Human resources management in selected hospitals in the CR.
LHOTSKÁ, Martina
Human resources management, that is the management of the most valuable thing hospitals possess, is currently a much debated issue. Human resources are the key factor for success in all activities a medical facility deals with. The theoretical part provides the readers with information concerning the issue of human resources management in general; the paper focuses on individual SAK CR standards relating to human resources management. The second part is devoted to research, which took place in two stages. It was a qualitative and quantitative research. In the qualitative part, we addressed the deputy managers of nursing care from selected hospitals, with whom we had semi-structured interviews. The questions for the interviews were based on the SAK CR standard on human resources management. Ritchie and Spencer framework analysis was used for processing the qualitative data. After processing the talks, the quantitative part of the research was prepared. We defined hypotheses; we developed a questionnaire for nurses working in the above-mentioned hospitals for the verification of the hypotheses. The entire survey was carried out in January - March 2010. The main objective of the survey was to map the compliance with the SAK CR standards in human resources management of nursing care in the selected hospitals. Based on the master objective, we identified seven sub-goals: 1. To determine whether the hospitals define the number of qualified staff needed to ensure adequate nursing care for their patients and a plan to develop the qualifications of the nursing staff. 2. To determine whether the hospital keep individual files for each member of their nursing. 3. To map whether the hospitals have a job description for each nursing care worker corresponding to their qualifications. 4. To determine whether nursing care workers are trained with respect to their positions at the beginning. 5. To map whether the hospitals define clearly general competences for nurses. 6. To determine whether the hospitals carry out training of their nursing staff on OSH. 7. To map whether the hospitals monitor in an appropriate form the satisfaction of their nursing staff. All the objectives of the paper have been met. Hypotheses H1: The hospitals plan a sufficient number of qualified nursing staff, H2: the nursing staff is enabled continuous training for skills upgrading, H4: Each nursing care worker knows their job description their corresponds to their qualification, H5: The nurses are familiar with the aspects of the adaptation process of new employees, H6: The nursing staff knows the determined scope of their competence, H7: The hospitals carry out the training of their nursing staff in OSH, and H8: The employees are free to express their views on satisfaction with their jobs at least once a year, have been confirmed. The nurses believe that the number of nursing staff in their departments is sufficient; they have the opportunity to participate in lifelong learning but they must bear the expenses themselves. Nurses know their job description and they observe it and they know the scope of their competences as well. The nurses have information about the existence, duration and method of completing the adaptation process of new employees. The nurses confirmed that regularly participate in OSH training, and they have an opportunity to express their satisfaction with the job at least once a year. Only one hypothesis, H3: Each member of the nursing team has some knowledge about their personal file, has not been confirmed. The nurses are unaware of the documents contained in their personal files, and they do not know where their personal files are stored. This paper may be useful to members of nursing teams who would like to learn more about the issue of human resources management with regard to the SAK CR standard.
Nurse as Risk Manager in the 21st Century Hospitals
LAŠTOVKOVÁ, Marie
The main intention of this thesis was to monitor the activities of nurses as risk managers in selected healthcare facilities. The first objective was to map the current state of accreditation procedures in the Czech Republic, the number of accredited facilities and the types of accreditation. Another objective was to find out in which healthcare facilities there is a nurse as a risk manager or who has the responsibility of a risk manager in healthcare facilities. The third objective was to explore the activities of nurses - risk managers. To achieve these objectives, the following research questions were set: 1st What is the current state of accreditation procedures in the Czech Republic? 2nd In which accredited facilities there is a nurse - a risk manager? 3rd research question: What are the activities and roles of a nurse - a risk manager? (What activities nurses carry out in the performance of their duties; whether nurses feel a change in the provision of nursing care; which risks and incidents nurses encounter in their facilities and what are their solutions; what is the level of communication between the parties in the risk management process in a given facility; which benefits are seen in risk management). The research was conducted with respondents from those healthcare facilities that were willing to cooperate. The research conducted is a qualitative one. The respondents underwent interviews according to the structure of categorized questions. Subsequently, transcripts of the interviews were made as well as the analysis and categorization of the data was carried out and presented in tables under the framework analysis according to Ritchie and Spencer. The research revealed the answers to the research questions. In no of the facilities that have responded to the questions there is a nurse with the function of a risk manager. Activities within the risk management are taken over by staff at other positions, mostly quality managers, head nurses or auditors. Detailed proportion of these functions is clearly presented in the accompanying graphs. Research question 3 yielded a great number of responses. For example, nurses who have competence in risk management play mainly the roles of communicators, defenders, authors, auditors and mentors. In exercising their functions, they have observed changes, as compared with the past, in the responsibilities of nurses, patient activity, greater transparency, the quality and safety of the care provided. They describe in detail the risks and incidents which they have met at their workplaces, and how they have been solved. The respondents see the advantages of risk management in the fact that it is a good tool for defending not only patients but also nurses themselves; that it is a tool to increase responsibilities and activities of patients and to improve the quality and safety of the care provided. All the responses are clearly presented in tables and confronted with the knowledge derived from professional literature and our own experience. The results of the thesis can be provided to the healthcare facilities where the research has been carried out; it could be provided to competent institutions and also, in the form of an informative brochure, to nurses and other healthcare personnel. We would suggest a higher level of education about risk management potential. Our suggestions also include changes to legislation - powers, the establishment of clear procedures and rules, and the regulation of education in risk management so that implementation of this tool is unified and becomes accessible to all facilities.
