Multidisciplinary rehabilitation for pubescents patients with cerebral palsy

Cerebral palsy is a non-progressive motor funciton development disorder which is caused by disruption of the immature brain. There is about 16 000 - 20 000 patients living in the Czech Republic with this diagnosis to date. Based on the clinical picture, specific forms of this disorder can be classified. Besides the motor funciton development disorder, the patients also suffer from other comorbidities (such as epilepsy, oropharyngeal dysphagia, sensory deficits etc.). Early diagnosis of this disorder is crucial. The severity of the motor function deficit is evaluated by monitoring the gross and fine motor function. Cerebral palsy is a non-progressive, not unchanging, disorder. The theoretic part of the thesis describes secondary issues which are present during pubescent period of the patients. They are dealing with growth disorders, reduced bone density and increased bone fragility. Plenty of risk factors precede these problems. Also, question of sexuality, life priorities and about future play a big role for the patients in their pubescent period. There are possibilities to solve these problems in terms of complex rehabilitation care. Especially in physiotherapy, occupational therapy and nutritional therapy. Risk factors of osteoporosis and fractures in people over 65 years of age

This diploma thesis deals with risk factors of osteoporosis and fractures among people older than 65 years. Theoretical part contains the definition of osteoporosis, physiology of bones, hormonal regulation, determinants of disease, clinical manifestation, prevention of disease and old age. Essential chapters of theoretical part are about moving activity and nutrition. Aim of this thesis was to evaluate the occurrence of osteoporosis and risk factors of fractures in connection with risk nutritional factors. The research includes 86 responders, who filled out the questionnaire of risk factors and the risk of fractures was found out using FRAX tool. Based on these results patients were divided into 2 groups: with high risk and low risk of fractures. 10 patients of each group provided 3-day menu. The densitometric examination was accomplished to 30 responders and the probability of fracture formation was evaluated. The results of diploma thesis show, that 45 % of responders have higher risk of fracture formation. This group has significantly higher age and lower BMI. This research then showed that patients with higher risk of fracture formation get significantly less energy, proteins, fats, carbohydrates, and calcium. The role of nutrition in the treatment of osteoporosis

Introduction: Osteoporosis is one of the most frequent bone metabolic disease. It is a systemic metabolic skeleton disease, where the architecture of bone tissue is damaged due to the loss of bone mass, and therefore the bone quality is reduced. Thus, it comes to increased bones fragility and reduction in mechanical resistance, which leads to a higher risk of fractures. The principle of pathogenesis is an imbalance between resorption and bone formation. Due to increasing lifetime and lifestyle changes, the prevalence of the disease increases. Aim: The aim of the practical part of the diploma thesis is to evaluate the eating habits of postmenopausal women treated for osteoporosis and to evaluate their nutritional condition in relation to bone mineral density (BMD). Methods: Nutritional habits were obtained and evaluated through a questionnaire survey. Three-day meal records were collected and analysed using the Nutriservis Profesional nutritional software, to determine the energy intake and selected nutrients. Osteodensitometric data were measured using dual emission X-ray absorptiometry (DXA). Problems in prevention of vitamin D and calcium deficiency in geriatrics

Vitamin D and calcium are essential for the health of the elderly population. Due to the changes that occur in the body during aging, it is necessary to monitor the intake of these micronutrients and, if necessary, to supplement or substitute them appropriately. This thesis focuses on prevention of vitamin D and calcium deficiency in old age. The theoretical part explains the function of vitamin D and calcium in the human body, their metabolism, sources and problems of deficiency or excess. There is information about diseases related to the deficiency, especially osteoporosis and sarcopenia. The practical part has two sections. The first analyses the data from the SPRINTT trial. Data related to vitamin D, calcium, their supplementation and sarcopenia, as a disease associated with vitamin D deficiency, were used. The second section analyzes the data from a questionnaire survey in nursing homes, which obtained information about the health of seniors, the frequency of consumption of selected foods and vitamin D and calcium supplementation. The effect of serum 25-hydroxycholekalciferol levels on musculoskeletal system in children with inflammatory bowel disease Background: Low bone mineral density and osteoporosis represent severe secondary complications that can be a result of childhood chronic disease. According to Frost's mechanostat theory impaired muscle functions may contribute to the changes observed on the skeleton. Aims: The aim of this study was to: 1) evaluate parameters of bone mineral density, bone geometry and dynamic muscle functions in children and adolescent with chronic disease - inflammatory bowel disease (IBD) and type 1 diabetes (T1D); 2) evaluate a possible effect of vitamin D deficiency and vitamin D supplementation or duration of the disease on the musculoskeletal unit; 3) determine clinical or laboratory predictors of muscle and bone parameters. Methods: The study was divided into two substudies according to the diagnosis. Seventy patients with IBD (median age 13.8 years) were included in one study, fifty-five of which completed all of the planned procedures. Turčeková, R. The efect of the vitamins in camelids diet. Bachelor thesis. Brno, 2017. The main goal of this thesis is to describe, on the basis of a literary review, the zoological classification and morfological and physiological traits of camelids with a focus on the metabolism of vitamins. Because of a number of physiological adaptations for living in their natural environment it is necessary to provide a higher dose of some vitamins (mainly vitamin D) while keeping camelids in different climate conditions. This thesis subsequently presents all basic vitamins soluble in water and in fat, describes their physiological functions and health issues related to their lack or redundance. Circumscribed specifics of the vitamin metabolism of camelids show a need of knowledge and respect of physiological traits and coherent nutrition needs of species bred out of the region of their natural presence.
Changes in Bone and Mineral Metabolism and the Role of Vitamin D in Very Low Birth Weight Infants

