Patient data trasfer over GSM

This diploma thesis is concerned with possibilities of patient data transfer from a pacemaker or implantable cardioverter-defibrillator to physician over GSM. Theoretical part describes options of data transfer in GSM networks, data appropriate for sending and a structure of message is proposed. A device, that is able to send medical data in a form of SMS messages is designed and constructed. The device uses a Freescale MC68HC908GP32 microcontroller, character display with a Hitachi HD44780 controller and a cell phone Sony CMD-J70. The program for microcontroller is written in assembler for HC08. For tabular view of received messages, an application software for PC was created.
Electromagnetic interference and pacemakers

This thesis deals with electromagnetic compatibility of implantable electro-medical devices using in cardiology. The main aim of this thesis is detection of the resistance of implantable pacemakers (PM) and implantable cardioverter defibrillators (ICD) to the electromagnetic interference. In the first part of this thesis, a function of the heart, which is necessary for understanding of PM and ICD operation, is described. The function and construction of PM and ICD is detailed in the theoretical part as well. The next part is focused on the electromagnetic compatibility. The next part is description of the measurement method and the experimental arrangement used for practical experiments with electromagnetic resistance of PM and ICD.
External pacemaker

This master´s thesis deals with the problem of heart stimulation by using external pacemaker. Such external pacemaker is used for a short – time stimulation of patient to overcome the time before the implantation of such apparatus. The master´s thesis informs about the basic function of heart and possible heart disorders which are treated by pacemaker. Great deal of master´s thesis is devoted to the design of external pacemaker wiring. This section describes the possibilities of controlling the pacemaker, circuits for detection R-R interval, a design of output circuit eith regard to required of stimulation and pacemaker feeding. The program Pspice was used to check the functional appropriateness of designed circuits and it was successively simulated in a non - soldering field.
Cost analysis of remote and in office pacemaker check ups after device replacement from the perspective of health care payer

Current pacemakers allow check up of their function using telemonitoring. Even so, the majority of the checks are carried out on an outpatient basis. It is not known, whether remote follow ups of the patients after pacemaker replacement would be cost-effective compared to in-office controls. A retrospective analysis of patients files of patients after pacemaker replacement in 2002-2005 in Faculty hospital of Kralovske Vinohrady was done. A total of 1407 records from the years 2002-2015 was analyzed. It was demonstrated that the vast majority (96%) of planned controls of these patients was not associated with a change of program of the pacemaker, so it could be performed remotely. Based on the obtained data a model, which compared the cost-effectiveness of outpatient and theoretically made remote controls of pacemakers was created. Using an analysis of cost minimalization, it was found that the remote controls of pacemakers, was not associated with a significant reduction of the costs compared to the ambulatory controls.
Treatment of cardiac arrhythmias related to nursing care

Nursing care continuing the treatment of cardiac arrhythmias is an inseparable part of the patient?s care. As there are various types of arrhythmias and different types of treatment, similarly, the subsequent nursing care has its specifics as well. It relates not only to the post-treatment care but also to pre-surgery preparation and subsequent education. The theoretical part of the thesis contains the information on arrhythmias, their development and treatment. Organizations associating nurses working in arrhythmologic centres are mentioned as well. The practical part consists of the survey research. The quantitative enquiry is conducted in arrhythmologic centres by means of questionnaires addressing the nurses. The qualitative part contains interviews with patients who underwent some of the types of cardiac arrhythmia treatment. The objective of the thesis was to find out whether the nurses are aware of the specifics of the nursing care that follows the treatment of cardiac arrhythmia, in which stage the treatment is the most demanding for nurses and whether they can apply standardized nursing procedures in their work. Another objective is focused on mapping of the educative activity within the nursing care. Interviews with the patients are aimed at the areas of admission, prior to surgery, after surgery and during education. They should outline the view from ?the other side?. All the defined hypotheses were confirmed. The cardinal hypothesis of the thesis was the assumption that nurses, when providing nursing care to patients undergoing treatment of cardiac arrhythmias, do not work according to standardized nursing procedures. Subsequently, a draft of standardized nursing procedure focused on nursing care of patients undergoing implantation of a permanent cardiostimulator was prepared ? it could be applied in the University Hospital in Plzeň where most of the enquiry was conducted.
The Influenece of the Permanent Carciac Stimulator on the Life of the Client

This work deals with the impact of permanent pacemakers on patients´ lives. Over 6,000 permanent pacemakers are implanted every year in the Czech Republic. Cardiac pacing is a method of medical treatment through slow hearth rhythms. Although pacemakers improve the health of their holders, the situation the patients have to face is quite demanding and affects every aspect of their life. The aim of this work was to examine and identify the care for the clients before and after pacemaker implantations, who instructed them and who helped them most to deal with the given situation. We inquired whether the patients had enough information about the life with a pacemaker and what kind of changes occurred in their lives after the implantation. We chose a qualitative research to achieve the above mentioned goals. We interviewed 7 clients who had lived with a pacemaker for more than one year. The interviews were held during the patients´ visits to the cardiac pacing clinic within their regular checkups. The research revealed that the care for these patients was very good. All the patients were satisfied. They had enough information both from the doctors and the nurses. Many clients found information in information leaflets and media, in particular on the Internet. Most of the clients found the necessity of an implantation quite stressful at the beginning and had to cope with it. Despite their initial worries and frequent monitoring of their heart rhythms, they managed to adapt to it and no longer think about it. Their life has grown better in most of the cases. The research proves that pacemaker significantly affects the health of the clients but does not significantly change their personal lives.