The Contribution of the Accreditation of the Hospital in Písek for Nursing
ČAPKOVÁ, Radka
The theoretical part of the work describes briefly the history of the Hospital of Písek and of nursing as an independent discipline. Further chapters are dedicated to accreditation, national accreditation standards, accreditation investigation, results of accreditation investigation and current status of accredited health care facilities in the Czech Republic. The research was executed with quantitative method. Two anonymous questionnaires focused on the target group respondents were used for data collection. The first questionnaire was intended for managers of individual departments of the Hospital of Písek and the other was for clients hospitalized in standard wards. The goal of the work was to ascertain whether the clients and the nurses perceive the accreditation of the Hospital of Písek positively, whether the nurses consider the accreditation beneficial for the health care facility and to find out the criteria according to which the clients select the health care institution. The bachelor work verified four hypotheses. Hypothesis number one, "The clients perceive the accreditation of the Hospital of Písek positively" was confirmed. Hypothesis number two, "The nurses perceive the accreditation of the Hospital of Písek positively" was confirmed too. Hypothesis number three, "The nurses perceive the accreditation of the hospital as beneficial for the hospital" was confirmed but hypothesis number four, "The main criterion for selection of health care facility is accreditation" was not confirmed because none of the clients selects the health care facility on the base of that facility being accredited. Nevertheless, most clients would prefer selecting an accredited health care facility for their next hospitalization. Through this work, it would be possible to point out the significance of accreditation both for clients and for workers of health care facilities. In my opinion, this work is beneficial for the Hospital of Písek because it shows whether the clients are informed about the accreditation of this hospital and whether they know how the accreditation can benefit them and what do they have guaranteed in the health care facility thanks to it.
The observance of the national accredited standards concerning the patient{\crq}s rights in the selected health establishment
POLÍVKOVÁ, Eva
The topic of this bachelor thesis is "Observation of national accreditation standards concerning rights of patients in the selected healthcare facility ". The main research objective was to map observation of rights of hospitalized patients in conformity with the accreditation standards. Four hypotheses were set up. The first hypothesis "Each patient is after the hospitalization informed about his/her rights". The second hypothesis "Patients are kept informed about all facts concerning their health condition". The third hypothesis "The information provided to each patient by the nurse within the education process is sufficient". The fourth hypothesis "The nurses posses sufficient knowledge about means of immobilization and restriction of patients". A quantitative research survey has been used to achieve our goals. The method of the research survey was inquiry through questionnaires. Two types of questionnaires were used for data collection. Questionnaires for patients and questionnaires for nurses were distributed in the hospital Nemocnice České Budějovice, a.s. The resulting data were processed into tables and diagrams using SPSS 15.0 and Microsoft Office Excel software. The results have shown that the first hypothesis was not confirmed: not all patients were informed about their rights after the hospitalization. The second hypothesis was confirmed: the patients have sufficient information about their health condition. The third hypothesis was not confirmed: the results failed to meet the purpose of the accreditation standard. The education of patients performed by the nurses is insufficient and, if the education is provided, the nurses fail to record in into the respective documents. Despite that, the patients assessed the education by nurses favorably. The fourth hypothesis was confirmed: the nurses have demonstrated sufficient knowledge about the application of restriction means. The bachelor thesis will be provided to the deputy director in charge of nursing in the hospital Nemocnice České Budějovice. The results may serve as a guideline for implementation of corrective actions and for preparation of the accreditation survey. I would like this bachelor thesis to serve not only to healthcare professionals but also to the general public as a valuable source of information about patients´ rights.
Problems lifelong learning of nurse
HALUZOVÁ, Jaroslava
My thesis addresses the issue of lifelong learning of nurses.Lifelong learing promotes high standards of profesional practice. The principles and values of lifelong learning are increasingly important. The objective of the research was to find out how motivated the health care personnel are to improve their skills and knowledge in the interest of their patients. In my research I also obsesrve if nurses have the responsibility to grow professionally and if they have enought access to information about lifelong education programs. For data collection the quantitative research was used. The summary of my research: nurses are concerned about lifelong learning, they are aware of the need to enhance the profesional practice. But they are not informed enough about relevant chances. The mandatory registration has enhanced the involvement in lifelong learning of nurses. But the most important motivating factor is the prospect for promotion and a better salary.

Preparedness of selected hospital to accreditation
ŠIMKOVÁ, Veronika
Accreditation is an adjustment mechanism which should reduce variations in medical care provided by heatlth care facilities. At present there are only twenty-one hospitals accredited by state. This accreditation guarantees that the medical care of the institution meets state standards. The hospital does not meet all standards so it cannot be accredited. It has, however, all preconditions to supplement the standards in near future. This work should help the hospital to assess its preparatory work for accreditation. I hope it will help other health care facilities and private people in their work concerned with accreditation.
Accreditation of Colleges and Universities as a prerequisite of quality and successfulness
Stejkoza, Josef ; Lipka, David (advisor) ; Chalupníček, Pavel (referee)
In the thesis, the main focus is concentrated on the regulation of tertiary education sector by means of the accreditation. According to The Academic Ranking of World Universities (ARWU), there are chosen the countries with the highest representation of universities in Top20-Top500. Especially in the point of view of their regulation, the systems of tertiary education of those countries are analyzed and divided into two groups: (1) In the first group, there are systems running liberally (or on more liberal base); and (2) In the second one, there are the systems considered as behaving more regulative to tertiary sector. The comparison of those two groups is trying to answer the question if the regulation might be the reason of successfulness or unsuccessfulness of the given group. In comparison of world's best practice, special attention is dedicated to the Czech Accreditation Committee and accreditation process transparency in the Czech Republic. Due to the lucidity, there is to found the illustration of the systems according to each analyzed country at the end of the thesis.
