Nurse and patient with ankylosing spondylitis in ambulant care

Bakalářská práce se zabývá péčí o pacienty s ankylozující spondylitidou (dále AS) z pohledu všeobecných sester a pacientů s touto chorobou. Aby byla pacientům s AS poskytnuta adekvátní péče, sestry by měly mít teoretické znalosti o tomto onemocnění a vědět, co od nich pacienti očekávají. Cílem práce bylo zjistit očekávání pacientů s AS od sester v ambulantní péči a intervence, na které se sestry v revmatologických ambulancích nejčastěji zaměřují při péči o pacienty s touto chorobou. Pro empirickou část práce bylo zvoleno kvalitativní výzkumné šetření. Za pomocí polostrukturovaných rozhovorů jsme získávali potřebné informace od dvou výzkumných vzorků. První výzkumný vzorek tvořilo pět všeobecných sester pracujících v revmatologických ambulancích s pacienty trpícími AS. Sestrám bylo položeno dvanáct otázek zaměřených na ambulantní péči o tyto pacienty. Druhý výzkumný vzorek tvořilo pět pacientů trpících AS. Pacientům bylo položeno osm otázek, zaměřených na jejich očekávání od sester, se kterými se setkávají v revmatologických ambulancích a na zájem těchto sester o potřeby pacientů s AS. Z analýzy prvního výzkumného vzorku vyplynulo, že se sestry v revmatologických ambulancích nejčastěji zaměřují na edukaci a komunikaci s pacientem. Analýza odpovědí u druhého výzkumného vzorku ukázala, že sestry v ambulantní péči očekávání pacientů naplňují.
Laboratory methods using in diagnostic morbus Bechterew

The aim of my bachelor thesis was to get familiar with laboratory methods, which are used in the diagnostics of morbus Bechterew. Very important for the diagnostics of morbus Bechterew is the marker HLA-B27, which is identified by screening, with the help of flow cytometer and then is the presence confirmed or refuted with the help of molecular-biological methods. Other laboratory examination, which is essential when it comes to interception and observation of the activities of morbus Bechterew, is the determination of the C-reactive protein level, which is an indicator of inflammatory reaction in the body of the patient. The last examination I got familiar with was the determination of the sedimentation speed of erythrocytes, which is used as an indicator of ongoing inflammation in the body. In the theoretical part of the thesis I focused primarily on the definition of morbus Bechterew. I dealt with the question what are the symptoms of the disease, then also according to which criteria is the disease diagnosed and what treatment is applied. In the second part of the theoretical part I focused primarily on the major histocompatibility complex, which is highly connected with morbus Bechterew. I dealt mostly with the structure and presentation of antigens by the means of MHC molecules of class I. I also mentioned the structure and the function of C-reactive protein. In the practical part I described laboratory methods used in diagnostics of morbus Bechterew. The first method I tried was screening examination of HLA-B27 marker by the means of flow cytometry. I introduced how the following processes should look like: collection of biological material, preparation of the sample and the analysis made on the flow cytometer. At the end of this part I put an example of the analysis results, how do the results look like when they are positive, negative, or in the grey zone. As a second method I introduced genetic typing of HLA-B27 molecule, which is made by the means of real time PCR. I mentioned the preanalytical part and the process of preparation. Moreover I dealt with the measuring of C-reactive protein by the means of nephelometry and then I also mentioned examination with the help of a test strip. The last method I tried was measuring the sedimentation speed of erythrocytes, I mentioned the principle of method, the process of measuring and the measured results as well. In the last part of my bachelor thesis I worked with the data I got from Motol University Hospital, more precisely from the Department of Immunology, 2nd Faculty of Medicine of Medicine, Charles University in Prague and Motol University Hospital. During the year 2017, overall 452 patients were tested on the presence of HLA-B27 marker and 75 of them tested positive. Then I worked only with HLA-B27 positive patients. I researched how much is the disease represented in both sexes, what age do the HLA-B27 positive patients have and what is their diagnosis.
Epidemiological aspects of inflammatory rheumatic diseases and diffusional diseases of binding tissue.

