Cervicogenic headache

Title Cervicogenic headache Objectives The aim of this master thesis is to present a comprehensive overview of the topic of cervicogenic headache. It focuses on the prevalence of this disease in the general population and on the cervical spine segment where the dysfunction in patients with cervical headache occurs the most frequently. It also focuses on the link between suboccipital muscles, cervical dura mater spinalis and headache on the effectiveness of different therapies for this diagnosis. Method The thesis is conceived as a scientific literature research. In order to obtain sources professional database such as Pubmed, PEDro, Medline, HighWire, Science direct were used. The thesis is divided into a theoretical and an analytical part. There have been 4 questions and 4 hypotheses set for the purposes of work the thesis. Results Based on the analysis of available studies, the thesis confirmed that the incidence of cervicogenic headache in the general population is not negligible. It has also been proved that the diagnosis is quite frequent in patients suffering from headache, which may represent up to 35 % of patients. The effect of menstrual cycle phases on muscle tone

Title: The effect of menstrual cycle phases on muscle tone Goals and methods: The aim of this thesis is to prove the effect of menstrual cycle phases and participating hormons on the viscoelastic properties of the sceletal calf muscle via the non-invasive myotonometer device. The research part of this thesis is a pilot study in which participated seven female probands aged inbetween 24 to 28. Each proband participated in four weekly tests. Only probands who had not used hormonal contraception for at least six months were selected. Results: The results of the tests are represented in graphs picturing parts of curves representing the force caused by the tip of the myotonometer. The muscle soleus tension is changing throughout the menstrual cycle but its impossible to eliminate other endogenous and exogenous factors. The measured relative values of muscle tension are different for each proband. The menstrual phase has the highest average value of muscle tension. The assumption was, that agistic-excentric contraction reduce muscle tension of musculus soleus after anaerobic load. The contralateral lower limb was control limb for comparison agistic-excentric contraction and passive relaxation. Muscle tension was measured in laboratory in Faculty of Physical Education and Sport, Charles University: five students were measured. As a measurement method was used myotonometry, which simulated palpation. Measured data were displayed by using an analog-digital convertor and consequently evaluated. The results of this study found out positive effect of agistic-excentric contraction on muscle tension. Control form of relaxation, passive relaxation, didn't influence muscle tension so significantly. Recruitment curve of H reflex in diagnostics of pseudoradicular syndrom

Title: Recruitment curve of H-reflex in diagnostics of non-radicular syndroms Objectives: The main objective of this work is to verify the conductivity of nerve structures examination recruitment curves H-reflex and M-wave in the EMG examination in probands with non-radicular syndrome L5/S1. Methods: We tested 12 probands clinical trial diagnosed with non-radicular syndrome L5/S1. The test sample was compared with an equally large group of control probands. H-reflex of m.soleus was examined by bipolar stimulation in the fossa poplitea of tibial nerve. The results of an electromyographic examination was evaluated to asymptomatic and symptomatic side. Amplitude of H-reflex and M-wave, slope of recruitment curve, peaks of recruitment curves for the H-reflex and M-wave and the ratio Hmax / Mmax were evaluated and statistically compared. Results: Results of non-radicular syndrome L5/S1showed no significant change in the H-reflex. Title: The biomechanical effects of levator ani muscle laceration injuries after vaginal delivery Objectives: The aim of this thesis is to sum up current knowledge about the normal structure and function of levator ani muscle and findings about dysfunctions of a pelvic floor. By means of biomechanical instruments to analyse how the lower part of levator ani muscle closes (puborectalis muscle) during unilateral avulsion lesion. On the basis of computer simulation of biomechanical properties of the pelvic floor during avulsion lesion to describe the theoretical solutions for the therapy of the main types of dysfunctions of the pelvic floor. Methods: Biomechanical analysis was used with a help of computer model of muscular pelvic floor with a help of the ABAQUS model. This model with matched properties of muscular tissue was influenced by the standard pressure which influences the pelvic floor for a person of 80 kilos standing at rest. This model simulated unilateral avulsion lesion and biomechanical variables were observed during compensatory activation of uninjured parts of levator with the activity 100%, 50% and 10%. Title: Viscousand elastic properties of soft tissue"in situ" Goals and methods: The aim of this thesis is to measure the viscoelastic properties of human soleus muscle and Achilles tendonin vivo and post mortem in situ. It is a pilot study that uses myotonometry as the method of measurement. Based on the response of connective tissues on deformation made by tip of myotonometer, resp. its viscoelastic properties, curves in graphsare created. Three main described parameters of thecurveare steepness, deflection and its surface area. Main goal of the experiment is to compare properties of different types of tissue and their potential differences while denervated or innervated. Results of this study may help with better understanding of the soft tissues behavior in response to manual therapeutic contact. The study also describes the differences between in vivo and post mortem tissue properties that may be help further studies which use post mortem tissues to predict in vivo behavior. Results: Soft tissues in vivo have higher viscosity. In comparison, post mortem tissues have significantly higher stiffness and energy dissipation than in vivo. Hallux valgus in the context of physiotherapy