Changes in bone and mineral metabolism and the role of vitamin D in very low birth weight infants. Firstly, the aim of dissertation work was to estimate physiological parathyroid hormone (PTH) levels and their relationship with bone metabolism parameters in otherwise healthy preterm newborns with birth weight 1000-1500 g. Secondly, to evaluate vitamin D status in mothers and their very low birth weight infants (VLBW) at birth and at discharge with currently recommended supplementation of vitamin D. Thirdly, to compare clinical outcomes of VLBW infants with 25-hydroxy vitamin D [25(OH)D] levels ≤ and > 25 nmol/l in umbilical cord blood and finally to evaluate umbilical cord vitamin D as a risk factor for respiratory distress syndrome in preterm infants. It is a set of prospective observational studies involving immature newborns with birth weight below 1500 g. The parameters of mineral and bone metabolism were analysed in umbilical cord blood and newborn serum and urine during hospitalisation (PTH, 25-hydroxy vitamin D, S-Ca, S-P, ALP, U-Ca, U-P) and in pregnant women before delivery (25-hydroxy vitamin D). Serum concentration of vitamin D in patients with femoral neck fracture

Vitamin D significantly affects metabolism of the bone tissue. It keeps homeostasis of calcium and phosphor and thus contributes to healthier mineralization of bones. Lack of those substances pertains to risk factors for development of osteoporosis and subsequent complications during possible downfalls and fractures. The role of vitamin D in bone disease has been increasing in recent years, but its importance in the early post-fracture period is not yet fully understood. This thesis is a prospective study of metabolism of vitamin D and its metabolites in post-operative stage of patients with femoral neck fractures. It is a basic quantitative research, where received data are analysed. Blood samples obtained from a total of 18 patients, whose blood was taken on the day of surgery and 6 weeks after surgery (± 1 week), were used as material. Resulted values of 25-OH vitamin D increased from 23,02 ± 16,02 nmol/l to 25,75 ± 16,30 nmol/l (diameter ± standard deviation). In contrast, 1,25-(OH)2 vitamin D decreased from 76,81 ± 41,84 pmol/l to 55,88 ± 26,34 pmol/l. Statistically significant results at a significance level of p≤0,05 were only observed for parathyroid hormone (PTH), with values decreasing from 6,31 ± 3,95 pmol/l to 3,96 ± 2,25 pmol/l. Nutritional profile in patients with bariatric metabolic surgery

Introduction: Obesity is a major health problem that affects an individual's overall health. Bariatric metabolic surgery is most important and has permanent impact on weight loss in comparison with conservative therapy. The downside is that it often leads to a row nutritional deficiencies requiring long-term supplementation. Object: The aim of the thesis is to map the intake of nutrients in the diet of bariatric patients six months and one year after the procedure. The research is primarily focused on the intake of vitamin D and calcium in diet, vitamin D, parathyroid hormone and calcium levels are also evaluated. Marginally, laboratory values are assessed prior to surgery as deficiencies are known to occur before surgery. Methodology: The research sample consists of 30 respondents who are six months (M6) and one year (Y1) from the procedure. Micronutrients are evaluated from three-day dining records recorded by respondents for at least one week. The research laboratory data was used from the medical information system of the hospital information system. The evaluation parameters for vitamin D-25 (OH)D (limit level was established to > 30 ng/ml).
