Readiness of the emergency medical service for treating patients contaminated by radiological agens

The aim of my thesis was to find out the readiness of the South Bohemian Emergency Medical Service to deal with a potential extraordinary event of radiological nature. The content of the bachelor thesis is to analyze professional, material and technical readiness. The theoretical part deals with characteristics of integrated rescue system, exceptional events, ionising radiation and, last but not least, pre-hospital urgent care for irradiated persons. The practical part of the bachelor's thesis investigated the level of rediness of the Emergency Medical Service of the South Bohemian Region both in professional and material and technical level. There were two hypotheses in the thesis, H1: "Medical rescuers have knowledge how to treat patients contaminated with radiological agens," and H2: "The emergency medical service has material equipment for the treatment of patients contaminated with radiological agents." In order to achieve defined goals and to verify hypotheses, a questionnaire was put together and a survey was carried out. A statistical investigation was made using descriptive and mathematical statistics. The questionnaire consisted of closed questions. It contained 11 informative questions, 9 questions focused on problematics of treating patients contaminated with radiological agens, and 4 questions focused on the material and technical equipment of the medical rescue service. Respondents' questions were addressed with 3-5 possible answers, only one of which was correct. Data was collected during March 2017. Respondents were members of medical staff of the South Bohemian Emergency Medical Service. Sixty anonymous questionnaires were distributed in the South Bohemian Region, more precisely in the territorial centre České Budějovice and the regional resorts Jindřichův Hradec, Český Krumlov and Tábor. Total of 42 respondents participated in the survey. The results of the survey show that the knowledge of paramedic in this area is below average. The arithmetic mean of knowledge was 5.79 points out of 9, for material and technical equipment it was 1.79 points out of 2. The results of the survey show that the goals of the thesis were fulfilled, one (it intended hypotheses H1: Medical rescuers have knowledge how to treat patients contaminated with radiological agens) hypotheses was rejected and one (it inteded hypotheses H2: The emergency medical service has material equipment for the treatment of patients contaminated with radiological agents) accepted. The benefit of the bachelor thesis is mainly the preparation of a basic comprehensive overview of the readiness and materiál and technical support of the South Bohemian Emergency Medical Service for the treatment of patients contaminated with radiological agents.
Prague Emergency Medical Services Preparedness for Medical Intervention During Chemical Threat in Metro

The goal of this theses is to analyse the preparedness of Prague Emergency Medical Services (hereinafter Prague EMS) for chemical attack in subway (metro). The theoretical part states general information about chemical terrorism and the Integrated Rescue System's solution. The research part focuses on procedures following from Integrated Rescue System Common Type Activity during joint action - Reaction on chemical attack in subway (hereinafter STČ 13/IZS). This document was subjected to content analysis to meet the goals. The work then focuses on the evaluation of Prague EMS training Metro 2014 whose goal was to practice the procedures and joint deployment of Integrated Rescues System members and other subject according to STČ 13/IZS. For the evaluation complexity, the results were supplemented by SWOT analysis which pointed at another usable knowledge. All conclusions are clearly organized in charts and they are efficiently worked with to set new measures and recommendations which would increase the preparedness of Prague EMS for chemical attack in metro. The goal of this work to assess the level of preparedness and to submit final protocol to organisation's crisis management was fulfilled. The main benefit of this work is a recommendation for the reinforcement of crisis preparedness at Prague EMS. Furthermore the work brings new expert findings to solved problematics.
Prehospital emergency care in neurotraumas

The Bachelor thesis is focused on the topic of Prehospital emergency care in neurotraumas, especially on the approach of paramedics and their knowledge about Prehospital emergency care in neurotraumas. Globally, for children and adults to 45 aged, neurotrauma could be placed in the highest position of death causes. This statistic testifies about the importance of the professional and early intervention in prehospital emergency care. In the thesis qualitative research was used and two purposes and questions were defined. The method used was a semi-structured interview with paramedics in Vysočina Region and South Bohemian Region. The first aim of the thesis was to chart knowledge and approaches of paramedics in Prehospital emergency care and the second aim was comparison of the previous aim in Regions mentioned above. From the outcome of my research results that paramedics are not qualified enough and they lack information about Prehospital emergency care. In interviews, information mentioned was incomplete and their approaches were inconclusive. However, it was detected that approaches used in both regions are nearly identical. In Vysočina Region, cooperation with workplaces of the follow-up hospital care is really difficult. This fact could be considered as a side result of the thesis. The workplaces mentioned are mainly traumacentres with neurosurgery workplace accessible 24 hours per day. To sum it up for the further use in practice, the educational system of paramedics is inadequate and improvement would be highly recommended.
Protecting the Health Operations Centre of Emergency Medical Services Pilsen Region as a Critical Infrastructure Element