Objectives: This work solves the problem of hallux valgus deformities, aetiology, biomechanics, the possibility of conservative therapy, surgery and post-operative treatment. Methods: Data collection was made by questionnare survey of people during thein hospitalization for hallux valgus surgery and than after 4-5 months. Questionnaires are targeted primarily to conservative therapy and post-operative rehabilitation therapy. Results: Due to the small number of probands (20 people) can not be drawn significant conclusions. Hallux valgus deformity is often solved by surgery rather than by conservative treatement, mostly because of the decision of patients for treatment, when the deformity is in an advanced stage. Postoperative rehabilitation treatment has a positive effect on recovery, particularly in terms of reducing pain and swelling, to facilitate walking (subjectively).

Hipotherapy is a complex therapy, broaden and recently very sought after rehabilitation method, which is being more and more recognized by experts for its broaden scene. It is a complex therapy thanks to involving medicine, psychology, pedagogy and also a social sphere. That's why is this method indicated at many different levels of disablement. It's being used with pacients with children's cerebral palsy, diverse level of cerebral dysfunction and multiple sclerosis. And it's the therapy's effect on multiple sclerosis that this thesis paper discusses. It also applies to brain and spinal traumas, scoliosis, back pain, orthopedic defects, cardiovascular disease, degenerative muscle disease, obesity, as well as it provides help at emotional and mental aspect, e.g. children with behavioral disorders and many others. At hipotherapy is being used a three dimensional (multidimensional) horse motion. Horse's spine moves up - down, forward - backward, right - left. Imitating a mechanisms of human walking, which leads to relaxation of spasticity. Impulses are transmitted onto back which brings interaction of impeller and back muscles, stand up pose, and posture interference. Title: Effect of exteroceptive stimulation on muscle tone after physical activity Aims: The aim of this diploma thesis is to summarize theoretical findings regarding the muscle tonus, biomechanical characteristics of soft tissues and the influence of exteroceptive stimulation on muscle tonus. In practical part, the influence of exteroceptive stimulation on muscle tonus after anaerobic exercise was evaluated using the measuring instrument - myotometer. Methods: Diploma thesis was approached as pilot and was processed at FTVS-UK in the form of analytical-comparative study on the group of six probands who experienced endurance Wingate test and then they underwent the exteroceptive stimulation technique. The main objective of this experiment was the evaluation of effect of stroking - the exteroceptive stimulation on muscle tonus. The muscle tonus was measured before the exertion, after anaerobic exertion, after passive rest and after exteroceptive stimulation. Measuring of muscle tonus of m. soleus was performed in FTVS-UK laboratory using myotonometer. The data were displayed through analog-digital convertor and were subsequently processed in program Matlab.
