolated defect is neither lethal nor associated with mental failure . It is clear that the defect is a problem both health psychological and socio-economic . Therefore , the issue of treatment of patients with cleft recognized worldwide as so important that in this respect works close international cooperation, which is also included Czech Republic . It should be emphasized that the solution to the question of facial clefts has in the Czech Republic long tradition whose beginnings are associated with on behalf of Professor František Burian and Plastic Surgery Clinic in Prague, where he is currently the Center for cleft defects . The Czech Republic has center two , one already mentioned in Prague and one in Brno.
