Author: Lenka Fialová Title: A case study of physiotherapeutic care of a patient after knee arthroscopy Objectives: The aim of this bachelor thesis is to provide comprehensive information about knee joint their injuries, examinations, treatment and rehabilitation procedure. The aim is also to document the physiotherapy care of the patient after knee atroscopy for anterior cruciate ligament lesion, cartilage defect and plica syndrome. Methods: In the general part of the thesis I summarize the theoretical knowledge concerning the knee joint, including anatomy and biomechanics, mechanism of injury and treatment of some injuries of this joint. Furthermore, I focus on the possibilities of clinical examination and subsequent rehabilitation. In a special part of the thesis I present a case report of a patient after ASK of the knee joint with cartilage and ACL lesion, and plica syndrome. This section contains a description of 8 therapeutic units including initial and output kinesiological analysis. All literature used is listed at the end of this paper. Results: In the general section, information regarding the knee joint was summarized. In the practical part a case report of physiotherapeutic care of a patient with a relevant diagnosis is then presented. Methods: This bachelor thesis is primarily divided into two parts, a theoretical part and a special part. The theoretical part deals with functional anatomy and biomechanics of the knee joint, arthroscopy of the knee joint, examination methods and procedures specifically focused on the given segment including special tests, the final part consists of the theoretical basis of therapeutic methods used in this thesis. The special part consists of a case report of a patient with whom I worked during my continuous practice at the Homolka Hospital. iv Abstract Author: Adam Viktora Supervisor: PhDr. Lenka Žáková, Ph.D. Title of the thesis: Case study of physiotherapy treatment of a patient after Total Knee Replacement Objectives: The goal of this thesis is to gather and understand the theoretical knowledge of the knee joint, osteoarthritis, total knee replacement surgery and its rehabilitation, and then evaluate the therapeutic sessions that were applied. Clinical findings: This study evaluated the condition of a 73-year-old patient after total knee replacement of the right knee. The examination showed a reduced range of motion, reduced mobility of joints, muscle imbalances and differences in conditions of soft tissues. Methods: All methods used for this study were based on the knowledge learned and the literature provided by the Faculty of Physical Education and Sports, Charles University. Results: Following the eight therapeutic sessions, the patient had progressed well overall. Progress was seen in terms of pain, range of motion, muscle imbalances and condition of soft tissues. Conclusion: The therapies applied were effective and valuable in this specific diagnosis of the patient. The theoretical part also consists of information about the ACL ligament reconstruction and recovery after the reconstruction. The second part of the thesis is written based on a physiotherapeutic methods used during the rehabilitation process including initial kinesiological examination, day to day therapy, and final kinesiological examination, and evaluation of the therapy process to understand the therapy effect on patient. Methods: All methods used were noninvasive. The information in the theoretical and practical part of the thesis are based on knowledge gained in Faculty of Physical Education and Sports in Charles University. Each physiotherapeutic method used was discussed with the supervisor at Medicentrum Praha to ensure patient's well-being. Possibilities of physiotherapy after knee arthroscopy

This bachelors thesis talks about the possibilities of physiotherapy after knee arthroscopy. The thesis is divided into many parts. The first theoretical part summarises important information about the anatomy, kinesiology and biomechanics of the knee joint. Then the mechanisms of knee injuries are described with their following arthroscopy treatment. The theoretical part ends with the list of possibilities of physiotherapy after knee arthroscopy where specific methods and concepts are explained. The research questions and goals of this thesis follow in next chapter. The first goal is to propose treatment and rehabilitation plan after knee arthroscopy. The second and the main goal is to focus on the problem of knee instability connected with this surgery. The third goal is completed by creating an information brochure with adequate exercises after knee arthroscopy. For my thesis I picked qualitative research. The research group was formed by 3 women between 35 and 58 years which underwent knee arthroscopy surgery. The reseach was done by regular meetings for 2 months. The methods of obtaining data were entrance and output kinesiology analysis. The results are presented in the form of case reports. The case reports contain the progress of therapy, short-term and long-term rehabilitation plan and the summary of all therapies. The final chapter contains evaluation of the research in the form of discussion where I list the strong and weak sides of this research. This thesis can be used in physiotherapy, by the public or patients after knee arthroscopy.
Pressure pain threshold evaluation in relation to the knee joint position - pilot study

