Nurses´opinion on regulation fees in public health care in connection with nursing care provided

This bachelor work is focused on regulation fees in public health care that sere introduced in the Czech Republic on 1/1/2008. The issue of regulation fees in public health care is a very delicate and discussed topic at present, still filling front pages of newspapers, magazines and television programs after almost a year. The work is divided into theoretical and practical part. The theoretical part lists the principles when the regulation fee is and is not to be paid. We mention also the protective limit and the office job related to collection of regulation fees. We describe the situation abroad to give an idea how regulation fees work in neighbouring states. The practical part of this work had three goals. The first goal was to find out how nurses perceive the introduction of regulation fees in public health care. The second goal was to find out whether the regulation fees affect the nursing care provided, and the third goal of our research was to find out what nurses consider the most problem-loaded area within the regulation fees. Those goals were met. Research questions were set on the base of the goals. How nurses perceive regulation fees in public health care, what they would like to change in the collection of regulation fees, what the provided nursing care should look like from the nurses{\crq} point of view and what they consider the most problem-loaded area within the regulation fees. The research questions were answered. Qualitative investigation, questioning method were used to process the bachelor work. The data collection was implemented in the technique of non-standardized structured interview. Our inquiry took place in the Nemocnice České Budějovice a.s. and the Nemocnice Strakonice a.s. hospitals. The nurses{\crq} answers were processed into twenty pieces of casuistic. The investigation took place in February 2009. The results of our bachelor work could serve as feedback for the hospitals where the investigation was implemented. Subsequently they could contribute to create a united form for collection of regulation fees, both from the point of view of payment and of office work. Further the results could serve to support cooperation between the patient and the health care staff. I would also suggest training events for health care staff so that they are able to master problem-loaded situations. For example how to approach a patient who refuses to pay the regulation fee. I would suggest regular training events also for ward sisters and head nurses in the area of legislation of regulation fees.
Level on public knowledge concerning of helth insurence policy

Public health insurance is an important part of the Czech health system. In the Czech Republic the model of national health insurance is administered using system of several insurance companies. The main target of these companies is to collect insurance rate from the payers and to cover health care provided to the insuree. The aim of this work is to map and assess professional and non - professional public knowledge of the system of the public health insurance. Three goals were set within the work. The first one was to find out the level of the professional public knowledge of the public health insurance. The second one was to find out the level of the non - professional public knowledge of the public health insurance. The third one was to compare both groups knowledge of the public health insurance. To realize the above mentioned aims three hypotheses were set. Hypothesis No.1: Most of the professional public is informed about the valid legal arrangement of the public health insurance. Hypothesis No. 2: Most of the non -professional public is informed about the valid legal arrangement of the public health insurance. Hypothesis No. 3: There is no difference between the professional and non - professional public knowledge of the valid legal arrangement of the public health insurance. To prove the set hypotheses the form of quantitative research was used. The technique of questionnaire was used for data collecting. For practical part the identical questionnaire for both professional and non - professional public was used. The research group of the questionnaire investigation was formed by respondents living and working in České Budějovice. The professional public within the research work was formed by people who work or worked in the health or social sphere. The final research group was formed by 200 respondents of professional and 200 respondents of non - professional public. The target of the thesis and partial aims were accomplished. The first two hypotheses were proved. The third one was not proved by the research work. It has been investigated what areas is professional and non - professional public most or less informed in. The results of the research work may be used for publication in the professional journals.
Implementation of Health Policy Reform in the Czech Republic.

