Concurrent possibilities of minimallyinvasive surgical treatment of kidney tumours

Minimally invasive surgery in the treatment of kidney tumours has developed since the beginning of the nineties. A lot of specialized or top laparoscopic urological centres so-called "centres of excellence" have been created since the first laparoscopic radical nephrectomy (LRN). These centres have thanks to the centralization of kidney tumour patients sufficient cohorts for evaluation of operative data, operative technique and oncological results. A lot of large studies have been published so far, showing comparable oncological results with open nephrectomy and advantages of miniinvasivity ( shorter hospitalization time, less analgesia requirements, better recovery and cosmetics). LRN is considered a standard of care in the treatment of cT1-2 (≤ 8cm) renal tumours in patients who are not candidates for nephron- sparing surgery. In our department in Plzeň, we have been performing LRN since January 2003. The majority of procedures has been performed by transperitoneal approach. The first LRN in the Czech Republic was done in April 1998 within XII. Conference of Czech Urologic Society by guesting prof. Clayman. Selected Theoretical and Practical Aspects of the Quality of Nursing Care

The subject of this diploma paper is the quality of The Health Sevice. This is a major issue of interest, not only to medical experts, but patiens and the general public. Due to increasing competititon within The Nursery Health Service, hospitals must now, monitor and evaluate the quality of patient care and instigate changes as necessary . A glossary of words used within this paper, relating to quality in The Helath Service, is found in the Introduction. Systems of quality assurance, accreditation and resource issues are also introduced. The research focuses on patient satisfaction with the nursing care in the Intensive Care Unit within the surgery department of IKEM. The answers are categorised and either presented graphically or compared statistically. The conclusion contains recommmendations for changes that would improve the quality of nursing care.

The nursing care of a geriatric patient who suffers from chronic pain

The thesis deals with nursing care of a geriatric patient who suffers from chronic pain and is based on patients' opinions. The theoretical part defines the term "chronic pain" and characterizes its' causes. It also describes various ways of diagnosing chronic pain, ways in which chronic pain can be monitored and how could chronic pain be treated. There is a general nursing plan - according to the model of M.Gordonova - for a geriatric patient with chronic pain treatment worked out at the other part of the thesis. The empiric part of the work deals with patients' opinions. For the research questionnaire was used as the most suitable method for given purpose. 54 geriatric patients suffering from chronic pain, who are hospitalized either in internal department or department for the elderly in Havlíčkův Brod, took part in the research. They were then dividend into 2 groups - men and women - which was also taken into account when evaluating the answers. The questionnaire is focused on character of pain, its' location and intesity with regard to previous week. The target is to find out what patient's preferences in chronic pain treatment are and the level to which daily activities were influenced by pain over the last week. Measurement of the B0 meson lifetime at ATLAS detector

Title: Measurement of the B0 d meson lifetime at ATLAS detector Author: Pavel Řezníček Department: Institute of Particle and Nuclear Physics Supervisor of the doctoral thesis: Dr. Zdeněk Doležal Supervisor's e-mail address: Zdenek.Dolezal@mff.cuni.cz Abstract: The lifetime of B0 d mesons is determined from their decays B0 d → J/ψK∗0 reconstructed in ATLAS experiment at the LHC using pp collision data at a center-of-mass energy of 7 TeV and corresponding to integrated luminosity of 40 pb−1 . The lifetime, extracted from the simultaneous unbinned maximum likelihood mass-lifetime fit, is 1.51±0.04 (stat.)±0.04 (syst.) ps. A total number of 2750±90 (stat.) signal B0 d decays are observed in the measurement, with a fit- ted B0 d mass of 5363.7 ± 1.2 (stat.) MeV. Both the extracted B0 d meson mass and lifetime are within the determined errors consistent with the world average values. Although the achieved precision is still significantly lower than the one of the world average value, the measurement successfully tested the feasibility of the fit-method and allowed to cross-check ATLAS detector performance. We will do that by measuring spectra of surface photovoltage, from which we can get particular parameters of material by their fitting: such as exciton diffusion length, thickness of space charge region (SCR), surface recombination velocity of excited electrons. The dependence shows where material is inducing the highest current density. Some measurements of organic materials enriched with nanoparticles will also take place. Failure to comply with the correct blood practices may affect the safe transfer of blood products. To ensure the safe transfer are therefore important theoretical knowledge of health professionals, who are entitled to the blood products. The result of the test, which I form, clearly speaks for more training in this area. The correct blood practices relating to: before transfusion procedures performed outside of the blood bank (clinical Department), before transfusion examination carried out on blood bank, handling with transfusion preparations for clinical departments. Failure to comply with best practices we will post the patient at risk of post-transfusion reactions. Even in the case of suspicion of post-transfusion reaction are important theoretical knowledge of health professionals, they should know how to proceed. Significance of subclassification of sentinel lymph involvement in breast cancer for following surgical treatment

Summary of histopathological analysis Sentinel lymph node biopsy is relatively new staging method in breast cancer women .Sentinel lymph nodes are assessed by hematoxylin eosin (HE) and by immunohistochemistry (IHC). The reason of the histopathological analysis of sentinel lymph nodes was to compare histological work up by HE with histological work up by IHC and based on this comparisone find if immunohistochemical examination of sentinel lymph nodes brings benefit to the patients or not. The goals of the histopathological analysis were to compare detection of macrometastases, micrometastases and submicrometastases in sentinel lymph nodes using hematoxylin-eosin examination and immunohistochemical examination according to the Czech Pathological Society guidelines. Between January 1st 2006 and December 31 st 2008 there were operated 173 patients with 174 breast cancer tumors at the Department of Surgery Pardubice. Based on size of metastases patients were stratified into groups according the American Joint Commitee on Cancer (AJCC). Totally, 173 patients underwent sentinel lymph node biopsy.
