Assessment of Parkinson’s Disease Based on Acoustic Analysis of Hypokinetic Dysarthria

Hypokinetická dysartrie (HD) je častým symptomem vyskytujícím se až u 90% pacientů trpících idiopatickou Parkinsonovou nemocí (PN), která výrazně přispívá k nepřirozenosti a nesrozumitelnosti řeči těchto pacientů. Hlavním cílem této disertační práce je prozkoumat možnosti použití kvantitativní paraklinické analýzy HD, s použitím parametrizace řeči, statistického zpracování a strojového učení, za účelem diagnózy a objektivního hodnocení PN. Tato práce dokazuje, že počítačová akustická analýza je schopná dostatečně popsat HD, speciálně tzv. dysprozodii, která se projevuje nedokonalou intonací a nepřirozeným tempem řeči. Navíc také dokazuje, že použití klinicky interpretovatelných akustických parametrů kvantifikujících různé aspekty HD, jako jsou fonace, artikulace a prozodie, může být použito k objektivnímu posouzení závažnosti motorických a nemotorických symptomů vyskytujících se u pacientů s PN. Dále tato práce prezentuje výzkum společných patofyziologických mechanizmů stojících za HD a zárazy v chůzi při PN. Nakonec tato práce dokazuje, že akustická analýza HD může být použita pro odhad progrese zárazů v chůzi v horizontu dvou let.
Quantification of Prosodic Impairment in Patients with Idiopathic Parkinson's Disease

This paper deals with quantitative analysis of prosodic impairment in idiopathic Parkinson’s disease (PD). Experimental dataset consisted of 97 PD patients and 55 healthy speakers. The prosodic features expressing monopitch, monoloudness and speech rate deficits are extracted from stress-modified reading task. Classification accuracies of 70:71% for females, 70:03% for males, and 63:20% for a mixture of both gender were achieved. According to permutation test (1000 permutations, a = 0:01), the models were shown statistically significant. Promising potential of prosodic features to identify HD was confirmed.
Potential of Prosodic Features to Estimate Degree of Parkinson's Disease Severity

This paper deals with non-invasive and objective Parkinson’s disease (PD) severity estimation. For this purpose, prosodic speech features expressing monopitch, monoloudness, and speech rate abnormalities were extracted from recordings of stress-modified reading task acquired from 72 patients with idiopathic PD. Using a single feature regression (esimating values of subjective clinical rating scales) with classification and regression algorithm, following performance in terms of root mean squared error was achieved: 10.72 (UPDRS III), 2.16 (UPDRS IV), 4.76 (FOG-Q), 17.89 (NMSS), 2.13 (RBDSQ), 6.43 (ACE-R), 1.41 (MMSE), and 4.82 (BDI). These results show a promising potential of prosodic speech features in the field of objective assessment of PD severity.
Acoustic Analysis of Sentences Complicated for Articulation in Patients with Parkinson's Disease

This paper deals with acoustic analysis of hypokinetic dysarthria. Hypokinetic dysarthria is a speech motor dysfunction that is present in approximately 90% of patients with Parkinson’s disease (PD). The work is mainly focused on parameterization techniques that can be used to diagnose or monitor this disease as well as estimate its progress. Acoustic analysis can be used to estimate a grade of hypokinetic dysarthria in fields of phonation, articulation, prosody and speech fluency. Regarding the parameterization, new features based on RASTA method were proposed. The analysis is based on parametrization of sentences complicated for articulation. Experimental dataset consists of 101 PD patients with different disease progress and 53 healthy controls. For the purpose of feature selection we employed mRMR (minimum Redundancy Maximum Relevance) method.
Speech therapy for Parkinson's disease