The introduction of user fees and surcharges for medicinal products and foods for special medical purposes is considered to be one of the steps preceding the implementation of health policy reform. As of 1 January 2008, four basic fees were introduced in the Czech healthcare system with the objective of reducing unnecessary demand and misuse of health care. The objective of my Diploma Thesis was to map and evaluate the effects of the introduction of user fees on patients{\crq} number of visits to outpatient specialists. Three main objectives were formulated during my work on this thesis. The first objective was to provide a coherent overview of the issues related to the introduction and use of fees introduced on 1 January 2008 for outpatient specialists. The second objective was to compare patients{\crq} rate of visits to outpatient specialists with the year 2007. The third objective was to evaluate whether the patients{\crq} number of visits to outpatient specialists decreased in relation to age, gender, care provided, and kind of disease. Subsequently, four hypotheses were formulated. Hypothesis 1: The patients{\crq}rate of visits to outpatient specialists decreased compared to the year 2007 as a result of the introduction of user fees. Hypothesis 2: Compared to the year 2007, the introduction of user fees resulted in no considerable decrease in the rate of visits for patients with chronic diseases. Hypothesis 3: The impact of user fees on the rate of visits to outpatient specialists will be comparable for both genders. Hypothesis 4: The introduction of user fees will not result in a prolongation of the interval between the respective visits of patients to outpatient specialists. Quantitative research and method of questioning were used to achieve the goals and verify the hypotheses. The questionnaire technique was used for data collection. The research survey group for the questionnaire comprised physicians with outpatient specialties practicing in South Bohemia, Hradec Králové, Liberec,and Plzeň Regions, Capital City of Prague, Central Bohemia, Ústí and Zlín regions. The final research group comprised 79 specialist physicians with specialties as follows: allergology and clinical immunology, anesthesiology, dermatovenerology, gynecology, surgery, internal medicine, pneumology, ophthalmology, ENT, oncology, orthopedics, neurology, psychiatry, rehabilitation, and urology. The Diploma Thesis met its main objectives, as well as its partial objectives. The second, third, and fourth hypotheses were confirmed. The first hypothesis was not verified successfully in this research. The results of my research suggest that user fees had an impact on patients{\crq} rate of visits to outpatient specialists and can be used for publication in professional journals.
Unemployment and its impact on number of elective surgeries at surgery department

SUMMARY A surgery ward is one of the crucial departments in the České Budějovice Hospital. It provides a highly specialized care not only to patients from the district of České Budějovice, but also to people from the whole South-Bohemian region. Nearly 4,000 patients are treated in this ward every year. In my work I focused on a group of people who are able to work, but at the time of hospitalization they had no job and were registered in the employment agency and they came to the surgical ward for optional surgeries. The reasons why they did so could be as follows: they either neglected the care of their health due to a large amount of tasks at work, or they postponed the surgery since they were worried that they could lose their job due to a long sick leave. The objective of the work was to identify the influence of employment on decisions of clients to undergo surgeries. Questionnaires were used to gather data; they were distributed to patients hospitalized in the surgical ward of Nemocnice České Budějovice a.s. (the České Budějovice Hospital). The hypothesis which assumed that people registered in the employment agency for a short time have more surgeries than people registered in the employment agency for a long time was confirmed in the degree work. The second hypothesis assuming that people registered in the employment agency have more optional surgeries than employed people was confirmed as well. The third hypothesis where I assumed that the fees charged for hospital stays would influence the length of hospitalization of unemployed people was not confirmed since political changes resulted in the conclusion of a donation agreement between Nemocnice České Budějovice, a.s. and the South-Bohemian region; the agreement enables people to take advantage of the South-Bohemian region{\crq}s donation to cover the hospital fees starting from 1 February 2009. The results of my work could be used as arguments in solving the unemployment policy.

Privatization of the health insurance companies under the proposed reform of health service

The object of this thesis is the status and operation of health insurance companies in the Czech Republic. The theoretical part focuses on the historical development, function and the position of health insurance companies in the Czech Republic and the development of public health insurance. It analyzes the problems of management of insurance companies, their financing, the allocation of funds which they hold and their management. In addition, my work deals with the U.S. health insurance system, with its market-based principles that are subsequently compared with the system in the Czech Republic. Last but not least, we are dealing with proposed changes to health insurance companies that are part of the proposed reform of the health sector, their potential benefits or disadvantages. There is examined the most recent part of health reform, the regulatory charges, in the last stage.
Impact of regulatory fee

This study is focused on the analysis of regulatory fee and its impact on the financing of health care system of the Czech Republic. The study consists of two parts. The first chapter describes the principles of regulation and the medical service from the economical point of view. Furthemore, it evaluates the reasons for regulatory fee and the impact of chosen factors on the health of population. The second part considers the influence of regulatory fee in Czech health care system. The problematics of solidarity between relatively healthy and seriously ill people is also mentioned there.
Zavedení regulačních poplatků ve zdravotnictví

Tato bakalářská práce zkoumá nejvíce diskutované opatření v rámci reformy zdravotnictví ministra MUDr. Tomáše Julínka ? zavedení regulačních poplatků u lékaře. Především se zabývá účelem použití tohoto opatření, tedy snahou zamezit nadměrnému plýtvání finančních prostředků ve zdravotnictví a zneužívání zdravotnických služeb ze strany pacientů. V teoretické části je jako příčina tohoto chování označena metoda financování zdravotní péče pomocí zdravotního pojištění a dále přiblížen koncept poplatků včetně jejich pozitivních a negativních důsledků. Analytická část demonstruje teoretické závěry o nadužívání v systému českého zdravotnictví a zkoumá konkrétní dopady poplatků zavedených na Slovensku v letech 2003 ? 2006.