The aim of the bachelor thesis is to assess the conservation status of the critical infrastructure element, in this case medical operations centre of the Emergency Medical Service of the Pilsner region. Firstly, the work deals with situations of the imminent danger of the building on the basis of emergency or crisis situations, which are based on the risk analysis. This risk analysis is a part of the Emergency Plan of the Pilsen Region, which has been prepared by the Fire Brigade of the Pilsner region. On the basis of local investigations there are evaluated some current measures in the context of already existing documents, namely the processed crises preparedness plan. These most probable "classic risks" are then further elaborated including evacuation as a typical measure for some of them. In the next section this Bachelor thesis has focused on the "new threats", mainly on a violation of a cyber security, with an assessment of the current state of backup systems and their own security, storage and the backup of the electronic data. Analyzing the current hardware and software of the operating medical centres and related infrastructure network, there are identified configurations of some individual elements, their relationships and especially their security. Some remedial measures are suggested in case of the unsatisfactory situation. The benefit of this work is the inclusion of results of the investigation related to cyber security in the current crisis preparedness plan, which has not included this issue yet. The results and conclusions of this bachelor thesis are therefore used to improve crisis preparedness of Emergency Medical Services of Pilsen region in terms of the protection of the critical infrastructure element.
One instead of six: does the centralization of Emergency Medical Dispatch affect the quality of care?

This thesis engages in its theoretical part description of the process of pursuance and assurance of quality in emergency medical dispatch centre. There is also a comprehensive description of key quality parameters used in this environment in this part. The content of the empiric section is an organizational case study, which aim was to evaluate an extensive organizational change in emergency medical dispatch centre of the Emergency Medical Service of the Central Bohemian region and its impacts on the quality of care. The first phase of centralization of emergency medical dispatch centres in the Central Bohemian region went through during the first half of 2011. Four local emergency medical dispatch centres were gradually brought together into one entity. The aim of the research was to evaluate impacts of the organizational change on key quality parameters. The observational retrospective study consists of two groups of data, collected from identical area during two equivalent periods. Group A contains data from the period of six months before and group B contains data from the period of six months after the centralization. Technical conditions for imlplementation of the EU standard at the Emergency service

In 2010, a decree of the Ministry of Health of the Czech Republic entered into force, which adapts among others also requirements for equipment of medical service vehicles. The European standard was thus implemented in the Czech legal regulations. The thesis describes problems associated with application of the standard in vehicle production, the verification system and documentation of conformity of the vehicle construction, and also documentation of equipment of vehicles with medical devices, in accordance with the standard. The thesis also evaluates the benefit in respect of patient safety and quality of care in the system of medical rescue service.
Cardiopulmonary resuscitation and the use of special instruments

The bachelor thesis analyzes cardiopulmonary resuscitation and the use of special equipment related with cardiopulmonary resuscitation in pre hospital emergency care. The main study introduces practical application of special equipment and expertise experience of paramedics and nurses with use of this equipment.Theoretical section consists of two parts definition of using special instruments and description of cardiopulmonary resuscitation. The first one describes specified equipment related to cardiopulmonary resuscitation in pre hospital emergency care and legislative standards specified for operation of emergency services in Czech Republic. The second deals with the issues of cardiopulmonary resuscitation. Basic and advanced methods of urgent cardiopulmonary resuscitation in pre hospital care and updates of Guidelines from the years 2010 and 2015 related to cardiopulmonary resuscitation are also introduced here.The practical part contains transcribed interviews with professionals, which were obtained by qualitative semi-structured interviews. Paramedics, nurses and a physician at Emergency. Medical Services in South Bohemia were selected as interviewees. Altogether, six people were contacted. Interviews contained 15 questions related to the issues of practical application, interpretation of values and personal experience with special technology used in urgent emergency resuscitation.The study of this thesis shows that health care workers do not have a problem with the interpretation of data available thanks to this technology. In some cases, there is a problem with the proper use of technology or differences of opinions on the application itself among workers from different ambulance stations. It was also found that no one is happy with trainings and the amount of information provided by an employer, because all respondents look up information concerning the technology beyond their job duties. As a contribution of this work I see a brief equipment overview non medical staff use during cardiopulmonary resuscitation. The thesis summarizes everything from the description of basic values to actual applications of the equipment.
Nursing staff in relation to professional supervision of Emergency medical services of South Bohemian region.