This diploma thesis presents an analysis of speech impairments accompanying Parkinson's disease. This paper is divided into two sections - theoretical and practical. The first section gives a brief overview of available Czech and foreign literature and articles about this neurodegenerative disease. The section examines the questions of its aetiology, symptomatology, diagnostics and treatment. There can be found also a description of speech impairment connected with this disease, which is mainly hypokinetic and hyperkinetic dysarthria and dysphagia. It also deals with the problem of other limitations in communication such as facial bradykinesia or speech intelligibility. In the last chapter of theoretical part is described speech therapy and intervention with the emphasis on therapy, diagnostics and the effects of pharmacology an non-pharmacological treatment such as deep brain stimulation on speech performance in Parkinson's disease. The research part analyses speech impairment of people with Parkinson's disease. The aim of this diploma thesis was examination of this speech impairment using the Test 3F: Dysarthria profile and patient's own perception of this specific speech disorder. Application for the calculation of speech features describing hypokinetic dysarthria

This thesis is about design and implementation of application for computing speech parameters on people with Parkinson disease. At the beginning is generaly described Parkinson disease and Hypokinetic dysarthria and how it affects the speech and speech parameters when it occurs. Mainly there are described areas of speech like phonation, prosody, articulation and fluent speech. As a part of next topic this thesis describes specific speech parameters with bigger meaning during diagnosis Parkinson disease and it's progress over the time. There are also mentioned few significant studies dealing with examination of speech of the subjects with diagnoses of Parkinson disease and computing some speech parameters in order to analyze their speech impairments. Part of the thesis is description of implemented standalone application for calculating, exporting and visualizing of speech parameters from selected sound records.
Django framework based web application for objective analysis of hypokinetic dysarthria

This master´s thesis deals with the calculation of parameters that would be able to differentiate healthy speech and speech impaired by hypokinetic dysarthria. There was staged hypokinetic dysarthria, which is a motoric disorder of speech and vocal tract. Were studied speech signal processing methods. Further parameters were studied, which could well differentiate healthy and diseased speech. Subsequently, these parameters were programmed in Python programming language. The next step was to create a web application in Django framework, which is used for the analysis of the dyzartic speech.
Correlation analysis of Parkinson's disease in the acoustic field

The topic of this bachelor thesis is the correlation analysis of Parkinson’s disease in the acoustic field. The first part is about the Parkinson disease and its symptoms. It looks closer on problems with speech production, which is called hypokinetic disarthria, and describes the causes of the problems as well as the kind of treatment that is used. The next part involves a study of the pre–processing of signal, i.e. removing the direct component, a preemphasis and a segmentation to smaller frames. Afterwards, individual parameters are calculated in the next step. It is also necessary to calculate simple statistics, for example a median, a standard deviation, etc. after the calculation of some parameters. The calculation of Pearson’s and Spearman’s correlation coefficients is included. Moreover, a block diagram for the data processing is suggested, which involves a description of the functions of the individual blocks. The program is explained in the practical part, which also features parts of the tables with parameters' values and the calculated coefficients. As the conclusion of the work, there are graphs which display correlations of the parameters and the paraclinical data.
Acoustic analysis of gender-related patterns in Parkinson's disease

The bachelor's thesis is about acoustic analysis of gender-related patterns in Parkinson's disease by analysing speech task: reading passage. Parkinson's disease manifests in all subsystems involved in speech production (respiration, phonation, articulation and prosody). The aim of this thesis is familirization with symptoms of this disorder and speech parameters influenced by this disorder. Thesis contains preprocessing, parametrization of speech signal and statistic analysis of parameters. System of speech signal processing is created in MATLAB programming language.
De-identification of speakers with hypokinetic dysarthria

This paper discuses design and implementation of a system that performs de-identification of speech recordings of patients suffering from Parkinson's disease. The paper describes causes and symptoms of Parkinson's disease and effects of hypokinetic dysarthria on speech. Part of the paper is devoted to speech features that can be used for diagnosing hypokinetic dysarthria from speech. It also describes ways of speech de-identification and system for evaluating results using recognition of speakers and patients. De-identification system uses vocal tract length normalization (VTLN) and evaluating system uses Gaussian mixture models (GMM). PARCZ database was used for testing. It contains recordings of speech of patients affected by Parkinson's disease and control speakers.