v anglickém jazyce Introduction: No information was known about frequency of common inflammatory disorders in rheumatology in the Czech Republic. Aims of the study: To estimate the standardized annual incidence (INC) and point prevalence (PREV) of six diseases (rheumatoid arthritis-RA, juvenile idiopathic arthritis-JIA, gout, psoriatic arthritis-PsA, ankylosing spondylitis-AS, reactive arthritis-ReA) in a population-based study in two regions of the Czech Republic (CR). Methods: INC: Incident cases were registered on condition that the definite diagnosis was confirmed according to existing classification criteria during the study period. PREV was studied on the basis of identification of established diagnoses at a time point. Crude rates were standardized for age and sex. Results: Both INC and PREV are shown per 100.000 inhabitants. RA INC:31 (95%CI 20-42), PREV:610 (95%CI 561-658). Gout-INC:41 (95%CI 28-53), PREV:300 (95% CI 266-334). JIA-INC: 13 (95% CI 1-20), PREV:140 (95%CI 117-280). The casuistry of physiotherapeutic care for patients with the diagnosis of ankylosing spondylitis

The thesis is split into two parts - the general one and the special one. The general part focuses on the theory of the disease of ankylosing spondylitis and describes its placement in the field of rheumatology, its clinical image and diagnosis, which play an integral part in the treatment. Furthermore, it focuses on the complex treatment of this diagnosis, mainly on kinesitherapy. The general part is based on both Czech and foreign literature, which is listed in the chapter Reference List. The special part describes the casuistry of a patient with the diagnosis of ankylosing spondylitis. This part contains the patients entire case history and an entry kinesiology examination, upon which a short term and a long term physiotherapeutic plan is based. It then presents applied therapies and an evaluation of their effectiveness. I gathered the basis for this thesis during my internship in the Institute of rheumatology in Prague, which took place from the 4th until the 29th of January 2016.
Population differences in the distribution of HLA-B27 allele in ankylosing spondylitis

Ankylosing spondylitis (AS) is a chronic inflammatory disease affecting mainly the spine and the sacroiliac joints. This disorder has a genetic background and is strongly associated with HLA-B27 gene which occurs in about 90 % of patients. The prevalence of ankylosing spondylitis usually correlates with the frequency of HLA-B27. The strength of association HLA-B27 with AS varies between different populations and is also distinct for individual alleles of HLA-B27 gene. Some alleles can increase a risk of ankylosing spondylitis while others may have a protective effect. This work deals with the population differences in the occurrence of HLA-B27 alleles and their relation to development of ankylosing spondylitis. Case study of physiotherapy treatment of the patient with the diagnosis of ankylosing spondylitis.

Title: Case study of physiotherapy treatment of the patient with the diagnosis of ankylosing spondylitis. Objective: The main target of this bachelor's thesis was to take up with the issue of the ankylosing spondylitis, as the rheumatic disease and to outline the possibilities of its treatment. The next target was the elaboration of the case study of the patient with this diagnose, followed by the appraisal of the therapy's effect. Methods: In the general part is concerned with the Ankylosing spondylitis issue, namely with its symptoms, manifestations, methods of examination and a subsequent treatment of this disease. The second, special section of this bachelor thesis is focused on the individual patient with ankylosing spondylitis, physiotheapy assesment, subsequent therapy and the final effect of it. Results: The results of the physiotherapeutic care of the patient were observed to be positive in objective (clinical) and subjective way. The painful situations became milder and the patient assumed a positive approach to the kinesitherapy, which she didn't have till now. Title: Case study of physiotherapy treatment of a patient with the diagnosis of ankylosing spondylitis The target: The main target of this bachelor's thesis was to highlight the problems of rheumatic disease ankylosing spondylitis. The emphasis is placed on the role of the physiotherapy practise as a part of treatment. Additionally there are also written possibilities of pharmacological treatment. Methods: The bachelor's thesis is divided into two parts. In the general part I deal with the symptoms of the disease on the basis is subsequently diagnosed the ankylosing spondylitis. Additionally there are also analyzed the typical symptoms of the disease, methods of examination and treatment. The special section is focused on the specific and individual patient with ankylosing spodylitis, subsequent therapy and the effect of therapy. This case study was created during the passed practice in the Institute of Rheumatology in Prague the period 1st-6th January 2014. Title: Case study of physiotherapy treatment of the pacient with the diagnosis of ankylosing spondylitis. Objectives: The aim of the bachelor thesis is familiarization and resume with the general issues of ankylosing spondylitis. The next is processing of case study of physiotherapy treatment of the pacient with the diagnosis of ankylosing spondylitis and create a complex perspective on the rehabilitation care. Methods: The thesis consists of the general part, where a basic disease is described in more detail, including possible complications, way of examination and therapy. The second, special part is focused on concrete patient with ankylosing spondylitis. This part described patient's medical history, entery assesment, follow-up therapy and resulting efect of the therapy. Results: The problems of disease were theoretically clarified.