The diploma thesis deals with the relationship between the level of stress of soft tissues and their pressure pain sensitivity. It aims to find out whether the level of tension in soft tissues, given by the position of neighbouring joints, affects the pressure pain threshold. The thesis assumptions were verified on a model situation with the sensitivity of the tissues of the ventral and dorsal sides of the thigh depending on the angular adjustment of the knee joint flexion. The theoretical part deals with the findings about biomechanical properties of soft tissues in relation to mechanical load and in the sense of tensile and compressive forces. It describes the mechanisms of tissue nociceptor activation and afferent signal transmission for processing by neurvous system. It distinguishes between the concepts of nociception and pain and deals with methods of their quantification, especially pressure algometry. A group of 30 (15 men and 15 women) healthy probands of younger adult age participated in the research. The values of the pressure pain threshold were measured for each person with an algometer from the ventral and dorsal side of the thigh with angular settings of 0ř, 45ř and 90ř flexion of the knee joint, with a constant position of the hip joint. Methodical procedures and comparison of various techniques of taping in the area of the knee joint

This bachelor thesis has theoretical and didactic character and it concerns with methodical procedures of taping in the area of a knee joint. Analytical part was made on basis of analysis of scientific literature, and it contains detail description of anatomy, kinesiology, and the most frequent injuries of the knee joint. In the thesis there is also described method of taping itself, division of the taping tapes on flex and stiff tapes. Next there are described concrete methods of taping, functionality, and principles of correct taping. Crucial part of this thesis is synthetic part. There are described and compared methodical procedures of taping in a knee joint. Compared methods were complete stabilization, lateral ligaments, patella and taping of injured ACL. Part of the synthetic part is photodocumentary of the procedures with stiff and flex tapes with detail description of the procedures. There is also comparison of the taping methods. The final result could serve as a tool for sport trainers or individuals, who are devoted to regeneration or to sport itself.
Physiotherapy in patients before intended knee joint replacement within the concept of prehabilitation

This bachelor thesis deals with the concept of prehabilitation and its use in orthopedic surgery, specifically in total knee replacement, from the perspective of physiotherapy. Osteoarthritis is the most common degenerative disease of the knee joint, with total knee artrhroplasty being the standard procedure in the treatment of osteoarthritis in the late phase of the disease. In the theoretical part, this work is devoted to an overview of existing knowledge about the concept of prehabilitation in planned operations and current methods in treating gonarthrosis and the possibilities of physiotherapeutic intervention in the preoperative and postoperative period. This work aims to record the preparation and implementation of the physiotherapeutic intervention in a group of patients before the planned knee replacement and then evaluate its impact on the patient's condition. The research group consisted of 2 patients with advanced gonarthrosis who underwent therapy in the period of 7 weeks before the planned operation aimed at strengthening weakened muscle groups, increasing functional capacity, and raising awareness of the upcoming operation with subsequent rehabilitation. During the evaluation of the research, changes in the musculoskeletal system were found, especially the stability and strength of the lower limbs, the strength of the upper limbs, and the position of the pelvis. There was a positive effect on movement stereotypes. Both patients had a reduction in stress from the upcoming operation. The work can serve primarily as a comprehensive overview of the care of patients undergoing total knee arthroplasty for both the professional public and for patients or future physiotherapy students.
Design of a set of targeted exercises aimed at compensating for anterior cruciate ligament injury

Bachelor's thesis, where the goal is to create a set of compensation exercises leading to the elimination of the consequences of injuries to the anterior cruciate ligaments. The analytical part of the thesis is devoted to the anatomy and kinesiology of the knee joint, further in this part we describe the injury of the fibrous apparatus and the principles of compensatory exercises. In the synthetic part, we are based on the findings that lead to the creation of the program, including detailed relaxation, stretching and strengthening exercises. All the exercises included in the created compensation program lead to an improvement in the state of health and eventually to a return to normal functioning within the framework of physical condition. The exercise program can be used not only by individuals from the ranks of amateur or elite athletes, but also by the general population.

This work was created as a part of a bachelor's practice, which was completed at the Rehabilitation Hospital Beroun. Part of the work is also a theoretical part, where the general issue is discussed. Methods: All of the methods used in the therapy are based on a bachelor's degree in FTVS.