This thesis deals with the differences in the supervision provided for Emergency medical Services in the South Bohemian Region. For the purposes of a research this thesis is closely focused only on Emergency Medical Services in Czech Budejovice, Pisek, Czech Krumlov and Milevsko. It is designed primarily as a reminder of the wide diversity of different approaches to professional supervision and as an effort to stop the deepening of this diversity, not only in the South Bohemian Region. The theoretical part is divided into two main sections. The first one pays attention to the importance of professional supervision and the reasons why it is such an important part of the paramedic profession. The second part deals mainly with the possibilities and innovations that are possible within the professional supervision and which are already carried out in some places, especially abroad. The practical part is made on the basis of qualitative research using the technique of semi-structured interviews. This part is completed by a short knowledge test. The sample of respondents includes paramedics from Emergency Medical Service in Czech Budejovice, Pisek, Czech Krumlov and Milevsko. At each workplace respondents were chosen among both men and women all regardless of age or achieved education. The answers of the fresh graduates were the most interesting because they have got the most topical experiences with the professional supervision. The results of the research showed that all respondents evaluate the provision of professional supervision to be helpful in the course of obtaining assurance to practise the activities which are paramedic´s competences, both in activities that are performed with indications of a doctor and in those that can be performed without indications of a doctor. Despite the obvious benefits of performing of professional supervision the members of the Emergency Medical Services in the South Bohemian Region do not have sufficient theoretical knowledge about professional supervision. Most respondents associate professional supervision simply with supervision and training that they were or have been provided for a period of their starting practice. It was evaluated also on the basis of the research that professional supervision is performed differently in the context of Emergency Medical Services in the South Bohemian Region. The respondents classified the application of professional supervision as absolutely insufficient at some ambulance stations on the contrary to ambulance stations whose provided professional supervision is evaluated very positively. In conclusion it must be noted that professional supervision is a very important part of professional life of paramedics and nurses working in emergency medical services therefore we should pay an appropriate attention to this subject.
Issue of the Emergency Medical Service help to homeless people.

This bachelor thesis deal with homeless issue from the view of emergency service worker. The theoretical part deals with the pre-hospital emergency care, with the related ethic and homelessness, as extreme form of social exclusion. Furthermore, the theoretical work is describing the health status of homeless people and their relationship with medical personnel. Practical part of thesis is qualitative research which had been done by semistructured interview of the emergency workers. This research had lasted since december of 2014 untill march of 2015. Interviewee were question by snowball sampling method. There were ten people questioned altogether. The transcription of interview is quoted in practical part. These interview are summarise in different charts partly for better clarity. In suplement part there is a list of questions which interviewee received. The aim of research is explored intervention of emergency service to help to homeless people and in which areas emergency service workers see special treatment to them. From interview have emerged that the most often interventions to homeless people are alcohol intoxication and different kind of injuries. Research has also revealed that emergency service workers are not stressed when they help them but all of those people agree with statement that it is unpleasant especially due to low hygienic standarts. Is also very interesting that emergency service workers meet homeless people at least once a month. The main point of this bachelor thesis is demonstrate the homeless issue which is getting more serious in these days. It has not have solution only in providing social welfare to homeless people. It is necessary to create the complex solution of this phenomenon which would include also health care to these people.
Paramedics opinions on their competencies

The bachelor's thesis name is: Opinions of paramedics about their competencies. The focus is therefore on problematics of competencies which paramedics at present have, and also their views on possible extension. In theoretical part of the thesis an integrated rescue system, the part of which the emergency medical service also is. The history, activities and also providers of emergency medical service are outlined. Further it deals with distribution and types of rescue groups, and technical and material equipment of vehicles. Finally, competencies are mentioned which at present a paramedic has, and also specialisations in urgent medicine, through which the competencies extend. In practical part of the thesis a qualitative method of research was used. Data collection was performed with help of semi-standardized interviews with eight paramedics, who work at Medical Rescue Service of Southern Bohemia Region. A part of the research maps knowledge of competencies in the field of paramedic and to find out opinions of their prospective extension. Also cooperation with doctor on each ambulance trip is discussed. The research file was subjected to the length of practise which the respondents went through within their occupation. The research question was directed at particular prospects which paramedics would welcome compared to the current state. The aim of the thesis was to determine what the opinion of paramedics on their current competencies within their occupation is. A partial aim then was which opinions they hold on extension of their competencies. It was revealed from the obtained and processed interviews that paramedics are mostly content with their current possibilities. An extension of competencies from their point of view would concern such parts at which the least risk of threat to patient, or incidental complications during treatment in prehospital urgent care are posed. Opinions on extension of competencies leaned mainly on bigger responsibility which in case of extensions supervened.
